Why will excess secretion of ACTH cause lightening of dark skin?
I think with the precursor for ACTH being ProOpioMelanoCortin(POMC), along side acth, alpha nd Beta MSH is secretion which acts on appetite and stimulate melanocyte to increase their synthesis of melanin, which darkens skin due to hyperstimulation not so sure about lightening it. Also one has to consider the etiology of the hypersecretion... Is it primary from pituitary or secondary due to secondary adrenal insufficiency.

For aspirants and those who are yet to make decision on whether to study medicine,

so far since my journey into medical, I won't say it's totally a smooth one. There are lots of up and down, the joy, happiness, the disappointments, stress and various challenges, I and my colleague encountered.
Medicine has a way of making you look like a mediocre(obviously most of us were the best in our former schools). Most of us were told stories about how students who were on 7.0 in their first year were sent packing out of medical schools. We were told disheartening stories about how people who after years of utme attempts were asked to withdraw because they couldn't cope.
We have heard lots of them, but we don't allow fear to conquer us, we have always offered word of encouragement to one another. We believe that since we have started the journey, we must finish it.
Some of you have various motivation for choosing medicine e.g prestige, job security, peer pressure, love for children, wealth, love for the sick and others but please make sure that your reasons or motivations for choosing medicine should be able to keep you motivated throughout your stay in med school because believe me med school is really tough if not extremely stressful, you spend 6 to 7years of your life reading, writing series of tests and participating in academic activities. You tend to have a reduced to zero social life, you are always glued to your books throughout the day. I can't count the number of sleepless night, You read through out the night till dawn, because there are loads of material to read, a medical student will understand what I mean.
Motivation keeps you fired throughout your stay in med school and makes you forget about the whole stress.
Also learn how to set goals and priorities as it will enable you to make good use of your time and ensure proper time management. Believe me, the greatest enemy of medical students is Time.

And to my esteemed colleagues i.e doctors in training, I salute your doggedness and your zeal to push on in face of adversaries. I respect all of you, especially those who braved all odds and make it this far despite all the fears and insult(I remember a certain lecturer telling my colleague in front of the whole class, that he had no brain and ought not to be med school)and "shoe size scores" .
Special gratitude to Dr lomaxx, I read some of his comments and observations, they were quite helpful.
Lastly I will tell you this, "no motivation is totally good or evil". Either your motivation is to acquire wealth, help others, job security or to treat the sick, it's definitely not evil or good but use your motivation to help humanity, by so doing you leave the world a better place than you met it.

We are almost done with Abdomen, after which we move to Pelvis and perineum after which we write anatomy incourse. 300l is for head and neck and neuroanatomy.

In physiology we are in endo and reproalready, did renal and git already, cvs, body fluids, and some others we did last semester. We still have temperature control till end of semester. After which is special senses and neuro physiology next semester(300l)

plus . one handbk like that too by gbemisola..
not that common in bookshop. but u ll like it if u see it.

I have always wanted to ask if essential clinical anatomy is a good book, because it's the only anatomy book that doesn't seem to overwhelm me, yet seem a bit interesting. Makes me wonder if all the good stuffs have been stripped away.

Also you're like the only doc I've seen that uses gray and snells, I wonder why. Why not last, big Moore and charusia like everyone else?
Big moores guy when do i wanna finish reading it with physiology nd biochem to study.
I don't know essential clinical seems ok basically what i need to know about anatomy and grays and snells is also good and straight to the point. Grays is very descriptive. I dont really like charusia and last. Tried them just wasnt understanding.... So i stuck to essential, grays and snells

just like wen i read liver with charausia, went to last , i flow well with some added point..

If u read surface markings of liver with last anat alone , it isnt enough, u might b surprise wen i tell u tinz last didnt mention as i merge it with charausia surface markings , it makes my friends whu read last alone become dump while we do d oral talk of anatomy.

i like ur talk on reading without atlas after a while..it works well in exam@using ur imagination of atlas smtimes

lol sometimes anatomy will be pushing u as if its the only course in Basic med science till close to exam wen u realise BCH wont forgive u..


Balancing in all course is the key though..

Men dem, I combine grays with essential clinicals Keith moores (its shorter than normal clinically oriented)... The snells to revise for exams... Not to mention online videos... And animations

Been busy with incourses sorry for late input, I hope those that need this see it.
For direct entry if you have a degree in Zoology, microbiology, physiology, anatomy, biochemistry and a good cgpa around the 2nd class upper you can apply via jamb for direct entry. If your state has medical school be sure to take advantage and appply. If not try your luck with federal.

About using awaiting result to apply, not sure but you cam try as lomg as your result nd transcripts are out by the time the admissions process starts.

How to apply is easy, just go to the jamb site, get the form, apply, submit your documents to jamb office, send a similar copy to your school of course when they call for admissions.

Sorry but i entered via Direct Entry, I didnt use jamb, but you should be looking towards getting some A's and few B's in waec and a very high jamb score. Also make the right choices. Let ur catchment be on your side plus God.

