They never complained to a doctor, they self medicated and confirmed the efficacy of whatever cream themselves. Confirmation bias if you will.
There's no study to confirm the efficacy of those creams, because people hardly even know the active agent in the cream. It could be petroleum jelly for all we know. So, no scientific backing to vaginal tightening creams.
