I heard any declaration of age certificate, must not be done 4 years from the screening exercise. How true is it, cos mine was done 2012.

The time you statutorily declared your age doesn't matter so long you did it in a court of competent jurisdiction.

If you've uploaded all that is required, look up you'll see the stages for the registration click on finish.
