Thanks Ajolayo. That is an enlightening contribution, however in science (medicine) im sure you agree that common things occur commonly, so when an adolescent becomes diabetic i will probe in the direction of type 1 DM so while type 2 has a strong association with obesity and family history, it is still predominantly not a disease that appears in teenage and adolescent years, youd find hundreds of adolescent with type 1 before you can come across one with type 2.

what exactly do you feel is not well with you? you probably will do well with someone you can talk to, not everything is a physical malady that can be diagnosed and treated as such, if you are unaware of any physical symptoms then you will do well to talk to someone, preferably a psychologist but really anyone who you trust you can relate to will do, it helps to identify what the problem is but you need to identify it first before you can deal with it, without which no doctor can offer any remedy or suggestion.

staph does not break out in painful, blisters and ordinary lab evaluation of HVS will not show a herpes infection. She needs to see a doctor for treatment and advise. Herpes has no cure but there are treatments to keep the disease more or less quiescent.

Aside the fact that herpes may be complicated by a staph infection, there is no association between the two. Genital Herpes is a sexually transmitted intection caused by herpes simplex, whereas staphylococcus is a very, very, very rare cause of bacterial vaginosis, in females it is associated with toxic shock symdrome, mostly in women who uses tampoons during their periods.

my friend has recently been hospitalized because she drank about a quart of antifreeze. she is currently on life support, the Dr.s say that she is brain dead!, my questions are:

If she is brain dead:

1. can she hear me?

2. she slowly opens and closes her eyes when we speak to her, is this normal for someone who is brain dead?

3. when i speak to her her heart rate increases, does this mean she understands me?

4. is it possible for her to just be in a coma, and not be brain dead?

5. what effect does antifreeze have on the body?

please doctor, i need some answers for my own sanity, it has been a really hard time for everyone and i need some insight please!!

What an unfortunate incident! If she drank an antifreeze and she is brain dead then she can neither hear you nor respond to you.

Now when she opens and closes her eyes you have to understand that someone that is brain dead has lost all higher cortical (cerebral) function, however may still retain some lower (brainstem) function the fact that she is even on life support means that she has also lost a considerable amount of her brainstem function too.

her heart rate racing has nothing to do with you talking to her depending on how much and what type of neural function has been damaged, you could have anything between slowing down and racing of the heart. Also don't forget that other things play a role here too and not just the brain, her body fuild and electrolytes, for example.

Now is it possible that she is just in a coma and not brain dead? YES! but then i cannot tell you that from where i am sitting, you will have to ask the doctors managing here to be sure of that. If she is brain dead, i am sorry her condition is only reversible by God, a coma is still reversible (but how long has she been in this state?)

Well to start with, one of the components of antifreeze, METHANOL, is notorious for causing BLINDNESS almost always, that is almost 100 out of 100 people that ingest it will go blind (that is if the patient is still alive)

Moreso, it would be an understatement to say antifreeze is dangerous, IT IS A KILLER, a teaspoon of it is enough to kill anybody. other components of antifreeze, like ETHYLYNE GLYCOL, which in itself is potentially non toxic (IF THE PATIENT GETS TO THE HOSPITAL ON TIME AND AN ANTIDOTE GIVEN ON TIME, BUT IN MOST CASES ITS TOO LATE BY THE TIME THE PATIENT GETS ENOUGH HELP TO COME TO THE HOSPITAL, treatment of which is basically ethanol with hemodialysis to get rid of it from the body) but is converted to a toxic metabolite in the body which can cause RENAL FAILURE, ACIDOSIS (too much acid in the body) and HYPOCALCEMIA, which basically means low calcium in the body, this will result in muscle cramps and overactive reflexes, which could be an explanation for the opening of the eyes which you noticed, which then only points to the fact that we are dealing with a "hopeless" situation, redeemable only by the creator Himself.

Now no woman can be said to have a constant length of menstrual cycle her entire time, it will always vary. Also one of the reason we give a range for what is "supposedly" your safe period (for those wishing to use it for contraception) and your fertile period (for those wishing to use it for conception purpose) is because even the date we calculate is in retrospect, that going by your last one to three cycle length, but there is absolutely no guarantee that your next cycle length is going to be so, that is why your next ovulation date CANNOT be a fixed date, but a range, but we can tell you your ovulate date of your last cycle but the next one, complex but that defines the line between us and God.

Also the male seed can survive anytime between 12hrs and 5days in a woman, how long the next ejaculate is going to survive in you, no one can guarantee that also. Another reason why we must need give a range and another reason the natural mode of contraception has a very high failure rate, second only to coitus interruptus.

So for a cycle length of 25 to 30 days, your "fertile" or likely ovulation period can be anytime between the 12th and the 18th day from the START of your next bleed (im sure you can do the counting yourself) we usually work with +/- 2 or (3) days (opinion of whether to use 2 or 3 days varies with different centers) but the basic principle remains the same.

