Hepatitis C is a liver disease caused by the hepatitis C virus: the virus can cause both acute and chronic liver disease, which may be mild or severe. It is important to note that Hepatitis C does not always require treatment because the body’s defenses may fight it on its own. However, some people with chronic infection need these medicines to clear the infection and prevent chronic liver problems. Although, treatment is available, most people in resource constrained settings do not have access to treatment. Therefore, finding cheap, affordable treatments in Africa and other low income settings is laudable. Happily, the introduction of generic forms of these medicines have brought down the prices remarkably.

Treatment of hepatitis C has been reported to be effective in trials conducted in three African countries, according to a report presented by Dr Karine Lacombe recently. This is based on the preliminary results of an ongoing clinical trial evaluating the safety and effectiveness of new antivirals in persons with Hepatitis C in three African countries: Cameroon, Senegal and Cote d’lvoire . The researchers included 12 persons living with Hepatitis C that had never received any form of treatment.

The following medicines were found to be effective in different combinations:

Sofobuvir
Ribavirin
Ledipasvir
The result was presented at the 9th IAS Conference on HIV Science in Paris, on August, 1 2017.

According to the main author of the study, Dr Lacombe, an associate professor at Saint-Antrine Hospital, France, “The treatment of chronic Hepatitis C by Direct-Acting Antivirals (DAAs) is possible in the African context with good adherence, good safety and a laboratory follow-up that poses no particular difficulty. Prof. Alain Attia of CHU Yopougon, Abidjan in Cote d’lvoire and myself ran the trial. It included 120 patients with chronic Hepatitis C, who had never been treated for the infection. Each of the three most frequent HCV genotypes (1, 2 and 4) were represented by 40 patients. Patients with genotype 2 HCV were treated with the DAA sofobuvir plus ribavirin. Those with genotype 1 or 4 HCV were given a combination of 2 DAAs (sofobuvir and ledipasvir) in the form of a single tablet for 12 weeks.”

What did the Breast Surgeon do wrong?

Concerns about Paterson’s activities had been going on since 2003, when his colleagues observed he was not removing enough breast tissue during lumpectomies and mastectomies, increasing the risk of cancer recurring. His colleagues described him as aggressive and impatient while his patients felt he had ‘great bed side manner.”

He advocated for breast surgeries that spared the cleavage because he felt that the cosmetic outcome was as important as curing breast cancer. Sadly, this increased the recurrence of cancer in some of his patients. Nevertheless, it took multiple investigations over nine years before Paterson was suspended by the General Medical Council in October 2012. Additionally, it took another 5 years to bring him to justice. By that time, he had operated unnecessarily on over 500 patients and cost the health system about £17.8 million.

This case highlights the value of regulating the health systems. Lives and resources may be lost in countries with weak regulatory framework.

One of his victims, Diane Green, who was given two unnecessary double mastectomies by Ian Paterson, said: “I lost everything – my home, my job, my marriage – as a result of what that man did. While no amount of jail term can repay or make up for what he did, a sentence of 15 years… was never going to be enough for maiming and butchering hundreds of women like me. It is encouraging to see the Court of Appeal listening to victims like me and extending the sentence to 20 years.” as reported by BBC.

Ian Paterson, 59, who appeared at the Court of Appeal via video link, was told his current sentence was “not sufficient” to reflect the seriousness and the totality of his offending. Paterson was found guilty in May at Nottingham Crown Court of 17 counts of wounding with intent. Jurors also convicted him of three further wounding charges.’Brutal and sustained ‘Explaining the decision to increase his jail term, one of the three Appeal Court judges – Lady Justice Hallett – told Paterson his victims had been, “left feeling violated and vulnerable”. She described his treatment of patients as “brutal and sustained”.”They have lost their trust in others, particularly some in the medical profession,” she said.

Can a movie be bad for public health? Yes! According to researchers at University of Southern California which published the first data on whether 13 Reasons Why a recent movie is bad for public health.

Experts have expressed concerns about a spike in suicide rates as a result of an online movie ”13 reasons why”. The movie was released in March, 2017 and is scheduled to continue till at least 2018 when the second reason will be aired.

How did the researchers arrive at these conclusion?

They looked at online searches for the word ”suicide” between the release date of the movie, 13 reasons why in March 31, 2017 and April 18, a period of three weeks.

They were surprised to find out that there had been a massive spike in online searches for information on suicide. Suicide queries went up nearly 20 percent in the 19 days following the show’s release—between 900,000 and 1.5 million more suicide related searches than expected. Furthermore, at least 5 out of the top queries were specifically on methods to end one’s life suggesting that millions of queries were on how to commit suicide. Such search terms included “quick suicide,” “painless suicide,” and “how to kill yourself.” “That’s troubling,” Ayers the author of the report said. “Even with the best of intentions, it’s clear this show has real world consequences.” Why it is not possible to conclude that such queries reflect people’s intentions, it certainly a source of concern.

