Our spine isn't rigid but possesses some anatomical curves to allow for a number of biological processes to take place (chief of which is breathing). Even though the vertebrae in the lumbar spine are the largest and most prominent of the spinal column, they still represent the weakest point of the human back.

A few helpful points to note;

1) As we age, the water content of the inter-vertebral discs (which act like shock absorbers) decrease considerably making the connective structures more susceptible to injury.

2) All the Inter-vertebral discs are avascular i.e. they lack their own direct blood supply. They get their nourishment from osmosis when movement occurs.

3)Years of poor posture in sitting and standing, improper lifting techniques, mechanical stress and repeated trauma to the spine (in this case, compressive forces acting on the spine from prolonged sitting) usually manifest as one acute (focal) episode of excruciating pain e.g. while turning backwards to reverse, bending down to tie ones shoe laces or bending to pick a heavy object.

The pain can be prevented by;

1) As some posters rightly noted, we were not designed (or if you like; we did not evolve) for prolonged sitting. So hourly breaks are recommended.

2) @Seun gave an excellent recommendation by way of ergonomic chairs with firm lumbar supports.

3) In the absence of executive/ergonomic chairs, a firm lumbar roll would suffice.

4) Spinal derangement usually begins to set in with a weakening of the spinal muscles(those located at the small of the back - and are usually the first to give way with prolonged sitting). So strengthening exercises that target those specific muscles usually help.

Finally, self medication or prolonged use of strong pain-killers (NSAIDs) can be harmful to your health as they can cause ulcers.

You may benefit from seeing a physiotherapist if the pain persists.

A group of a pain-relieving drugs termed Non-Steroidal Anti-Inflammatory Drugs (NSAIDs) still remain the analgesic drugs of choice for severe pain. They come in several generic names, but their active ingredients include; piroxicam, diclofenac etc.

At this point however, I would strongly advise that you do not self-medicate as long-term use of these types of drugs have harmful effects on the body eg The GIT (inside the stomach) - where it can cause ulcers or worsen already existing ulcers and in tendons - where the abolished inflammatory response of the body caused by these NSAIDs lead to poor tendon healing. It will be advisable to seek your doctor's opinion on the type, dosage and a detailed explanation of the side-effects of these drugs.

2) The good news is that not all knee pain is attributable to arthritis. You did not indicate your age, nonetheless, the pain in your thigh which accompanies the knee pain might suggest an Ilio-Tibial Band (ITB) syndrome. For a more comprehensive assessment, investigations to rule out other causes of the knee pain and treatment, kindly consult a physiotherapist.

A couple of factors are implicated in declining stamina. The principal factor is age. (I am assuming that you are above 30 years old). As we grow older, our muscles become less supple and less pliant. In addition, our joints too become less mobile or flexible culminating in reduced athletic output. Some studies have theorized that peak performance begins to decline around age 30. Other factors to be considered may include; inadequate warm-up routine, poor technique, poor conditioning, poor equipment, altitude etc.

Physically demanding sports such as football coupled with advancing age often lead to discogenic back pain (i.e. back pain caused by a pathology of the intervertebral discs). Back pain is also often present in subjects with weak abdominal muscles (read 'pot belly') or tight hamstrings (or both). Typically, Long term postural adaptations (bad posture), poor lifting techniques and other mechanical assaults to the spine often manifest as one single acute episode of back pain.

I would recommend that you see a physiotherapist for the back pain and for your stamina (or lack of it). In assessing your condition, you may be required to carry out some tests - so be prepared. If time and money is no consideration to you, you may also seek the counsel of a nutritionist and an exercise physiologist.

Finally, with proper nutrition and exercises, it is possible to delay the deterioration that accompanies aging. With discipline, you can join the ranks of Ryan Giggs, Kanu Nwankwo and Roger Miller who all played well into their forties.

@all, the physiotherapy curriculum and practice in Nigeria has been expanded to accommodate 4 areas of specialization (for now) with plans underway for further areas of specialization. They are (in no particular order);
1) Orthopaedic and sports physiotherapy (which is widely regarded as the mainstay of physiotherapy practice)
2) Neurophysiotherapy
3) Cardiopulmonary physiotherapy and
4) Ergonomic and industrial physiotherapy

In this case, I believe that @adewummy was only making a reference to the area of physiotherapy specialization and not to the part of surgery (orthopaedics) which deals with interventions to remedy abnormalities, diseases and injuries of the locomotor system.

unless you want to do surgery or fix a brace then visit go ahead and visit orthopaedic surgeon. Note brace will only slow down the spinal curvature( useful when you are under 21), surgery will restrict the spinal curvature and leave you with some limited mobility so if you had a goal of being a dancer or runner scratch that. However NONE of them will compensate for the misalignment of your hips, help with pain or lung capacity, neither will it derotate your spine or reduce the size of your picquet on the shoulder or even untwist your muscles and other muscles compensating for the imbalance.

However if your are looking for exercise to improve your quality of life then search for physiotherapist that specialise in scoliosis, I repeat all physiotherapists can not manage scoliosis.

Please google SOS clinic or other scroth method based clinics worldwide.
Hopefully one day A specialist clinic will run in lagos focusing on physio for those living with spinal curvatures whether hyperkyphosis or scoliosis.

1) You are right about the presence of neurophysiotherapists in Nigeria. In fact, the physiotherapy curriculum in Nigeria has, over the years, been expanded to accommodate 4 specialization areas for now; Orthopaedics & Sports physiotherapy, Neurological physiotherapy, Ergonomics & Industrial physiotherapy and Cardiopulmonary physiotherapy. Neurological physiotherapy is the fastest growing area of specialization following othopaedics & sports physiotherapy (which has traditionally been perceived as the mainstay of physiotherapy practice).

