Yes, he is free from the blame because he made it clear but she out of selfish needs decided that she was gonna get pregnant for him. A man will always be a man. Only a woman will know of her own body and not her man. A man would just assume at all times but its all up to the woman to decide what is best for her. Same thing applies to a guy. If the guy is doubting his gf's personal hygiene, by all means he should wear a cap before doing it. He and he alone is responsible for his own body and the fault should not be taken out on the other person.

I have one good example, when I first started having s3x, I wanted it so badly and my bf refused to use condoms but I made it clear to him that I am very fertile and although Im on the pregnancy pills, I would really want him to use a condom or else no sex. Its because I know my body well wnough to say that. To him, he would just assume cause he doesnt own my body, I do.
