What your son has is not dandruff, He has a problem with his immune system, He has scalp psoriasis which is not curable but manageable (you can greatly reduce the frequency of flares).

Visit a dermatologist, they'll probably prescribe corticosteroids (prednisolone) and a prescription shampoo containing salicylic acid and coal tar.

This doesn't look like Dandruff to me. If it is, I think. It's wild.

Step 1: Try and shave the boys head as much as you can.
Step 2: look for a herbal soap called Eyin Aro in Yoruba. Mix the soap with Ose dudu (black soap)
Step 3: use the mixture to bath the child morning and night.
Step 4: Buy cacatin hair cream, rub on his head after bath.

But ensure you see a consultant dermatologist. At a teaching hospital/FMC. Not just a doctor in any hospital.
Your boy obviously has issues with allergy. Tongue and lip swelling may be adverse reaction and can become quite severe as anaphylaxis at other times as to block airway. Risky.

Have they checked him fully with blood test/his immune system/allergy test to know what to avoid in food/cream/medication/jewellery, etc?

The key is nutrition. I strongly believe this is due to food intolerances in the body.
Remove all grains especially white rice and corn. Remove all Refined sugar i.e white sugar
Remove artificial sweeteners.
Remove a legumes e.g groundnut, cocoa, beans anything in that family.
Remove all white flour from cakes, cookies ,pastries, and all derivatives of white flour. Do this to remove gluten from the body.
Remove all processed foods.
Do not anything from a factory.
Remove Maggi.
Remove Dairy and eggs.
Eliminate pasta and its derivatives such as indomine ,etc they could contain gluten too.
And allow 30 -60 days to clean out the system.
This has worked for me. I am not a medical doctor but only suggesting this.
Lots of vegetables and organic foods and of course water.
After you notice positive changes. You can either just continue or reintroduce the foods one after the other to find the roots cause.
That will take a longer time and cause trigger the condition again.

You can Use any of these 2 methods twice a day:
1. Apply soda soap and wash with slightly hot water
