The story is a bit fishy, and there are a lot of holes in the case.

But, I strongly suspect that the issue is money, and the disposition of it. And when a man has money, and a lot of it, it can cause issues and divisions between him and his family.

I also doubt that a medical doctor would be conspiring to defraud the man of his money, along with the Matron too. I also suspect that the issue is between the Sister of the man and his lover. or lovers....plus the man was most likely a 'ladies man'
