Be honest in your visa application. It is point based so you don't have to worry. Once you fulfill all requirements you'll get the visa.
I declared mine and I'm almost done with my studies.

This maybe edema. It is highly likely to do with his Kidney.

Could be his kidney isn't expelling extra fluids. Those fluid have built up and is the cause of the swollen eyes, legs, arms etc.
