Hi doc,
Really hope you can help. I'm a 34 years old mother of two but I'm not always interested in Sex, it has caused a lot of fight btw my husby and I, he has accused me of having it outside and has also threatened to divorce me if I don't change but I just don't have the urge, is there any drug I can take.
Thanks.
Hi,

Sorry about your experience. You are only one of many women and men that have what is considered too low sexual drive.

The greatest sex organ is the brain/mind. A psychotherapy will greatly help.

Specific treatments for low sex drive are not common in orthodox medicine, except when they are meant to correct obvious hormonal deficiences.

I will do some study in the coming days to see if there are helpful and safe drugs you may try.

All the best.

Hello Drs in d house weldone.
Pls help me
I've been having signs of fever lately..bittermouth,cold,weakness all over & nausea,so I went for MP/Widal tests.here is d result.
Malaria parasite(+)Ring form of Plasmodium Falciparum seen.
Widal reaction report:Salmonella Groups AO,AH,BO,BH,CO,CH,DO,DH Titres all less than 20.
Titre greater than 20 is significant.
Pls help interprete before I see a Dr next week.Note:I'm 7 weeks pregnant and feeling like fainting/vomiting.am really worried cos I learnt malaria drugs are not to b administered in early pregnancy.
Hi,

First, note that your pregnancy status may explain these symptoms you have: weakness, dizziness, vomitting/nausea, bitter mouth so it might not be a true "sickness". You will adapt with time.

MP (+) means mild parasitaemia, or mild 'malaria'.

There is no evidence of typhoid fever according to the widal test.

You should have done PCV (blood level test) to rule out anaemia (shortage of blood)

Treatment of malaria in early pregnancy: Quinine (in normal or low dose) is the drug traditionally recommended. 300mg or 450mg 3 times a day for 3 days will be ok.

Some newer studies support the use of ACT such as Coartem/Lonart, but this is not conclusive yet. However, in practice, Coartem/Lonart is more commonly used in most antenatal clinics in Nigeria with no reported problems.

Do you have fever, like hotness of the body/increased body temperature?

If you are not seriously ill, you should wait till you see your doctors this week. If you are, you may see them at once.

All the best.

They look beautifully simple.

Look at the simple-looking sandals or slipper he puts on. That's admirable.

Unlike some people who would choose to wear the whole gold in the world.

Dear Dr,

I had an Okada accident recently and sustained injury on my left leg, stitches were carried out fine and the wound is healing no much pain.
but one thing that got me thinking every time is when my friends/family visited they told me jokingly stop having sex for now for the wound to heal quick.
Please how true is this medically because me i can stay long like that?.
also when in the office i feel cold alot than any one else what s the cause of this cold and me.

best regards
Hi,

There is no corelation between wound healing and having sex. Biochemically and physiologically, having sex will not interfere with your wound.

We have different thresholds when it comes to cold or heat tolerance. In other words, people feel cold or hot differently.

If yours is exceptionally bad, ie if you cannot tolerate cold at all, then it may be a sign of low thyroid hormone which can be caused by several factors too. A simple blood test will comfirm this.

All the best.

okolola25:
Doctor, please how can Pus cells and agglutination be treated in this case?
The pus cells are a sign of infection or contamination. To comfirm this and treat, you need a culture test of the semen.

Agglutination is thought to be caused by presence of sperm antibodies. There is no specific treatment. Some studies support the use of antioxidants to manage it. Commonest antioxidants are vitaminC and E.

There are also much antioxidants in fruits and vegetables. Somewhat, it boils down to the healthy diet and exercise we generally advise.

A urologist/gynaecologist/fertility expert will advise you better though.

All the best.
HealthRe: Doctor In The House:Obstetrics And Gynecology by omicron(m): 8:07pm On May 29, 2015
okolola25:
Good evening Doctor, kindly help me interprete this semen analysis.
Normal count

Normal motility

Normal Morphology

Presence of agglutination is a bad feature. It does not expressly translate to male infertility but can be a significant factor.

Semen also has high pus cells. Will require culture.

It is an essentially normal semen analysis, except the presence of agglutination.

Good morning doc pls I want you to help me interpret this result

volume. 5ml
Viscosity. present
appearance. white opaque
constitency. viscous
liquidfaction. liquified within an hour
PH. 8
semen count. 49 mill
semen motility. 1hr to 2hr
progressively motile. 50 to 60%
sluggishly motile 20 to 25%
non motile. 20 to 25%
abnormal form. 10%
sperm agglutination present
pus cells. 4-5/hpf
R.P.C absent
epithelial cells 1-2/hpf
Sperm count is Normal.