Try getting adequate sleep, drink enough water and take breaks when studying the key is to study for short periods over time, like 45 mins with 10 minutes break interval where you do any other thing. With this when u come back you would be coming back with fresh eyes. Plus i guess u eed to take your fear and let it motivate you

On the kidney, prolonged hypoperfusion which is a squeal of reduced blood pressure, will first result in a decrease in the amount of urine produced (oliguria) which is more like a compensatory mechanism aimed at fluid retention. If the hypotension persists, the initial oliguric phase now moves to anuria, where no urine is produced at all, the is also aimed at fluid conservation. Also note that, the kidney needs a reasonable amount of blood supply for its normal functions, and in protracted hypotension as this, this is drastically reduced. Then after a while, the nephrons begin to die (tubular necrosis), due to the fact that they don't get enough nutrients and oxygen .
Then when an appreciable number of nephrons have been lost (let's say 70%),the kidney loses its ability to conserve fluid, and the anuric phase in which no urine is produced now becomes a polyuric phase in which excessive amount of fluid is lost due to the inability of the renal to reabsorb fluid. Then the kidney fails...

The same thing happens to the liver and other vital organs, if the liver doesn't get enough oxygen and nutrient due hypoperfusion secondary to hypotension, there is a gradual reduction in its synthetic functions and other sequels.
But the effect on the liver is not as acute as that on the kidney, this is because unlike like the kidney that is perfused by just one artery, the renal artery. The blood supply to the liver is not restricted to a single vessel, and liver failure takes a more gradual process.

Relating hypertension to the renal and hepatic system, is basically the opposite of what I have been talking about. In hypertension, there's adequate perfusion of the kidneys and RAAS is not activated. The nervous system also plays a part to reduce the blood pressure to normal by reducing sympathetic discharge.
But protracted hypertension just as I explained for hypotension, has several damaging effects to all organ system, and especially the small blood vessels... Which could rupture due to excessive pressure.
And in specific conditions like portal hypertension, the function of the liver is drastically affected.

Well put, just exactly what i was hoping to see.... I understand the cardiac part but havent gotten to the renal part of the story yet, we just gonna start that block in physiology, but you have demystified the second part for me.

I understand that when there is hypotension, perfusion is low which leads to ischemia of the cells of kidney and liver also there would re retention of some bad proteins and toxins which would have been filtered and detoxifyed by the liver. I salute you sir

Hmmmm nice one but isn't the aim of TPR in arterioles so that the blood flow to kidney nd liver can be increased?

Ok in a case of a prolonged case of hypotension not from hemorrhage, also you didn't relate hypertension and renal perfusion

I'd disagree with you bro . . some schools have better medical facilities than the other,such school provides an enabling environment for the students making their reading less tasking and some schools also have more cadavers than the other . . theoretical knowledge and clinical practice differs from each other.
In the end it all comes down to the students... Plus I don't think they make it in Nigerias curriculum to make reading less tasking for medical students... Also about the cadavers it's not like it's everyone who comes for dissection so it still comes down to a dedicated student who actively studied and participated.

See........ if you want a BMS, stick to it. If you want medicine, go for it now. What you do in A'levels is the same thing that will be done in 100 level. The same Physics, Chemistry and Biology. That's why you can skip 100 level after A'levels.
Concerning your interest in the BMS: you start doing them from your 200 level; Anatomy, Physiology and Biochemistry. So, why not just go for medicine?
And about wanting to go further abroad, you can easily do that with CAMBRIDGE A'levels (I stated that earlier).
So you see, it all comes down to basically the same thing but going through the long path of BMS to MBBS........ Well..... 'Father Time' isn't patient, you know?
But like I stated before, I'm only making suggestions from my own little pool. The choice lies with you.
You really don't wanna be going through the long process except its something you just have to go tru... Because u didn't get med in the first place... Go for it now nd get it over with than unnecessarily prolonging the inevitable... Mind you getting admitted via direct entry isn't as easy as it seems you have bsc nd a level who graduated with you nd those Before you who didn't get admission initially to compete with nd its really hard. Like someone said earlier getting med via DE is like passing a camel through the needlehole. Have confidence nd chase ur dream if get put down try again... Just be smart about it. Mind you am speaking from experience DE Admissions aren't that easy to get..d

In jl war ......he was obviously dictating to hal , superman was turned to a brainless raging parademon batsy talked some sense into him...with little or no emotions..... The only people that see a flicker of batmans emotional side is the bat Family especially damian wayne and dick grayson and even to them he is very rigid ,dick even concluded that living with the bat is loneliness...... So no batman doesnt give "we can do this guys". He gives " you have to this, do it quickly.. There is no time"
Exactly that's more of the bats I know.... Either that or he beats some sense into you... Like he did with green arrow once, and to flash... Bats can't be allowed to get all emotional and mushy it defeats his personality... He might as well start wearing blue or white....
Theres a reason he's call a dark Knight... He has to fight to protect without necessarily needing your praises if possible get your abuses.... Maybe on another earth maybe bats would do the whole we can do this guys.... He's more likely to say you need to stand and fight to save the world....... Or else all its lost because it's your duty

Why does it seem like diseases and ailments get more severe during the night?? Over the Years, I've observed that coughs get stronger at night, likewise pain, feverish conditions too.

During the day when a sufferer of a particular ailment or condition is being told to rest and he/ she does so, the condition subsides, but when it turns dark, it escalates. At least, at night our bodies are relaxed now.

Is it medically linked or its just nature. Curious to know
Ionno but from my little understanding I guess at night when the body is at general rest the activity of antibodies reduce maybe.. But then sleep is a good time for antibodies to work as most of the atp is directed to their battle... My more experienced ogas will know why but I know it's related to the biochemistry nd the physiology

Honesty is better, you need that in career nd life... Its stressful to start changing nd all plus that fraud.... You frauded the university nd they can expel you. Just stick to your state... Honesty is better