2) The 'MIS' model you proposed has been in practice for awhile and is quite popular with middle-upper class Nigerians (but not because of the reasons you might think). Challenges of accessibility to healthcare and the welcomed convenience of obviating mind-numbing traffic are huge plusses.

However, there are some formidable challenges that have to be considered if the model is to become not only more common place but financially viable as well. Typically, most people see a physiotherapist for the first time upon referral from a Doctor - in order words, the perception of physiotherapy and what services it can offer is quite low. In addition, and especially considering the class of Nigerians you are targeting, there is a 'status' attached to travelling abroad for medical treatment that goes beyond just having access to world-class medical facilities. My point here being that even among the informed and well-to-do population, an assurance of an accurate diagnosis and comparable quality of treatment/service is no guarantee that they will stick around.

Your project is indeed laudable as the challenges of post stroke rehabilitation can be daunting. Unfortunately, in these parts of the world, we are stymied by a dearth of public records (data) and a shallow pool of dedicated health care professionals, social workers or volunteers to draw from.

Concerning your query on the number of such centres available presently in Nigeria (and possibly any other related enquiries you might have), a good place to start might be the Medical Rehabilitation Therapists (Registration) Board of Nigeria (MRTB). MRTB was established by Decree 38 of 1988/Acts M9 LFN 2004 for the regulation and control of the under listed professions in the Health Sector in Nigeria: Physiotherapy, Occupational Therapy, Speech Therapy, Clinical Audiology, Osteopathic Medicine, Chiropractic Medicine and Prosthetics & Orthotics.

The board is located at Yaba (opposite the old market). You may obtain additional information from their website, www.mrtbnigeria.org.ng

Hi Guys,

I am an avid basketball fan. While going through the Guardian newspapers this morning, I came accross a picture in the sports section (pg 57) with the caption,'D'tigers (down) blocks American Tyson Chandler at the london Olympic Games. Diogu is one of the nominees for the maiden nigerian Sports Award holding next month in Lagos'.

Ike is undoubtedly one of the hardest working ballers out there and was one of my favourite players on the National Team. But it is either the guy who wrote that caption is high on something or took journalistic license too far because from my angle, the block clearly belongs to Tyson Chandler (who plays for the knicks BTW) shocked grin

I can't wait for the NBA season to begin (Oct 31st).

PS I hope Tony Skinn is alright now? (Tony was our starting point guard on the National Team. He played for Goerge Mason University. He had perhaps one of the sickests highlights during the memorable USA vs NGR game at the Olympics. He crossed over James Harden - and his beard, into a full split..and made the shot! However he injured his knee in the following game)

Most of us are excusably skeptical of insurance companies especially when it comes to payments for justifiable claims. At the beginning of the relationship, the insurance company will usually project itself as a company that pays claims promptly for most classes of risk eg theft, damage (3rd party or comprehensive), fire etc as long the as the premium payments are regular and up-to-date. However, from anecdotal evidence, it appears that whenever claims are presented, the insurance companies employ all kinds of professional (and sometimes unprofessional) strategies to avoid paying - including recourse to the famed 'fine print' in the contract papers.

A former colleague crashed his Toyota Avensis awhile back and went through hell with the insurance company despite the fact that his mother was a high court judge at the time. It took the combined efforts of his mum's connections and other overt threats to get the insurance company to replace the totalled vehicle.

I am constrained to start this thread because I am interested in buying a new car and considering the deteriorating security situation of the country, I want to be absolutely confident that any Auto Insurance Company I deal with will pay or replace my vehicle promptly in the event of a theft without too much diversionary tactics or stories.

Kindly share your stories so we can learn. At the end of the day, my scepticism just might be unfounded. Also, I would appreciate it greatly if you could recommend any good auto insurance company that has a good track record for payment of claims.

Hi Nairalanders,

I have been a long time forum member. However, I hardly log-in or comment on topics, I just browse through the topics on the front page.

I was therefore intrigued when Busy body (f) asked today in her response to Seun’s post on ‘New Nairaland on old WAP phones’ what the difference was between a guest and one who is logged-in. I have always to know the answer to that.

I have also noticed severally in the past where new members to forum (often inadvertently) derailed active topics by making enquiries on how to enhance their NL experience e.g. asking how to post new topics etc.

With the current upgrade to NL, I guess this will be a great opportunity for matured/experienced NLers to assist Seun and the MODs as well as new members to the forum by posting answers to FAQs (frequently asked questions). Any tips or shortcuts that will enhance the user experience would be welcome as well. If you don’t have answers you could post your questions (about the forum) that have been bothering you eg how to post topics, how to follow topics, how to reply to topics, how to use emoticons, how to upload pictures or videos, how to add an existing user as a friend, benefits of being friends with a user etc.

Since we are all stakeholders, (and truthfully as a reaction to the tribalism thread), it could be a nice idea, if we can all collectively come up with ideas to draft a forum etiquette (an NL constitution if you like) that serves as an informal guide on how to behave on the forum.

MODs, your experience in these areas will be invaluable, so your support is solicited. I believe it would be helpful If you can move this to the front page.

@poster, please ask your friend to go with his family to his wife's family to report the matter, so they can call their daughter to order. If his wife refuses to come home then he should move on with his life. The only other thing he will need to do is to try getting his child back. But this will take a very long time to accomplish as the child needs the mother, now & will need her for quiet some time. I wish your friend all the best.
