Low prolactin should not affect your chances of conception, but may affect breast milk production following childbirth.

If you have low estrogen, the treatment will depend on the cause. Problems in the ovary, pituitary gland, hypothalamus and many drugs and even stress may cause imbalances in your estrogen level.

If you treat it randomly or in isolation, you may just be scratching the surface alone or cause more harm than good.

Please see your gynaecologist to know why you have low estrogen. The treatment will be straightforward.

M/c/s is microscopy, culture and sensitivity test of the sperm, used to comfirm if there is an infection affecting the sperm

Unremarkable urine analysis means there is nothing specially wrong with the result of the urine test.

Like any treatment, there are potential risks. Increased risk of post delivery bleeding and increased risk of Caeserean section are perhaps most important to you. Rupture of uterus is rare in first-timers, but not impossible, esp in unqualified or inexperienced hands.

Note that these are potential risks, and the odds of them happening is very high

Moreso, most times the decision to induce a pregnancy is taken, the benefit of the induction would have far outweighed the risks associated with continuing that pregnancy.

Hello doctors! Please help me. Am 39 weeks gone and I have not been feeling any serious pain or contraction but mild ones at the abdomen and also I have been urinating so frequently lately and I feel pains as the urine come (no sign of infection). I also feel pain in my anus like i wanna to poo and the poo not coming. I went for antenatal today and my doc said am due and ask if I have been contracting I explain the feelings I have been having and she said if by Tuesday I don't deliver I will be admitted and labour will be induced. It's my first time and that admission and labour induced thing is getting me scared. I will be 40 weeks on that very tuesday,please advice me on what to do to avoid being admitted. I have serious phobia for hospitals
Do not be scared of induction. Induction itself is not labour and not painful.

Coituus may stimulate labour. If you have had any major complications during this pregnancy, particularly episode of bleeding, you are not advised to have coiitus.

It is possible you have anal fissure - a crack in the lining of the anus. View it as a little "wound". It is a relatively common problem.

The causes could be mechanical, such as frequent passage of hard stools. Most times though, it is caused following what are called "spasms" of the muscles or sphincter or valve around the anus, which constantly place the anus under tension and cause the cracks.

Because it is a wound, you feel the symptoms after each stooling, because each stooling esp if hard or very watery, could potentially worsen it. The peppery sensation can also be explained by the fact that stool comes in contact with the crack after u have used the toilet.

Treatment: Avoid hard stools (lots of water, fruits and vegetable); avoid diarrheoa; prefer to wash gently after stooling as against using tissue paper; practice Sitz bath twice a day (pls browse it). There are prescription medicines used in treating selected cases, and surgery is used for extreme ones. Anal fissure can be stubborn to heal usually because each stooling episode counteracts the treatment efforts, but straight forward cases abound.

Please see a doctor for proper diagnosis and treatment.

Most important step is to register at a tertiary hospital for antenatal as early as possible. If this is the first pregnancy and she had no history of miscarriage or TOP or bleeding in this pregnancy then this baby will not be affected.

To prevent future pregnancies/babies from from being affected, she would receive anti-d immunoglobulins (Rhogam) at various points during her antenatal.

If she had had any of the incidents mentioned above, she should have started receiving Rhogam.

So, the best thing is to register for antenatal at a tertiary hospital and take it from there.

How many weeks was the pregnancy when baby was delivered? This is the most important factor for survival.

Such babies seldom 'cry' the way you may expect. They usually only attempt to cry or make funny faces in response to pain.

Key areas of managing the baby are incubator nursin(temperature control), fluid/electrolyte management (drip), nutrition and control of jaundice and treatment/prevention of infections, among others.

I believe doctors are already doing those and more to keep the baby.

If the cause of your lack of ovulation is high prolactin, bromergon is the drug of choice. You should bring down the prolactin level first before you may add clomid.

As a ttc, are all you and your dh's tests clean?

