It is unfortunate that every Nigerian now wants to be a doctor. I have made it now compulsory for every patient I give a prescription to bring back the drugs they got from the drug store for vetting. I have seen enough rubbish. It is unfortunate that other health workers feel that can take over the doctor's position with their poor knowledge base. Let them continue as for me and my family, we are safe but
to every other person, to your tents oh Israel.
And I will suggest u leave ur duty post and go and take up the pharmacy so u can stock all drugs in the market and make them available to your patient. Till tomorrow, Pharmacists will always continue to offer alternatives to ur already structured and not up to date prescriptions, there is nothing u can do, when it comes to drugs, that's is the only thing they stand with and they are ur superior there.Get that into ur head.

To Well meaning Nigerians.. .
It baffles me when a pharmacist equates him or herself to a Medical Doctor.. .
They should equate themselves to NATIVE DOCTORS before going to the Medical Doctors.

A truthful NIGERIAN PHARMACIST had intentions of studying MEDICINE and SURGERY ab initio....
Even the PHARMACISTS want their kids to study MEDICINE
Most FEMALE PHARMACISTS want to get married to the Medical Doctors, which is their utmost priority.
I think the earlier Medical Doctors start proposing to them, the more interesting the situation becomes.
PHARMACISTS rush their wives, husbands, kids and Parents to the Medical Doctors (Hospitals) in times of Emergency.
So why don't they manage them at homes, factories or pharmacy retail outlets..and watch out for recovery. .. ?
Since u are better than the Medical Doctors, then follow my instructions as stated above..

So what's all these nonsense ..?
And wen they rush to the hospital, the only people they meet there are the doctors strictly! No gateman, no ward made, no nurse, no pharmacist! Pls expand ur thinking faculty and understand that at any point whatsoever, the patient is the boss of every health worker and all work in his best interest.

And wat will u think if i say 'it is only in Africa that doctors who know zero about drugs prescribe' Pls think, before u make statements, nobody has it all, but everybody has something to offer. U spoke as if knowledge about diagnosis isn't in the books or it is exclusively restricted to those with MDCN licence.

Pharmacist ask questions about health histories in developed countries. May not be as detailed as that of the doctor. Their recommendation or catch might be what could save your life.

1) Pharm reps working for pharmaceutical companies sometimes hide the side effects of drugs they promote to doctors in hospitals. They have targets to meet with good bonus rewards.
They are usually Sales professionals/marketers with a Pharmacy degree.

2) Doctor prescibes the drug based on what he/she knows from the discussion with the pharm rep.

3) It is that community pharmacists (safety net) that may save your life from the killer side effects his pharm rep colleague hid from the doctor.

If this is common in developed countries where regulations are strict, how much more in Nigeria where anything goes.
I wish most people have this understanding.

In all developed countries .....there are prescription drugs and over the counter drugs.
This means there are drugs that could be abused cause you don't need a prescription to get it and there are prescription drugs which means the Doctor has to prescribe. It's called Physicians prescription in most countries which means the doctor prescribes.
Pharmacist in developed countries run pharmaceutical companies....that's where they're really needed. They make millions from there. There are no standard such here so the hustle for Hospital work where we can all kill the Golden Fleece.
I feel the Pharmacist should be the one seeing the patient then if they're that better.
Paracetamol and Diclofenac might both be indicated for pain management .....i wouldn't give one to an ulcer patient. So also Ciproflox and Augmentin.
Unless the Pharmacist plans to do a clerking all over.....the brand name is as important put as the generic name in a nigeria where half the drugs are fake to start with.

What baffles me is the way all these allied health workers - pharmacists, nurses etc rush their children to the hospital to be seen by a Doctor and yet they will be bragging to know better.
I presume,even doctors shouldn't on a normal day treat themselves or their close relatives when they are sick so as to be objective with the therapy, is that clear?

