My submission....

1) I am a medical doctor, so I have every right to comment here

2) Dr kevwe (or whatever his name was) was very likely a house officer, the most junior doctor in the cadre. I'm sure he was a freebie, fresh out of .medical school. But sad to say, he is a very stupid person and his license needs re-evaluation. Because when someone is vomiting and complaining of pains, didn't he know that the next thing is to resuscitate and give fluids, then call the surgeon on call for a quick ex-lap to relieve the obstruction??

3) the next senior doctor on call (likely Dr balogun) is very lazy, and can never make it if he continues this way. Your junior doctor called you for an emergency, and you were there giving silly instructions "call this, call that" even with the Lagos traffic, one hour would still save a life....

4) the anaesthetist did his job, he was just informed late, by the two stupid docs balogun and kevwe....

5) that hospital needs a overhaul, a panel must be set-up to investigate the matter, and prices have to be paid, at least to set an example to other lazy and negligent docs....

In conclusion, being a doctor is no child's play, the health sector in Nigeria is in shambles, but Mrs chazB has to move on, her husband is gone, nothing can bring him back....

Thank you

I think you are being very unfair to Dr Kevwe. He is still a trainee and can work only under supervision. I dont know if Dr Balogun was the supervising consultant.
His performance was of what was expected of a doctor at that level: He asked for help.

The report says that his provisional diagnosis was of intestinal obstruction. This is very likely to have ben the case.
We don't have the details of the phjone conversations betwen Dr Balogun and Dr Kevwe but I doubt if Dr Kewve would have discharged the man without being advised to do that by his senior, in this case Dr Balogun. I think he is the one at fault here and not Dr Kewve who is most likely going to be scarred by this unwanted exposure.

On another note, there are major systems failure in this Hospital such as how long does it take for the anaesthetist and the surgeon to arrive after being summoned for an acute abdomen.

My condolences to his family.

God bless you doc. You have said it all, Nigerians don't understand what it means to practise medicine in Nigeria.
Toksytoks:
Chaz B was a post renal transplant patient, he has had two major medical issues after the transplant which necessitated a laparotomy . Before we kill the hospital and the doctors, few people need to understand prognosis of renal transplant.

In America where best medical care is obtainable, 5 year survival chance is about 80% if post transplant treatment is carried out in the same center. He had his transplant in India, he had to fly back for another surgery few weeks after the transplant in India. Post renal Care in Nigeria was not too good, i am sure he did not get the necessary post renal support he needed.

one thing a renal transplant patient needs to be wary of are symptoms and signs of rejection. Chaz B's body had rejected the kidney and he had signs that were clear. so what was his post transplant treatment and follow up like? Nephrologists, General surgeon , cardiologists , Dietitian ( very important ) , Internal physician asides immunosuppressants.

He probably had uremia as a result of poor kidney function, Intestinal obstruction as a result of Lymphocele , complications of the renal transplant itself and laparotomy he had few weeks after his transplant.

FLUID THERAPY - in a patient with evidence of renal shut down not the best option.
ANALGESICS - no way

NG TUBE INSERTION ? - maybe to empty the content of the stomach , but the reason for the abdominal distention was most like fluid retention as a result of chronic renal failure.

UREMIA - only urgent dialysis could have saved him , but was he stable enough for the procedure? UREA shuts down the heart and brain in minutes.

Respiratory distress - Pulmonary edema, aspiration. This sums up a bad case, which St. Nicholas wasn't prepared for. Are the consultants full time employees of St Nicholas or part time. What is the working hours stated in the employment offer. Most doctors work 3 jobs to keep body and soul together, so dishing out instructions via the phone is part of the modus operandi.

only few hospitals can afford to keep 4 consultants on physical call every night, i doubt if there is anyone in Lagos. The healthcare system is messed up, poor training , poor remuneration , poor financing , poor and obsolete equipment. Government will not finance training , banks will not finance equipment purchase. Banks will rather finance oil and gas, shopping malls and telecoms projects.

Is Dr. Kevwe at fault? No. He transferred the case to his superiors who were supposed to take over.

we are quick to descend on doctors every time without critical analysis. so many Nigerian doctors have been 'bashed' away to other countries. Australia is recruiting them daily paying them up to 4600 dollars weekly, UK wants them , US and UAE. You down here call them block head.

Chaz B's case was bad, what he needed was a multi disciplinary team in a word class facility to save him. This is at today is not available in Nigeria.

Have you ever practiced medicine in Nigeria? you dont sound like a doctor and it is very uncouth, ill mannered and irresponsible of you to call your colleague 'st**pid'. what do you know in healthcare practice? who gives fluids indiscriminately to someone with renal shut down? you want to overload the heart and lungs? you are trying to fool the people here that you know 'stuff' but you are absolutely pathetic. " quickly resuscitate and give fluids" to a renal patient whose GFR you dont know.

where in the world would you do a "quick ex lap" a stressful surgery on a patient with such comorbidities - renal shutdown, uremia , possible pulmonary edema , hypertension and all. what anaesthetic route are you gonna employ? Chaz B IN THAT STATE CANNOT SURVIVE THE RIGORS OF SURGERY.

I trained in Ilorin my brother, i have practiced in the city, in the village in the dirt, with touch lights , with latterns, i know my stuff, i have passed exams that oyibos fail, because we dey this shit country wey things no dey work dont ever assume one is empty. A few things

1.) All that we have heard is the account of the wife of ChazB. An emotion as powerful as grief will distort any attempt to hear the correct details. It is a painful loss and there will be an urge to rationalize. We have not seen the report from the officials of Saint Nicholas.