Bacterial infections are curable, though chronic PID is harder to cure.

How do you mean it goes and comes? How was the PID diagnosed? I hope you are seeing a proper doctor and hospital, and not a laboratory facility?

Chronic PID and complications from poorly managed acute PID adversely affect pregnancy chances.

You should be able to conceive unless you have had serious complications from PID such as blocked fallopian tubes.

Not necesarily. The advice however is to scan for retained products. If any, do evacuate them. Even theoritically insignificant retained products may cause practical problems later on. A gentle curretage will do.

Hello doctors in the house, thank you for your helpful answers.
I'm 23,ive had irregular menstrual cycle for 9 years. First hospital I visited, after a scan and hormonal profile, the doctor said I have pcos and that my condition have to be managed . he prescribed an oral contraceptive which I took for some time and stopped. After two years, I went to another hospital which I've done some tests like pelvic scan, hvs, blood count, hormonal profile. The doctor said pcos is ruled out cos I don't have excess hairs, normal sized, and no acne. He gave me a combined oral contraceptive [combination 3 to be precise] which I took f9r three months and asked me to watch for the fourth month if I would see my period without the drug. I watched and didn't see, I'm totally confused . when I did the Hormal profile, it should my estrogen level is very high. My questions are
1.what could be the cause of my hormonal imbalance.
2.is it possible I have pcos without obesity, acne and hairs on my chin like a man
3.if I'm to take an ovulation inducing drug when I'm ready to get pregnant, what's the probability of sucess, I mean how long would I have to wait before I conceive
4.is the any final remedy for hormonal imbalance
1. Causes of reproductive hormonal imbalance are many. Any problem affecting the function and structure of any of the organs that control the production and or release of the hormone will result in imbalance. These include, problems of the ovary, pituitary gland and the pituitary gland inside the head. Psychological factors abound too. Usually, hormonal profile/pattern will give a clue as to where the problem is from.

2. Yes, the diagnosis of pcos must not include those symptoms. There are 3 group of symptoms in pcos. Any combination of 2 of the 3 groups is what is needed. The symptoms you mentioned are under just one group.

3. Nobody can say with certainty. But usually clomid is precribed initially for 3 successive menstrual cycles, after which it can be reviewed. Other factors upon which pregnancy success would depend include your age (the lower, the better. This is the most important too), your hubby's age and his sperm characteristics.

4. Hormal balance is only a symptom of an underlying problem, whether known or unknown. Any medical condition for which the underlying causes are known and understood usually has a cure. For eg, if it is caused by a tumor in the ovary and the tumor is removed, cure is expected.

Your high estrogen is very much against the diagnosis of pcos. Typically, pcos will cause increase in the opposite hormone, the male hormone - testosterone. However, only a doctor who has seen and examined you can draw reasonable conclusions. Hope you have seen a gynaecologist?

You should ensure that everything has been done to rule out estrogen-secreting tumors in the ovaries, or any other tumours anywhere capable of disrupting hormonal balance. There are many of such otherwise-harmless tumors that are common withing your age group.

Your condtion is medically called enuresis. There are many causes including physical, behavioural, developmental and psychological ones. Many times there are no known causes too.

Since you have tried the common behavioural changes advised, I think it is time you saw a doctor. There are pills known to help, but they are prescription medicines. You would need a neurologist and or a psychiatrist.

Don't stress yourself out. Nothing is permanent. Just take informed steps toward the solutions.

The car passed all scan test, so its not a matter of posting result here.

I have a professional scanner that reads Airbag and power train as well. It also reads freeze frame (not all scanners can do that as this scanner cost about $900)

I also checked the two O2 sensors and they passed, neither is the car ready for emission testing.

Car is still in factory mode, no modifications altered as per factory default
Then consider your driving habits

just be careful with that piritin prescription though, it contains chloro-quine or so if i am not mistaken, it really messes up my system when i take it, have only taken it once when i had malaria + typhoid and it was given to me without my knowledge/consent... come see itching nah, i do vigil by force.

