The incision used is called inframamary because the gynecomastia was a big one.( Check the preoperative picture and look at the size of areolar). Using periareolar incision would not work, it would be too small for that breast tissue to come out en bloc. We use periareolar incision for small gynecomastia, say Grade 1 or 2. Not Grade 4 as seen in the picture.
