Firstly, I would highly advice you to consider using barrier protection (condoms) during intercourse

Secondly, it is really difficult to ascertain whether she passed the "warts" onto you first or you passed it since I don't know your history well.

Honestly, it doesn't matter. I think that you both need to be investigated for HPV (and syphilis). HPV is a chronic, lifelong STD once you get it.

So, you both have HPV for life now. You are at risk for genital-anal cancer, cervical cancer.

IF you both are below the age of 25, I suggest getting HPV vaccination like Gardasil if it is offered in Nigeria.

Treatment is usually only done more for personal reasons, HPV warts are self limited and goes into remission.

Prevent (or rather limit) further transmission by using condoms; and remaining monogamous.

Re: premature ejaculat*ion: If you are young and quite healthy, then perhaps it's psychological. Since you are depressed, an SSRI such as sertraline would take care of both depression and the PE. There is also PDEIs like Viagra.

My policy is this: if are sexually active, a genital lesion of any kind is an STI until proven otherwise.

It could still be STI like herpes especially if it is painful.

Anyway, lets assume it is not STI;

If it is just pruritus (itchy), then try: applying cold compress/cold bath/cool water/ICE packs. Also, try anti-itch creams like 1% hydrocortisone cream (this is OTC in US), if low dose topical steroid doesn't work, then perhaps try stronger one.

But, best thing I can say is if this itch continues; see a gynecologist or a GP for a proper pelvic exam + or - swab. Has she ever gotten a PAP smear?

One thing though: you are sure this rash is a result of mere heat exposure?

Let's say it's heat exposure then, just limit your exposure to heat. The rash is self limited and not harmful. I know it's annoying, but as long as it is just heat rash, it will go away eventually once heat dies down.

Can try:

Drink lots of water to stay hydrated. Stay in cooler areas/fan/air conditioned areas. Cool showers or baths helps. Cold packs as well to affected area. Wear loose clothings.

If rash itches, try not to itch. If really bad itch, can also can use low dose cortisone creams to affected areas. Can also try oral antihistamines like diphenhydramine or loratadine- this should be over the counter for low potency effects

Caution: skin rash especially if it develops into open sores can breed superimposed bacterial skin infection. So, be advised for that. If you suspect open sores, can decrease risk of skin infection by applying topical antibacterial ointment like Neosporin-this should be over the counter as well. IF you suspect skin infection, go to your GP ASAP.

Due to her age, she is more likely to have a more serious cause for her back pain like compression fractures, spinal stenosis or worse yet, cancer.

I suggest visiting a GP for a proper exam to rule out those three things. Further more, three signs that you absolutely need to monitor in her back pain are these: numbness/tingling/loss of sensation in her private areas, unable to either urinate/defecate or hold them (incontinence). These signs do indicate urgency and she needs urgent evaluation and management. MRI of spine. + or - XRAY.

If you want; do tell me more about your mothers personal health history and other symptoms she is expressing or exhibiting, and I might be able to tell you if this back pain could be benign or more malignant.

Assuming her back pain is benign: all she really needs is acetaminophen, some stretching back exercises (can google search this), physical therapy if you can afford it. Just get her to move though, prolonged bed rest is not good. Prognosis is fairly good for benign back pain. But, again, you wanna make sure it is a benign cause.

Do visit a GP overall simply because of her age.

It means ecoli found in your urine is more sensitive to nitrofurantoin then ceftiaxone etc and resistant to ampicillin.

Meaning:

Nitrofurantoin is your best abx to kill off this bacteria
And ampicillin will not kill it at all.

It means ecoli found in your urine is more sensitive to nitrofurantoin then ceftiaxone etc and resistant to ampicillin.
Meaning:
Nitrofurantoin is your best abx to kill off this bacteria
And ampicillin will not kill it at all.

KK. I see we're going advanced microbiology....

Well,

usually sensitivity reports are not that ambiguous ( it's either + or -). So, the bug is either susceptible or it is not.

BUT!

In some cases, different STRAIN of same BUG can produce both + and - where by one strain in susceptible and one strain in resistant.

Case in point:

Staph aureus: there is MSSA (methicillin sensitive staph a.) and there is MRSA (methicillin resistant staph a.). But usually lab will just report it as either MSSA or MRSA not both. Enterococcus: there is VRE (vancomycin resistant enterococcus) and VSE.

It is also the idea behind influenza virus and why it is hard to establish a vaccine that is permanent 'cause the strain of virus changes year to year.

*Modified* : If both +/- are present on lab report, what this probably means is you have both resistant and susceptible strain of bacteria present. Therefore, the antibiotics in question can be used in higher doses in order to be bacteriostatic or bactericidal (balanced with toxic side effects).

First: since I can't see this skin lesions; it is hard to tell. Things it could be: some form of eczema, atopic or seborrheic dermatitis, psoriasis.

