Yesterday night that brought in today, I took call for a colleague who was having menstrual pain. She begged me to do the call for her. Then, a woman was brought in with what I suspected to be post surgical sepsis. The op site was soaked with effluent from the wound. The surgery was done at State Specialist Hospital three days prior to presenting to us. Her temperature read "Hi" on the thermometer, which means her body was so hot that the thermometer could not record it. I called my seniors who were in theatre then, told them what I have with me in the emergency, and I took instructions from them. I secured IV access. I gave 2g of IV cefriaxone (Rocephin brand) stat, and IV Flagyl 500mg stat. She was going. I called my seniors again and told them temperature is not coming down after one hour. I was told to give IV PCM 600mg stat. I then reopened the wound, washed with NS, and redressed with savlon and povidone iodine.

She was a Yoruba Christian. She was praying, holding my hands, saying that I should not let her die, that she still has little children at home. She then started behaving abnormally, talking irrationally and screaming, also convulsing intermittently.
At this point, I called a senior who came in, assessed her and told me privately that she won't make it. He told me to continue masterly activity which I did.

She finally died after few hours she was brought in. I certified her dead in her file, disconnected all life support, and went to my house to sleep around 3am when the call got less busy. I was weak and sad. Did you know, this woman followed me home. I was shocked when I felt there was someone looking at me from my window. My body was shaking. I've never believed in ghosts, but she came to my window and stood there, with same clothes she was wearing when she was brought in. She said why did I allow her to die. I was lost for words for a moment, thinking I was dreaming. But damn, it wasn't a dream. I just drank coke from the fridge few minutes ago. I rebuked her in Jesus' name, telling her I did my best. I told her to go back to where she came from. She stood there for a while, looking at me, then turned and walked away. I heard her footsteps as she walked away. I didn't sleep again till morning. I opened the fridge and took out coke and drank, to be sure I was not dreaming, then started praying in tongues to control the fear in my heart."

What can be a reasonable explanation for this?

Well I think its your fault, you should have tried your best and commit your best into Gods hand to take control, who knows may be if you have administer more drugs she could have made it back to live , 2ndly you should have explained with remorse that there was nothing you could do again rather than telling her to go back to where she came from, she needed your help and as a medical doctor, she confided in you but you failed her.

If only ghosts of the dead people carelessly killed by wicked doctors haunt the doctors, the health care system of our hospitals would be better.

Further more, if only ghost of the dead careless killed by corruption in high places in the country (like accidents on bad roads for which money has been embezzled) can revenge on our politicians, our nation would be saner

exactly my thought. hallucination is one thing ppl r yet to come to terms wit. I once lived in a house for many yrs without knowing that tins wia buried in d compound nd I did not experience any strange feelings.
But the d very day I was told dat tins were buried in the compound that was d moment I started having strange dreams.
The reason for d strange dreams was simply fear.

Who create those impression called hallucinations in our minds? Devil. Why? To create fear. What is fear? It is opposite of faith. A demonic weapon devil used to cast doubt on our believe in existence of God and his promises. Faith conquers fear and gives hope. Fear gradually kills faith, despondency creeps in and if we are not careful, we lose our minds. What am I driving at? Everything has spiritual connotation. It is why we must hold fast to faith and be very convinced about the hope we have in God. Do ghost exist? Yes and No. No because the scripture says to die is once and after judgement. Yes because the devil has power to manipulate our minds and create illusions. Saul sought to talk to the ghost of Samuel, he did in essence but God will later reprimand him for running after familiar spirit. Meaning? What he saw wasn't Samuel but a demon masquerading as Samuel. The scripture even makes us understand that the dark spirits sometimes camouflages the spirit of light. One of symptoms of those possessed is hallucination, They tell you they hear words and see things others don't. Those who take drugs also suffer the same fate because at that point the devil can manipulate a weakened (by drugs) spirit.
Life is a spiritual timespace I always say. Everything that happens here is one way or the other manipulated by forces outside in the spirit realm. St. Paul will say we wrestle not again flesh but against principalities and powers in high places and that our weapons of warfare are not canal. Devil will convince us to discontenance many things because then he has power over us in ignorance when we become open and vulnerable. St. Peter warns: the devil your adversaries roam around like lion looking for whom to devour, stand up to him strong in faith. . Our mind is being daily struggled for by forces of good and those of evil. We remain sane as long as it is safe in God's custody who grant us serenity and peace; we are tortured and tormented by demons with strange voices and hallucinations if we yield it over to the devil. His ultimate aim is to take away our peace. Jesus on the other hand aims to give us the peace the world cannot give.

ok, but if a Junior Doctor handle a critical patient and the patient die , no one to blame than such doctor because of being Junior handled senior Doctor's job, lack of enough knowledge/experience will make the work faulty which will result in loosen the life of such patient
why should there be a blame, the quicker you get that bit out of your head the better, there is no blame here cos it was not proven there was a bridge in practice or procedure.From the post, the Doctor is not in Nigeria, probably UK or US,you think they employ quacks there?.

Are they short staffed?
A qualified doctor doesnt need support afterall there are hospital maned by one doctor. The nurses are there to offer support but from this write up, our man was not strong enough to handle the situation hence the frequent call to seniors as he claimed. He may be doing his housemanship or one or two years experience. You dont buy experience, you earn it over time and you earn it by learning from seniors and cases dealth with over time.

I dont think you understood the post.More drugs will certainly kill her, M Jackson case comes to mind, he wants more cos the does was not keeping the pains away.

jave you checked where this Hospital was locate? the fact it says Nigerian Doctor should point you to a hospital abroad

It is better to make consultations before administering drugs on someone that is sick than to ignorantly do otherwise. Professionally, consultations are allowed, that does not mean he is not qualified.

I think she was in SEPTIC SHOCK...
U should have opted for vasopressors and IV. Fluids depending on your assessment.

It is normal in d medical field to consult a colleague who may not even be Senior but might have handled a similar case. It is absolutely accepted. That woman was already dead before arrival from my own point of view. If someone shud be blamed d doctors handliing her treatment previously shud receive d blame.

I think the Dr was too quick in certifying her dead! maybe she would have woken up lerra. I guess she later woke up and probably the Dr has removed all life support and she became angry,... Like why Dr, why did u give up on me so soon? cry

I experienced something like this when my grandma died too. I was still small in pry 2 or 3 and was not even aware of her sickness but I felt her presence in school almost 3 times before I fell asleep only to wake with a very nasty headache and high temperature. I was excused to go home before closing hour only to be met with a wailing crowd of mourners.



She used to be overly protective of and I was her favorite grandson by miles.

You're getting it wrong. Apparently, he is a much younger doctor who serves under senior colleagues. It doesn't mean that he is not qualified for practiced.

Also note that these senior colleagues were in the theatre, obviously, in the same hospital.

Should junior doctors in our hospitals be allowed to handle cases of life and death situation without the physical presence of a senior doctors?

please read up malignant hyperpyrexia and after that tell me if you still think you could have saved her.
But I dont encourage a defeatist attitude for drs. What if the thermometer was merely faulty? I would have given her my best till the end in case a miracle happened. It also eases your conscience in case it doesnt happen,

Thank you for the comment. But I doubt we are not reading fictions or else the doctor need to be seen by our colleague in mental health.
