Congratulations! When did they notify you that you won? I know they said they would send letters between May and September. I don't know what form you're referring to but you should go to their website and they should have instructions there on how to fill it properly.

Do not write below block 35 leave blank, you will fill it before the consular officer on the day of your interview after an oath must have been administered to you, there after the officer will also sign. If it is signed by the officer know u have 99.9% of d visa. And how you answer 30(a-i) will be a big factor.