Piriton is an antihistamin sir. It does not contain chloroquine. It is chlorpenamine, a 1st generation antihistamin. It is usually given along with chloroqine in order to relieve the itching caused by the chloroquine.

Your information is misleading, and it is based on a one-off experience you did not understand. Do not give health/medical advice if you are not certain of the facts. Though, your intentions are good, you may be doing more harm than good.

welcome. If there are retained products, the choice of management is evacuation/suction curretage. The cost varies across different hospitals depending on many factors. 26k is neither cheap nor expensive for a regular Nigerian hospital in an urban area.

Sorry about that. Normal levels should be below 30ng/ml. You have made significant progress so far. You should repeat the test to know the current value. You should continue seeing your doctor so he can follow your progress. And limit your worries. Psychological stress can greatly complicate your menstrual problems.

Having said that, 9 years of ammenorrhea is a long time. I hope they have ruled out conditions such as premature ovarian failiure and Asherman's (esp if there was a history of TOP)?

YOu may also require a new diagnostic work-up to check if the initial diagnosis of hyperprolactinemia is still the most likely cause of the problem.

Sorry about that. Maybe you 'over-cratched' the rashes. You may get the drugs from any pharmacy. Tabs loratidine 10mg daily. You may add any of Piriton or Phernegan to it esp at nights.

Other drugs you may need, esp when severe or complcated, are prescription medicines.

It's an allergic reaction. Your body reacts to the chemicals released in your body when you are stung by the ants. This is usually in the form of rashes, swelling of the sting sites and itching of the skin. Normal over-the-counter antihistamins such as Piriton, Phernegan and Loratidine will relieve these symptoms. The former two may cause drowsiness, but not the latter.

However, in severe reactions -which can happen following repeated stings - you could have dizziness, blurred vision, fainting and breathing problems. Seems like your symtoms are approaching the severe. While oral antihistamins can provide little relief here, you will need to see your doctor when you have features of severe allergic reaction/anaphylaxis

The best treatment however is prevention of ant bites, as there will be nothing to react to if you do not get bitten.

What is the cause of your secondary amenorrhea? How old are you, and for how long has the amenorrhea lasted?

The mosr imprtant step in managing your situation is to properly diagnose the cause. This depends on the history of the problem and some tests. Are u seeing a doctor offline? What have they said?

Hi doc.
First, I want to commend your effort coming here to help people. You are blessed.

I have been on this thread before and your advice helped me a lot.now I am back again cos my girlfriend is in a very bad state right now.

It all started when she and I decided to stop using protections during sex. So ever since, we make out without. Protections and I often ejacualte into her. Though she sometimes takes postinor 2 if it happens during ovulation and sometimes she take ampiclox when not ovulating ( that's after she has expeled almost all the sperm from her system by pushing it out).

Recently, while we were making out, she felt this sharp pain in her abdominal cavity.the pain was so strong that we had to stop. The pain lasted for about 3 hours before it became mild. But till this day she feel the pain once in a while but mild.

After the last occurence, she missed her flow and I urged her to run PT. She did at first but It was negative. After a week interval still no flow so she ran another PT and now it was. Positive. So we decided to abort. We thought it was still too early for D and C so she took abortion pills but it did not work. She took another prescription after sometime still no bleeding. So she had to go for scan and that was when we realized she has ectopic pregnancy and a small fibroid growing but not inside her uterous.

So doc. Pls I need to know if there is a way to go about this without surgery cos I can't afford it right now. The pregnancy is not up to 10 weeks yet. Just a month and few a week now.

There are pills used to treat ectopic pregnancy but I doubt they would be available ordinarily in Nigeria. Sometimes too it can resolve by itself

But usually, in our environment surgery is the main treatment, esp when it ruptures. Rupture of ectopic pregnancy is a lifethreatening situation.

Do see a doctor near you and prepare for surgery too.
