I have seen some cases where the believer possess all the above mention, they can pray,they can fast,they don't sin,they go to church and they also read their Bible, in fact,they are always with their Bible 24/7 yet their prayers has no result,not until somebody directed them to a specific prayer point or they tell them to move out from where they are living or they should go and take some little step that they never expect is the course of their blockage.

Just as a child is taught from the moment it is born to obey his parents, in much similar manner God calls on you to begin obeying Him from even before you enter into the Kingdom of God.