Yellow plaque sound more like psoriasis though, but it is not impossible for it to be something else though. Do you get allergies or are asthmatic per chance or anyone in your family?

I would first, keep skin area moisturized; get good moisturizer: Vaseline- cream based, not petroleum laden gel like one. Also, aloe vera, coconut oil/cream based, cocoa butter.

I would also go and get RX for low to mid potency steroid ointment or cream like hydrocortisone. There are other treatments but for more advanced cases.

Re High temp: go and get a basic lab work that includes peripheral blood smear; get tested for malaria and/or typhoid fever dear. And, then go from there.

*modified*:how high do you get exactly? Have you measured it with thermometer then or

Gud evening doctors in the house. Since I became conscious of my self, its either one illness or d oda. My mum told me dat wen I was still in d womb, she almost had miscarriage, she couldn't walk, still she gave birth to me wen I was 8month 2 wks. Since den, ve suffered from typhoid, pneumonia, malaria, ulcer, infections and kidney problems. Am always on medication. My question is, does my premature birth has anytin to do with my low immune system? And hw do I boost my immunity to be resistance to all des ailments. Even now dey said I have kidney disease and UTI, ve taken all manners of antibiotics, yet d pain is still der. Pls help advice me on wat to do.

You may have some form of primary immunodeficiency. I don't think prematurity has much to do it. 8 months and 2 weeks is not that bad at all.

If you want to make sure; get blood test to check your immunoglobulins (Ig) to see which you are deficient of.

Management involves: managing infections with antibiotics when they arise. Can prevent infections by getting vaccinations such as flu shots or meningococcal vaccination etc. Other preventative measures: eat well, good hygiene and the like.

If you are truly deficient in Ig, can get Ig immune therapy injections. This only temporary replaces what you are lacking. You will likely need to go for replacements every so often. It can get expensive too.

Have you been evaluated at all for this supposed kidney disease and UTI?! Like seen a specialist and/or gotten imaging done.

It's probably is all due to your immune deficient state, so you are predisposed to frequent infections.

The infections and hence, pain; will diminish once you manage the underlying problem.

My recommendation right now: first, determine the cause of frequent infections; is it inherited or acquired immunodeficiency? Have you been screened for sickle cell ? Whatever, it needs to be managed first. Once, immune deficient state is ruled out, then follow thru with nephrologist for your chronic kidney problems.

Otherwise, it's really just managing and controlling the infection and pain I am afraid. Sorry to say.

Suppressing pain is really the goal for chronic pain management; sometimes even after you know what's causing it. That is the nature of chronic 'painful' conditions. *Note I said "management", not "treatment" or "cure".

I am very empathetic to your pain thou. I wish you well! Let me know if any other questions

Not taking any medications, no major health condition (except for the fact that my body itches when I bath with sponge, cold water and when I stay under the sun for too long). No other symptoms.

I normally don't drink much water but I'm trying to adjust now.

Positive surface antigen means either you are acute infection or you are in carrier state depending on a number of other tests like: IgM or IgG, LFTs, and the rest of hepatitis B panel.

If your doctor ran all these tests and they came out negative, then you are most likely in carrier state.

In carrier state, your probability of converting to active state is low, but it can happen. This means that you have to periodically visit your doctor and keep rechecking the labs: LFTs, liver scan, Hep B panels. Your doctor cannot really give your anti virals because you are not acutely infected.

RE: Fatty Liver. This is mainly due to metabolic syndrome; a collections of factors like: diabetes, high cholesterol, obesity, hypertension.
There is really no specific treatment for fatty liver except for to target these risk factors listed above. So, your best chances of preventing or slowing down of cirrhosis form fatty liver is to modify your risk factors: weight loss, exercise, treat HTN, treat high cholesterol, treat diabetes and the like.

There are many causes for vaginal/vulvar itching.

There is infection: yeast infection, bacterial vaginosis or STI: Is she presenting of other symptoms: discharge, burning, irritation?
There is chemical irritants: has she changed anything her routine like lotion, soaps, perfumes, detergents, douche, shaving creams, sanitary napkins?
There is inflammtory conditions like Linchen planus. Other less likely: vulvar maligancy.

I am guessing she is still premenopausal since she is trying to concieve. Because menopause is primary cause in menopausal or post menopausal or peri menopausal women.

Her case sounds more like irritants of some sort. Investigate the hubby for what he is using on himself as well (lotions, perfumes, condom etc). If this itch persists, might want to take a trip to a gyn and get a pelvic exam. In the mean time, keep the area dry and use mild, non scented feminine wash if you are going to use any. Avoid any possible irritant that might be exercebating it or possibly causing. Tell hubby same; keep free of anything he may be applying on himself.

Pay attention to when you particularly feel the itch- that might point to the cause.

RE: Infertility is not diagnosed until one year of consistent and persistent unprotected intercourse. So, there's still plenty window here.