Liquefaction time is normal.

The sperm motility is not completely or correctly reported. Read again, and comfirm.

The pus cells are significant (a sign of infection/contamination). Requires a culture therefore.

when I went to the hospt, the Dr placed me on Primonult N, it came, but after that, I didn't see anytin again.
Ok

Well, many things could have caused this, ranging from problems in the uterus, ovaries and the structures in the head that control the reproductive system: pituitary gland and hypothalamus.

The fact that you menstruated with the aid of Primolut N (or oral contraceptives) proves that the problem is not from the womb (thereby ruling out Asherman Syndrome) and that your estrogen level is adequate.

It means your problem could then be either from the ovary (absence of ovulation), pituitary gland or hypothalamus.

Commonest ovarian problems that could cause this are Polycystic ovarian syndrome, or Premature Ovarian failure (less likely). A transvaginal scan will help to rule these out.

Commonest pituitary problem you could have is High prolactin hormone. Extremely excessive blood loss can also shut off the pituitary hormones. A blood test will rule these out.

Commonest hypothalamic cause could be excessive weight gain or weight loss, and physical or mental stress.

If your scans and hormonal assays are normal, then beam your search light on those hypothalamic causes.

Eliminate all sources of stress, including stress caused by your lack of menses (a negative chain reaction, if allowed to perpetuate). An irony in itself, but quite practicable.

All the best.

Thanks Doc, am not diabetic and my blood sugar is normal. Doc says i can have vaginal delivery. I actually saw 'show' dis morning and i went to d clinic, i was giving some oral drugs smashed in hot water and castor oil. Was told to come back by 7pm or as soon as contraction starts. I got home about 30mins ago and have gone to d toilet thrice and i feel lighter.

Dont have much experience about what contraction entails.
ok.

The castor oil is meant to stimulate some degree of bowel motion ("purging" ), which in turn should stimulate uterine contractions, so it's a form of induction of labour.

Contractions are pains that come and go. Last less than a minute when they come. The pains may start from the lower abdomen. But when it gets more serious it pains all over the abdomen, and finally the waist and back. They are what are called 'labour pains' in lay terms. When you have contractions, apart from the pains, you get the feeling that your tommy is becoming "hard" while it lasts. This is more obvious when the contraction is stronger.

Wishing you adequate contractions wink and safe delivery.

All the best.

Hello Doc Micron, pls am presently pregnant but 41wks counting, baby is 4.5kg and am yet to see any show or sign of contraction. Am getting worried cos my first pregnancy (4.5kg also) was 10 months and 2wks before my water broke with no sign of contraction or show but I later lost d baby 2days after delivery. Pls what can I take to reduce baby weight and start contractionhuh Thanks
At 41+ weeks your pregnancy is already too prolonged to be left to continue to progress.

What did your doctors say? Are you diabetic? Have you had your blood sugar checked in this pregnancy?

It's one of two choices: allow vaginal delivery and risk recurrence of what happened before or do a Caeserean...

Only the doctors managing you can authoritatively say which is best for you as managing situations like this is usually individualised.

Personally, I would have favoured an elective CS for you though, considering your past obstetric history, the big baby and that you have no live baby yet (Hope this is correct).

Nothing reduces the weight of the baby. Instead, he grows more as the days pass.

All the best.

Hi guys,
Please am a bit worried about something. Since sunday have been trying to have sex but I can't with my wife. I get hard easily, but at the point of penetration, my penis weakens. Just recently, my wife was even teasing me that I have started lasting longer than normal, I attributed that to my usage of folic acid cos we are also TTC. What could be the issue? What can I use to correct this?
Most erectile dysfunctions (ED) in otherwise healthy young men are thought to be mainly psychogenic (a thing of the mind); as such, long term solutions involve some counselling (psychotherapy) which your wife can even start.

Since your own started rather abrubtly (precisely since Sunday), it could also be something else, such as side effects of a drug you started recently.

Many drugs have ED as side effects especially drugs used in treating hypertension and recreational drugs.

Diabetes is also a common cause.

Drugs used in treating ED are effective, but are also prescription medicines and so I cannot prescribe them online.

All the best.