“Further study is needed to learn how to induce long-term HIV remission in infected babies. However, this new case strengthens our hope that by treating HIV-infected children for a brief period beginning in infancy, we may be able to spare them the burden of life-long therapy and the health consequences of long-term immune activation typically associated with HIV disease.

Interestingly, the child lacked genetic factors that protect persons from HIV, making scientist conclude that the remission achieved can be attributed to the medications and other factors. This unusual case of the South African child may improve our understanding of factors other than drugs that contribute to successful control of HIV in infected persons. What seems certain is that early treatment of infants with HIV, improves the long term outcome and likelihood of survival.

However, this will be the third case of medication induced remission in children with born with HIV.

The first case was of a child popularly referred to as ‘’Mississippi Baby’’ born with HIV in 2010. The baby was HIV-free for 2 years and 3 months. By 2013, the child was declared cured of HIV. Sadly, HIV re-emerged in 2014 to the disappointment of the child’s parents medical staff and scientific community.

The second case involved a French child who was also born with HIV in 1996. The therapy was initiated at 3 months, continued for about 6 years before stopping. The child has remained HIV free for 11 years now, the longest documented period of HIV remission without drugs.

It is important to note that persons with undetected levels of HIV may still be HIV positive in the future. So, it is difficult to declare that such ones have been cured. One reason is that HIV hides in places that are difficult to detect. For instance, the South African child still had pockets of Human Immunodefiency virus (HIV) and some immune response to the virus was found. However, the presence of HIV could not be detected by standard methods.

According to Anthony S. Fauci, M.D., director of the National Institute of Allergy and Infectious Diseases (NIAID), in an interview said

How long can an infected child remain HIV-free after an initial course of treatment? That important question was further explored in a reported case of a nine year old South African child who has remained HIV-free years after receiving initial treatment at the age of six months. The baby was found to be HIV positive at 32 days and received HIV medications for a 10 month period only. Remarkably, the child was found to be relatively free of the virus eight and half years after receiving the course of treatment.

Scientists are excited by these developments presented at the International AIDS society conference in Paris, because it raises fresh possibility of a cure for HIV/AIDS. The case report formed part of a clinical trial for children born with HIV known as ‘HIV Early Antiretroviral Therapy (CHER) clinical trial.’’

“To our knowledge, this is the first reported case of sustained control of HIV in a child enrolled in a randomized trial of antiretroviral therapy (ART) interruption following treatment early in infancy,” said one of the investigators, Dr Violari in a statement released to Sciencedaily.

7.Hepatitis C infection is similar to Hepatitis B. Sexual intercourse injection drug use, unsafe injection practices, unsafe health care, and the transfusion of unscreened blood and blood products are the common means of transmission. 1.There are five types of Viral Hepatitis, these are : Hepatitis A, Hepatitis B, Hepatitis C, Hepatitis D, Hepatitis E
2.Only 5 in 100 persons living with hepatitis know they are infected. Most people with Hepatitis B only find out when major liver complications develop. Recent studies conducted among government workers and traders in Nigeria reported very poor knowledge of the deadly condition.
3.The commonest variants of Hepatitis are Hepatitis B (which accounts for over 250 million) and Hepatitis C (about 70 million).

5.Hepatitis A is more likely to occur in developing countries where sanitation and water supply are poor. 6.Hepatitis B is transmitted through contact with infected blood and various body fluids, as well as through saliva, menstrual, vaginal, and seminal fluids. Close contact with infected persons and hospital equipment leads to spread of the infection. Health workers are at high risk. Unlike Hepatitis A, most people have no symptoms during the acute phase of the illness. Vaccination is the main way of preventing Hepatitis B. All children are expected to have three doses which confers immunity. Persons with HIV are usually treated for Hepatitis B because both diseases co-occur often.

Can babies growing outside the womb survive till time of delivery?

Unbelievable but true. A baby grows outside the womb in a condition called ectopic pregnancy. This is quite common. In Ilorin, Nigeria up to 98 cases were reported in a 3 year period. What is not so common is carrying such baby to term. The reason is that the structure carrying the unborn baby usually ruptures after 6 to 16 weeks which may lead to severe bleeding and loss of life. In the United Kingdom only 5 cases have been reported. According to mail online, one involved a 38 year old who had been trying to have a child for about a year. When she finally became pregnant, the doctors informed her that her baby was in the abdomen NOT the womb. With adequate support and huge risk to her life, the baby was delivered at 30 weeks. In this instance, the egg was implanted in the abdominal cavity which developed a sac that facilitated growth. In Nigeria, there have been cases of heterotopic pregnancies (ectopic pregnancy co-existing with intrauterine pregnancy) which were successfully managed.

It depends on what’s going on in their life.’
