nagoma: With rabies treatment there are many factors including the site of entry , a rabid dog bite on the face is nearer to brain and more likely to develop into rabies while a bite on the foot may be arrested more easily. About high mortality , if you noticed I said " when symptoms and signs of rabies appear" . That is when mortality is very high. Treatment of rabies in real sense is not treatment at all. It is actually prevention. You are trying to stop and neutralize the Rhabdo virus before it reaches the CNS. Ones it is in CNS symptoms appear and you have lost the patient. If ihave rabies and I start showing signs of infection please kill me in a gentle way ( I know you will love to do that) but this is much much better than going thru the horrid picture of full blown rabies illness . It is perhaps the most terrifying illness in men and also in animals. He will have huge viral load and low CD4 and may actually have mutation , you will than have to go to second line too daly and may end up with very expensive salvage therapy. It's a question of give a penny and take your choice. WHO gives guidelines only and the one you are reading is out of date. Actually almost all organs of the body can have cancer related to AIDS- skin, brain, liver, lungs, bowel and even blood leukaemias. I don't expect you to know all these. If you recommend treatment of AiDs with only AZT , you are at least 15 years behind time and you will do more harm than good. In this day and age even in naive patient I will not include AZT ( ZIDOVUDINE ) in my treatment , I may not even include any nucleoside analogs reverse transcriptase inhibitors at all.I would just go for effervirenz and tenofobir as first line and of course with emtricitabine to make a triple chemotherapy. I am assuming that the renal functions are good and you monitor regularly. ( if you stay with me you will learn a lot from quota teacher. There is a type of cancer that is prevalent in those at the final stages of AIDS. People will die from the common cold and pneumonia without treatment for AIDS. Which is why AZT treatment is so important, if you keep the CD4 count high then opportunistic diseases will not be as much of a risk.
