please i need, my wife and myself are trying real hard and its been a while already.

i will appreciate useful suggestions as my doctor is not helping matters as well.

Your sperm count is slighlty below normal range, but this is not the biggest issue: people conceive from far lower sperm counts.

Sperm motility is the most important determinant of male fertility, and yours is very low at 5%.

Do you smoke? Quit if you do.

Do you drink alcohol? Quit if you do.

Do you take recreational substances? Quit if you do

Overweight? Optimise your weight. Target BMI between 20 - 25kg/m2

Develop an active lifestyle: exercise daily or at least 3 times a week, register in a gym or do some road works

Eat right. Avoid junks including too much caffeinated and carbonated drinks.

More fruits and vegetables (extremely important). These provide large chunks of vitamins, minerals and antioxidants that improve sperm health.

Ask your doctor about vitamin and mineral supplementation, especially vitamin c, e, folic acid and zinc.

Above all, maintain a positive and stressless mental attitude. Stress will adversely affect the outcome.

Repeat your seminal fluid analysis quaterly to follow your progress.

You are five months pregnant, please do not take herbal drugs especially. It could very dangerous for you and or the foetus.

Your condition is facial muscle weakness. Many central nervous system causes abound, such as strokes. It occurs most commonly in pregnancy.

But in the majority of cases no cause is found, when it is called Bell's Palsy (googl it)

Usually it will resolve with time on its own (weeks to months), but steroids and other anti-imflammatory pills can acelerate its resolution.

Please remember anything you take isn't just for you, but also for your innocent baby. Do not take drugs/substances indiscriminately, but take only those prescribed by your doctor who has seen and known you.

I know how scary it could be, but in time it will all resolve, so long as no serious underlying cause is responsible.

Folic acid is good to prevent Neural Tube Defects in the baby (a kind of abnormal development of the brain). Should be taken before conception and through the first 3 months of pregnancy.

For general wellbeing preparatory to conception, balanced diet and exercise are recommended. Unless you have any specific challenges, no need to take other pills, unless as otherwise precribed by a physician.

After the 1st month of pregnancy, you may commence brands like Pregnacare.

@luckyG1, sorry for the ...., thar was used wasn't sure what he wrote there. I have been spotting since 2months now. Went to the hospital and have been taking treatment still the spotting hasn't stopped. Have also done Hysterosalpingography, the doc said when the bleeding stops and after my next circle I should come for another check-up and if need be we do evacuation of the womb I guess. But till now am still bleeding. Don't know what else to do

If the cause of your bleeding is structural, such as due to a fibroid or polyps, drugs will not make any difference. Drugs for shrinking fibroids will of course subside the symptoms, but these are rarely prescribed in Nigerian regular hospitals.

Gentle endometrial curretage (what you understood as evacuation of the womb) will help, especially if your symptoms are due to a polyp, or a functional disorder. But if due to a fibroid, the help -if any- will be temporary.

In the mean time, I hope you are taking iron supplements to prevent blood loss anaemia.

UTERUS: intramural echopenic or echopelvic mass 4.2x5.6cm & intramural echope... Mass 3.9x4cm siting on the cavity.

Endometrium:- it is 0.55cm thick and linear.
Adnexae:- the ovaries are normal. The cul-de-sac is free
Urinary bladder is normal.

Your symptoms of abnormal bleeding is most likely due to the fibroid sitting on the cavity.

Fibroids cause menstrual problems whenever they affect the endometrial cavity, whether they are intramural (within the musle/body of the uterus), intracavitary (growing into the cavity{the space where the embryo develops}) or submucosal (beneath the lining of the cavity). Your scan showed a moderate-sized fibroid sitting on your uterine cavity. If this was not a transvaginal scan, you may do one so as to enable a better appreciation of the relation of the fibroid to the endometrial cavity. I would also want to rule out endometrial polyps in your case.

Having said that, since they are causing you these much problems, and especially if you have been ttc, the advice is to remove the fibroids. This can be done laparoscipically or via open surgery.

Laparoscopic surgery is better, but lack of expertise and equipment-coupled with much higher cost- means open surgery is what is commonly done in Nigeria.