There are basically three types of headaches (MIGRAINE, CLUSTER AND TENSION) that occurs on its own, that is when it is not associated with any other conditions like an intracranial malady or a systemic problem.

In your case it seems like tension headache, and sometimes it persists after the precipitant may seem to have gone away because it has been postulated that it might still be present it the subconscious. try outdoor activities, try to confront whatever the disharmony is and solve it as best and as crises free as you possibly can, friends and family support helps and take some paracetamol as needed, if it gets too painful you may try the NSAIDS like ibuprofen 400mg three to four times a day but not for long, please.

A dose or two of valium at night might also help, say about 5mg or 10mg for about 2 or 3 days.

You'll feel better in no time and take care.

This is an online forum and there should be a great restricition to the prescription of medications for maladies but i suggest you try ZYRTEC 10mg once per day with the earliest onset of your catarrh or symptoms as there might be an allergic component to your condition. Discontinue the medication as soon as the symptoms abate.

Painless bumps maybe a sign of salient STI e.g syphilis, especially if it appears and disappears, i advise you see a general practitioner for an examination

on the other hand it may be a non specific occurence, so as i said seeing a physician will help alleviate your concern.

Ulcer simply put is a breach in the epithelial continuity of any part of the body surface, so "gut" ulcer as you put it is a condition where the protective lining of the "gut" is being constantly eroded by the gastric (stomach) acid content and other factors.

The ulcer can be anywhere from the oesophagus to the first part of the duodenum as this marks the "jurisdiction" of the gastric acid. So the ulcer is commonly known as PEPTIC ULCER DISEASE, and can be a GASTROESOPHAGEAL REFLUX DISEASE, GASTRIC ULCER DISEASE or DUODENAL ULCER DISEASE, depending on the part of gut affected.

The three have similar symptoms, which is essentially a "burning" or "gnawing" pain in the gut which is INTERMITTENT but quite different presentations classical to each:

Gastric ulcers pain will generally wake the patient from sleep and aggravated by food and commoner amongst people of blood group type A while that of duodenum will be relieved by food and commoner in people of blood group type O. An oesophageal reflux one tends to give the patient a feeling of shortness of breath as the acid content spills into the chest, in all though there is considerable overlap between all three presentations in real life and physicians need to be meticulous in diagnosing which is which.

As for the treatment it could be both MEDICAL (which would include lifestyle modifications) and SURGICAL (in severe cases)

The mainstay usually is the patient's lifestyle modifications:

1. They should lose weight if obese
2. Avoid tight fitting belts
3. Reduce (by about 60%) intake of coffee, pepper, spice, chocolate, cigarette, milk (now while milk can be used ro relieve the pain of peptic ulce when its on, overdependence on it daily use as a meal could also provoke it, and the same goes for Yoghurts)
4. Elevation of head of bed while sleeping
5. Eating night time meals at least 2hrs before sleep.
6. Discontinue any strong pain killers (especially the Non steroidal antiinflammatory drugs eg. Ibuprofen) and other drugs known to disrupt the protective linning of the "gut" EXCEPT WHEN ABSOLUTELY NECESSARY if an alternative cant be sought and in such cases STRICT DOSAGE COMPLIANCE IS ADVISED AND SUCH MEDICATIONS TAKEN WITH FOOD.

With all that in place i usually start my patients on a two week course of antacids (usually maximal recommendation is six weeks but experience have shown that combined with a religious observance of lifestyle modifications the ulcer heals quickly) however some patients may require the full six weeks course for full symptoms remission

There are a number of antacids in the market and the choice of which to use also depends of the severity of symtoms:

MMT solution and tablets (chewable forms are present in the market) and patients can take up to 2 tabs (teaspoon as the as the case may be) four times a day.

The H2-blockers (cimetidine, ranitidine, famotidine) commonly known as Zantac 150mg twice a day, which eliminates about 70% of the acid

And the proton pump inhibitiors; omeprazole 20 or 30mg (depending on the formulation) once daily (preferably at night), this eliminate about 95% of the acid.

There are others that will need your physicians recommendation to use it but generally the above are the mainstay of treatment.

If it is found that the ulcer is complicated by a bacteria known as H-Pylori in cases which patients do not heal despite adequate and full dose of antacids with good compliance then antibiotics will need be added, this need be prescribed again by your physician as the dose and combination that eradicates the H-Pylori is different from the everyday (ab)use of antibiotics.

Surgery is reserved for only the severe recalcitrant ones with frequent relapses.

Remain healthy.

Simply put Seun, the answer is yes and honestly there is nothing to it, its just a minimum of 6 years in an accredited medical school to become one (in my own case i graduated from what used to be known as the best medical school in Nigeria, so your guess is as good as mine) and then years of toiling in a residency program if you want to be accorded any form of respect in the field.
