Thanks Sis I have answered over 35 people on NL, not just once , but back and forth, people here can testify, if I dint answer you it probably cos I must have opened it and forgot to reply immediately or I was busy. None of the doctors have email. I got the hospital number from Google Doctor - (314) 432-3669 Hospital - (314) 996-5000 ask for financials, they would send you a scanned copy of the package i posted on NL. Check my earlier thread for the zoomed in version of the maternity package, you would see a direct number to call the financial councelor. Study their website too, they have alot of information too. I hope this helps. |

Thanks for that link , but I am a 1000% sure that more than 70% of the mums on this thread didn't use notarized consent form from the embassy, I am also aware of others too , that have used notarized consent forms from notary publics not In the embassy . My friend did has for 1k, no date was written on it, when the baby came she filled out the date and as I type, mother and son are in abuja. |

Amother4life: It's a uniform process. States do not issue passport or have any say it the requirements. it's the department of state that handles passport issuance. Only a nortary from the US embassy in Nigeria is acceptable . That isn't completely true, someone I know has used a notarized form from a notary public in Ibadan, this issue has been argued in the past my dear. |