Your endometrium of about 5mm is normal, although this varies with the stage of the menstrual cycle.

Treatment is lifelong.

Modification of diet/lifestyle is as important as her drugs.

When she complies with her medications and diet prescriptions, she will achieve control of her blood sugar over time.

That is an unspecific term that means nothing in essense.

At worst, it means a uterus larger in size than expected.

It does not indicate any particular problem in itself.

If there is no underlying problem, it should subside in about a week.

If you are not with peptic ulcer, you may take tab diclofenac sodium 75mg 12hourly for 4 days with food, vitamin c and paracetamol.

If it exceeds. 2 weeks you will need to see a doctor to run some tests to find possible underlying causes.

The high TSH is a sign of low thyroid hormones (T4 esp). The combination you have there suggests a "sub-clinical" hypothyroidism (low thyroid hormone), where the inapparent low thyroid hormones is still being fairly compensated by production of high TSH from the pituitary gland. Somebody with a sub-clinical hypothyroidism may not have any symptoms of thyroid problems.

Yes, when high TSH is due to hypothyroidism (even if subclinical), it can cause sub-fertility, recurrent miscarriage, abnormal foetal development and other pregnancy complications.

Treatment is straightforward and consist of pills meant to increase thyriod hormones.

However, caution is advised in initiating treatment, until you are 100% sure the lab can be trusted. Your doctor may even request a repeat of the test to verify that those results represent the truth.

Good morning to my doctors and everyone here. I've been ruminating over this issue for a while now, please kindly clarify me on this issue. For people like me who detests swallowing tablets I want to know if the idea of wrapping the tablets in a morsel food is a good one. My thinking is that since the food would eventually dissolve in the stomach the tablets should work, but I've heard rumors that tablets don't work when swallowed that way. What's your take on this?

It can still work. In fact some tablets are to be taken with foods, eg aspirin

Some are better on empty stomachs, eg some antibiotics.

But invariably, drugs wrapped in food will eventually be absorbed into the blood stream.

well, if you do not have any serious symptoms, it is better to visit on your appointment date. But if your symptoms worsen, please go to the clinic at any time, you will still see the doctor, though it may not be the same doctor who gave you appointment.

PID can affect conception but I hope her other tests are normal, and your own tests too?

You were afraid of pregnancy so much that it stressed you out. That disrupted your cycle. I think I explained it to you earlier. Everything normalised the moment your fears were allayed.

But if you really want to comfirm whether it was a miscarriage, do a serum pregnancy test. If it was miscarriage, pregnancy test will still be positive.

Besides the degree of harm posed by these diseases you mentioned, pales in comparison to that posed by HIV. For one, HIV is incurable and requires life-long medication.

Diabetes, hypertension, asthma are all incurable, it does not make them reportable diseases.

The MOH or any other organ may compile statistics concerning any disease including malaria, doesnt make them reportable diseases.

Reportable diseases list varies from country to country based on the prevalence, incidence and potential of such diseases to reach epidemic proportions.

In America, for eg, Malaria is a reportable as it is very rare there. But in Nigeria, malaria is not a reportable disease as it is grossly endemic here.

Poliomyelitis is almost extinct in Nigeria, and it is a reportable disease.

HIV is not a reportable disease. HIV is not even reportable to the American CDC

The medical ethics in Nigeria are protected under the medical and dental council of Nigeria Act, Laws of the FRN.

The law is clear about reportable diseases. HIV is not a reportable disease.

Doctor's are privy to lots of confidential information, unknown to even the closest people to their patients. Diseases, miscariages, abortions, drug use, genetic issues, parternity issues, all of them placing their partners at varying degrees of disadvantage. But should they now go about and start telling anybody everythin?

The confidientiality clause is there to protect the patient, whom the doctor is legally contracted to. And mind you, we are all patients at one point or another.

HIV is just a chronic disease, but so are hypertension, diabetes, cancer and asthma, etc.

Ebola is a reportable disease, like lassa fever, cholera, and all those diseases that can wipe a community within a few days.