2.) From what I've read, ChazB was a post-renal transplant patient who recently had surgery for a hernia. His immune system would have been suppressed to allow the kidney to function. This imunosuppresion doesn't mean that the kidney will thrive, it just reduces chances of failure.

3.)The only place Dr. Kevwe erred was by discharging a post-transplant patient after a few hours. Knowing how precarity of transplant patients. He should have admitted ChazB for specialist review the following day. If a transplant patient has as much as a slight cough, you admit him for a full work-up.

4.)St. Nicholas should have had a specialist on ground. No excuses.

5.) Contrary to what many people think, there are a lot of good Nigerian doctors. Unfortunately, they are constrained by the system. Medical training in Nigeria is one of the cheapest in the world and yet we still churn out good doctors who smash the USMLE and PLAB

6.) I don't understand the fascination of Nigerians with elderly doctors. If Mrs. ChazB had run into one Baba, maybe she would not have published this. The marker of any good doctor should be competence.

7.) Medicine is apprenticeship. Ben Carson did not fall from the sky. He was also an intern once, making mistakes. Many people argue and shout that they don't want to see 'baby doctors'. I ask, who is the son of a goat that the baby doctor will attend to? A doctor is a doctor. The only reason to doubt a doctor should be a clear lack of competence, not because he is fresh-faced.

lexybabe:
Sooo sad!!! Dr Kevwe that I know in st. Nicholas is actually a paediatrician. I fought with him a lot when my he was always pricking my new born baby in a bid to get her blood sample (she was very tiny with multiple needle marks on her fragile hands and legs). Poor guy was sitting in and taking the heat the so called consultants should take. why should a case file be locked in an office? and how can a paediatric handle such a matter? St. Nicholas has the habit of sending their trainee doctors to handle cases they shouldn't. I use them, so I know. I also complain a lot but, how better are the other hospitals?

This is one area I often feel a duty to share my knowledge.
Any doctor worth his salt will tell you that it is naturally very difficult to obtain blood samples from newborn babies. These babies still have very friable and delicate veins and get a tap is many times a matter of luck; it does not matter if it is a consultant or a medical student. The squirming and wailing of the babies doesn't help the matter.
So please don't fight with your doctor and give him heat, you will only put him under more tension to perform

It is high time that all these private hospitals are looked into & taught a lesson. They charge so much & yet they employ doctors that don't know anything. Imagine u going to a doctor for treatment & he'll be looking the case up in his book. I've been a victim in their hands, first it was my mum @ lasuth, they gave her overdose without checking her body weight, it reacted adversely and she died from d complication. Secondly, it was when I was pregnant, dey said i had blood infection& was given a drug. I took it in d morning bt threw it up d way it was @ night. I went back & insisted on seeing d consultant who said i shld discontinue drug cos it Wld affect my pregnancy. Also said there was no infection I've made up my mind that b4 I take any drugs prescribed by a doctor, he must explain its usefulness to me.

This story made me to remember my wife's cousin that died at general hospital idi Araba few years back.

Just the same story of negligence and carelessness on the part of the Dr in charge. The boy died cos he was not helped when he needed to throw up and got shoocked up with the vomitus and died in the process. I was so pained cos it was avoidable and at that poit I felt like seeing the hospital for neglegence but the parent would not allow on the notion that it was the will of God. The boy was the only child of the parent and till now. He died a day before we got his addmission letter for him to study MEDICINE in the University. What an irony of life.

Unfortunately here in Nigeria we take things for granted too mush and nobody gets punished so the killing continues.

One of my Prof. In the University once told my set that as an Architect you cannot afford to make mistakes as this could lead to the death of so many people at the same time. He said because "Doctors bury their mistakes but Architects are buried by their mistakes" I then ask, Do these Drs' Value life?

I have worked in a private hospital where analgin is still given to patients....

So what are you trying to prove

Maybe in another country tho...

To all the medical doctors in the house, i have just one piece of advice to offer :

Never ever argue with idiots (especially online). They will drag you down to their level and beat you with experience.

Vomiting, abdominal distension, abdominal pain , respiratory distress..what else do you need. You suggested a quick ex lap in such patient, please do me a favour and gerrout of here.

who is chazb? a friend asked me
he was a reverend without a cassok
a bishop without a building called church
his was a voice that changed lives
a shepard whose voice his flocks hearken
a bringer of souls to Christ

chazb, you inspired millions of Nigerians with your
radio programme 'sharing life issues with chazb',
you saved so many lives, relationships and marriages.
you drew more souls to God than so many proclaimed men-of-God.
as if you knew this will soon happen,
you mentored chike onyia and stephen with PH to sit-in there for you occasionally.

my heart goes to monalisa your daughter,
chinyere, your wife and millions of Nigerians
whom you have mentored and touched their lives positively.
I know you are right there in heaven smiling
and believing in a better Nigeria for all.
RIP Chaz B.

Brother, it could also be a cause of intestinal obstruction....

I am saying this because I have seen a patient die because of this mis-diagnosis....

This patient came in with abdominal pain, vomiting, breathlessness....

The docs in internal medicine thought it was enteric fever....

But an ultrasound scan revealed dilated bowel loops....

A quick ex-lap saved his life, he even had gangrenous bowel....

So please, don't tell me to gerrautahere... Just admit that the junior doctor was very careless in his management.....

Nigerians don't sue, that is the reason why.
The doctors in america are very careful to ensure they carry out their duties to the latter to avoid very expensive litigations which may even result in licence withdrawal.
Patients there sue doctors when they detect the smallest sign of negligence that results in bad outcomes.
Nigerians need to imbibe this culture.