It's also important to know that while cefutan and rocephin are trade names for ceftriaxone.....a doctor will know which he'll prefer in an emergency or a serious condition.
The pharmacist knows much about the drugs but the doctor knows the reality of which drug works.
I for one know some oral hypoglycemic agents I'll never prescribe again cos they never work.
There's no where in the world you change a Doctors physician. You can only recourse back to the prescribing physician or refuse to dispense.
The Pharmacist is a professional in his own capacity and is as well trained as the doctor in his work. There shouldn't really be any reason for comparison

Doctors know a whole lot but never more of how effective a particular brand is. Leave it to the pharmacists in nafdac that carry out actual tests on individual brands to ascertain their safety and efficacy.. I get where you coming from though. You try different brands on different patients and you might have observed a trend but don't forget it also depends on a whole lot of other factors.

I have been treated for malaria recently which should have covered me for 11 months. Having read myvcase note, he still went ahead to prescribe camoquine for me with other strong antimalarial drugs. I later discovered that only 3 tablets of this drug usually nocks people down. This guy gave me 9 of it. Up till now I have not been myself. Pls if you know where I can lodge complain pls let me know. I have called the hospital complain line but it is dead.
it seems you are educated enough, the dose he prescribed is the right one. the drug has known side effect which either ur doc or pharm should have explained to you. don't just criticize ur doctors' prescription based on hearsay and ur Doctors age

Of course you won't give ibuprofen as an alternative to paracetamol.
Sometimes pharmacist shud also stick to the brand. Some brands formulation are more potent. And for ur info some do alternate pcm wit ibuprofen, aspirin

do you have an idea of what we call medication therapy management or therapeutic drug monitoring? it seems Nigerian GP will go on everlastingly strike by the time pxcists starts practicing to their full potentials. Dr's hate seeing pxcists coming into the wards to interact with the patients. pharmacy practice is growing and expanding by the day. You can work on yours. stop flaunting ignorance here.
if u want to enter ward, go n buy jamb form and fill to study medsurg. Dont go about complicating doctor's treatments. U guys almost killed a patient all because u want to grow n expand

Well,sometimes you said.One thing about health practice is that there are some things you get to know by experience and not just 'book'.That's when you can insist this particular brand is what you want.
Using ibuprofen instead of paracetamol won't exactly be considered alternation.It's using something more potent than paracetamol in that case.For instance,you wouldn't give ibuprofen to someone in early cyesis instead of paracetamol.

when next you see a pharmacist in the ward chase him away. I know you are a product of one of these universities running only medicine so you know nothing about interdisciplinary team work and approaches in patient care. It's strange when you see other professionals doing their rotation and clerkship since you feel you own the hospital. I only pity your generation of professionals who are so arrogant and think they know it all.

It has turned into a Dr. vs Pharm. squabble as expected. Petty.. Very Petty. Everyone can stop crucifying the pharmacist now. The young man made another post saying he wanted ciorofloxacin for a cough and the pharm suggested azithromycin as an alternative.. It was a simple idiosyncratic reaction. A Doctor or pharmacist would not know you'll get an hypersensitivity reaction from a medication (especially azithromycin (a macrolide) that is a first choice antibiotic in asthma) until you get it the first time. Unluckily for the lady the first time was on her. The constant bashing even in the absence of all the facts just goes to show how messed up members of the health care team are.

There is what we call drug of choice in managing a particular condition and a quinolone is never one of such in the management of asthma not even in personalised medicine. some folks are just so full of their self.

A post NYSC doctor has more experience than a pharmacist with many years of practice?
I agree no pharmacist should be in a hurry to alternate prescriptions.

He came FOR CIPROFLOXACIN and the pharmacist suggested AZITHROMYCIN which is a drug of choice for acute bacterial cough In a patient with COPD. The irony in your last statement undecided

Lol. Experience in what exactly?. Pharmacist have the legal right and backing to alternate 'brands' of a medication. Pharmacist don't and won't change the medication in a prescription to a different compound. They could inform you on mistakes or refuse entirely to dispense a medication. It's as easy as that.