Similar problem as above... Just married, will have erection in my boxer, remove it to insert after some pre-intimacy, the penis will go ground zero... More romance to bring it up prove abortive.... I think I have suffered weak erection as a result of some mastubation few months before I married, never had real sex but mastubate once a while... Pls what drug can I go to the pharmacy to buy
Like his, your problem is mainly psychological especially considering your history of relative inexperience.

Calm down. Be natural with your wife. Do not see sex as a performance, but rather as a play. In time, it will all go away.

Masturbation is not a cause of ED, but may predispose to quick ejaculationn or pre-condition your system to getting sexual pleasure through masturbation alone. In that way, real sex may prove less enjoyable (less intense orgasm) than masturbation.

Gudevenin doc! God bless u 4 d work here, kindly Interprete my dh result;

Isolated: staphylococus spp isolated
Appearance- creamy opalescent
Volume - 2ml
Viscosity- normovisid
PH- 8
Shape/size defects - normal 40%
Small 20%
Large 10%
Tapering 20%
Pyriform nil
Amorphous nil
Double head 10%
Double tail nil

Vitality- alive 70%
Dead 30%
Motility rapid progressive 40%
Slow/sluggish 30%
Non progressive 30%

Count- 89.4 *10E6/ml
Total count- 178.8 *10E6/ejaculation.
The sperm count is within normal range.

The sperm motility is 40% where the normal is 50% and above. So his is slighlty short of the expected minimum. This parameter is the most important when it comes to male fertility.

40% have normal morphology, or formed well. This is just about the minimum expected of the normal

Generally, it is not a bad result, but if he has any lifestyle habits inimical to sperm health, this is the best time to adjust to avoid further damage and improve on the present, especially the motility.

Hello Dr have been having serious body pain for almost a month now, could it be related to the cyst I have I. my ovaries, what should I take to relief the pains.thanks
Cyst could cause pains in the lower abdominal regions, especially when they twist on themselves, bleed, or when they are so big.

General body pain is unlikely to be due to your cyst.

Do you have any other symptoms like fever, vommiting or headache?

Paracetamol may relieve the pain. If it does not, and you are not an ulcer patient or have any other reason not to take NSAIDS, you may take Diclofenac tablets 100mg 12hourly for 3 days.

Then see your doctor if the pains continue or get worse.

hi doc, please what can we do, my period is very regular but i have been trying for over a year to conceive but to no avail. hubby's count is 45million with 20percent active, and 25percent motile. pls what is your recommendation?
Dh's sperm activity/motility is quite low.

Lifestyle changes to boost sperm health are recommended. Exercise, stopping alcohol and smoking, maintaining normal weight, and healthy eating. Avoiding habits that increase scrotal temperature such as use of laptops on the laps, tight underpants, frequent hot baths. Vitamin, mineral and anti-oxidant supplementation.

Some people use fertilaid, but I have no practical experience with it yet.

It is not unusual for period to delay up to 6 months following delivery, especially for someone who did EBF.

I do not see any reason for worry in your case.

Your period will return in time, the less and less you BF and certainly when you stop.

my bone of contention is dat i am seriously TTC (delaying my ivf whic should be last month), and i dnt want it postpone anymore.
should i stop d drug or reduce d dose? pls suggest
Follow your doctors advice.

Obviously, the levothyroxin has tipped you from subclinucal hypo to subclinical hyoerthyroidism.

Stop the drugs and review later.

Rushing into IVF is not your goal; your goal should be to have the best possible shot at it. And to do that, you need to be patient with these work ups.

I perfectly know that time is of the essense, but the ultimate goal is to have the most successful IVF you can get.

Good evening doc, please what could cause of boil in the pubic area.......? what can my younger brother use to cure it, it's really paining him
Many things, from folliculitis following shaving to an STI, such as chancroid and syphilis.

HE needs a dose of antibiotics.

If the boil is any big, he will need an I & D too. This implies cutting through the boil and draining away the pus. It is a hospital procedure.

It's best for him to see a doctor to avoid complications.

I understand that ISPON is fundamental to HSE career in Nigeria, as their membership is required to fully get the benefits of the profession -in Nigeria.

I also understand that NEBOSH and IOSH are more internationally recognised certifications with more global relevance, but not necessarily fundamental to practice in Nigeria.

Is the advice then to start one's HSE career in Nigeria by obtaining the ISPON General HSE first, and then proceed to get for example, NEBOSH IGC?

Will obtaining the ISPON HSE General be enough to make one a member of ISPON?

Please, house I need some clarifications.
