3. Significant postpartum hemorrhage
4. Surgery related
5. Anaesthesia related
6. Medication error

Delayed diagnosis and intervention due to lack of diagnostic tools, invasive monitoring devices, radiological imagings, urgent emergency/ICU response, needed medications, blood transfusion services , oxygen etc.

This death MUST be investigated.

In normal countries, widal test is no longer used to diagnose typhoid fever. Blood, urine and stool cultures are preferred depending on the timing.

However, in developing countries like ours, a single widal test is of no use. Serial widal test ( more than 1) are done and compared to see the trend of the titre, suggesting typhoid infection.

Finally, it should be stated that other factors may affect the titre level aside salmonella.
