Hellohouse: I and my family did our medical and police report over the weekend at Ikoyi. If not that I had extra money on me the whole show would have ended just like that without any achievement. We arrived Lagos on Thursday 6th as at 7pm and lodged in Vintage suits Adehanmi Street in Surulere and on Friday by 5:30am we left for the clinics and where the first to reach there, it rained seriously. But, for the little umbrella we took from Port Harcourt my little daughter of less than three months would have been soaked. My people, all protocol observed we started and finished the medical the same day and we paid the total sum of #124,400 (One hundred and twenty four thousand four hundred naira only)

Me: 12k +12k = 24,000 Madam: 12k +12k =24,000 My son: 12k + 22k =34,000 (the boy is 17yrs) My daughter: 7k + 35,400 = 42,400 (the girl is less than 3months old)

Imagine I went with my daughter’s immunization card oh! I don’t know what would have happened if I didn’t go with her card. We left the clinics’ about 2pm and straight to Alagbon and by 4:30 the police report was ready we paid #15k and left Alagbon at 4:45pm back to our hotel room and Saturday morning we departed to Port Harcourt. I thank God for journey mercies waiting for the medical result and the real interview now.
Congratulation to all those who got current recently we are moving gradually and I know God has perfected everything to his glory IJN.

Note: if you are a family man, please go with enough money for medical so you would not be disappointed when you are given the bill.The doctor in room 2 upstairs that handled our physical examination was rude to us; my wife had serious fallout with him. He doesn’t understand what customers focus is at all, the medical director was not around I would have reported him.

Limited to the following U.S. Embassies and Consulates:

• Visa applicants will be interviewed at any U.S. Embassy/Consulate listed below:

While use of the online forms is currently limited to applicants who meet all of the conditions above, these forms will eventually be implemented worldwide and required for all immigrant visa applications.


If you are a Nigerian and your interview will be holding in Lagos kindly follow the instruction as contained in your 1NL, fill form DS230 and 122 and send to KCC forget form DS 260 it does not apply to you for now. Hope this helps

From your answers, It is clear that the agent did not use married for you during the initial entry, because if he did he would have used someone’s photo at the derivative aspect. The agent having known that you won only wants to attach a man to you in other to make more money. My candid opinion is get your 1NL from him if he refuses, go here https://www.dvlottery.state.gov/%28S%28blc0gi45b3thjo45nn2sg2ql%29%29/Secure/StatusCheck.aspx to get your 1NL with the details you provided during the entry, with that you can proceed with the remaining process incase you get confused anywhere feel free to ask questions. I am afraid that this is a hard one, but first before I proceed you need to answer some questions
1. Which name did the agent used as your husband, your fiance’s or the young man he wants to use?
2. Whose passport photograph was attached with yours during the initial entry?
3. Which date of birth did he used?
If the answers to the above questions tilt towards his choice of man for you, I am afraid there is nothing you can do than to play according to his rule or abandon the program.

Those with misplaced confirmation number can now check their results without much problem on this site https://www.dvlottery.state.gov/%28S%28blc0gi45b3thjo45nn2sg2ql%29%29/Secure/StatusCheck.aspx what this also indicate is that those with high numbers may not be too comfortable anymore especially with respect to DV 2014 winners. Please let us be guided in answering questions here, if you are not sure instead of giving someone a wrong answer go back and research or read the DV instruction, or instructions on the DS 122 and 230. The DS 230 form contains four (4) pages, at the end of the second page (2) you are expected to sign your form while the fourth page (4) is to be signed before the CO probably to confirm that, the first signature is yours. Saying one should not sign the form is not correct please kindly be guided accordingly.

TWO KINDS OF NPC BIRTH CERTIFICATE. Please take note:

@Kevwe_flex, My brother the birth certificate specimen you posted on this thread is for children not for adults, we have two kinds of birth certificate issued by the National Population Commission (NPC)
1. The first one is called birth certificate for (minors) children between 0 and 17 years.
2. The second one is called birth attestation given to (adults) from 18 years and above. The birth attestation carries your passport stamped at the down 2/3 sector while the spill runs over to the form, the form is an A4 size form.

Seguneji: I want to send my forms and my wife's form including the bar sheet code to kcc via email and that means I will b sending like 11 attachments plus the passports, I want 2 ask if there is a way I can Marge my forms to a single pdf file and do d same too for my wife or I shld just send all d 11 attachments Like that. Sending your form though email works well where there is a network scanner. Go to a good business center where all the electronic devices are networked, tell them you want your forms scanned as a single file, you and your wife should be two separate files. As for your photo scan it and use the tool on this site http://travel.state.gov/_res/flash/cropper/FIG_cropper.html# and crop photos to the required standard (save the photos with your names then attaché the photos two each with your forms and send it to KCC). They will reply you within three to six hours.
Note: if you cannot do it you are free to scan it and send it to me pjogutum@yahoo.co.uk I will do it for you free in my office and send it back to you. Hope this helps just my own contribution I am a DV 2013 winner.

I think in the case of your wife she is the petitioner, although she derives the right from you as a derivative. That was what I filled for my wife, son and daughter and KCC replied that the forms I submitted are ok for further processing. This is the reason why each person is expected to sign his or her own form except for infants whose parent needs to sign for like in the case of my daughter, please others with family can still contribute so that we don’t confuse our family member in the house.

@Kevwe: the Petitioner is the principle applicant that won the dv lottery if it is you then you are the petitioner. Address is your own address, your email and your phone number. Hope this helps

@YessWeeCan: I will advice you to continue with the processing, you have not committed any crime by so doing. The only thing that changes in your profile is marital status which is understandable, if you are ask why, say you did that to show commitments to your wife and seriousness to her parents. this will have no effect on your current status, afterall they select you for the second time you do not select yourself.
If atall they decide not to isue the visa, this way you have help ease the process when filling for her and her kids in the nearest future

@Seal777: if you follow the above advice you will run yourself into trouble, you don’t have any chance forget it since you are already a green card holder. Kindly follow the normal way and apply for your wife its better that she will be coming late than for you to be returned back to Nigeria. Kindly read the instruction on how to apply for spouse, the rule is very clear.

Nigeria but if your wife is from another country you can choose that if the country is eligible for dv visa.

Please start with your highest level of education if you are a masters degree holder choose (Advanced Degree)

7. WORK EXPERIENCE

If you answer yes, state the work title and job description that you have been involved in for the past five years briefly eg.

Work title: IT helpdesk analyst

Job describtion: resolving customer problem, doing backups, restoring data, reimaging and upgrading systems etc. then wirte the names and address of your employer.
