bro ,that regularization is a big scam ,it is mainly meant for NYSC mobilization ( those that gained admission through predegree or prelim and they don't have jamb reg nr ) Another registration number will be generated after u have done your DE registration which will make the one given to u after regularization a waste .

just your final grade (distinction or upper),your O level result ,the month and year you finished from poly,your previous course of study and the course you are going for .I think that's all I can remember for now .

if you checked your admission status on jb site with ur registration number and it doesn't reflect on jamb site ,you definitely need regularization because jamb doesn't have your details on her database .I am also an alumnus of poly Ibadan and already admitted to OAU through DE ,poly ibadan is careless when it comes to this kind of stuffs,almost all of us that processed DE last year did regularization,I don't think you can escape it .

They admit uncountable number of student(about 600 in a class)yet the school administration could not build a single hostel. But pay 3k for Internet access.
