You are welcome WHO recommends the following package of interventions for the prevention and treatment of malaria in children: use of long-lasting insecticidal nets (LLINs); in areas with highly seasonal transmission of the Sahel sub-region of Africa ie Northern Nigeria where the rainy season is shorter (approx 4 months), seasonal malaria chemoprevention (SMC) for children aged between 3 and 59 months; in areas of moderate-to-high transmission in sub-Saharan Africa, intermittent preventive therapy for infants (IPTi), except in areas where WHO recommends administration of SMC;prompt diagnosis and effective treatment of malaria infections. This refers to most of Southern Nigeria. Intermittent ie 'start and stop' prevention is better than long term continuous treatment. Now this advice is given for areas with high risk of malaria transmission like nigeria. The best approach is bite prevention. As for actual drugs and doses speak to a specialist, it is unethical to prescribe without a proper consultation and documentation of allergies, counselling on side effects and review of risk factors. Many antimalarials don't have paediatric formulations so it is easy to give a child an overdose hence the need for professional advice. Don't give your child medication because your neighbours says it works for his child. If you take your child to the village during the holidays etc where you have no environmental control it is a good idea to give preventative treatment. Most people however fumigate and use mosquito nets in their usual residence which is good practice. Your child will definitely get malaria at some stage, the above measures are designed to help the child develop a natural immunity. Malaria is the biggest killer of children under the age of 5 in Africa so don't be complacent. The risk of dying from malaria is reduced after the age of 5 though it still remains a threat. I second the above opinion, trying to save money by avoiding a paediatrician is a foolhardy exercise in this matter. Have you thought of the consequences of giving an inappropriate drug from non specialist advice or if the diagnosis was not actually malaria? A child being raised in Africa needs prompt treatment of malaria not prevention. No drugs are actually licensed for use in children because no pharmaceutical country wants to be sued in the future. Ask yourself how long are you going to give a child prevention treatment for? till they are 5? A growing child does not need chemicals or drugs in their system for long periods of time unless absolutely necessary. Discuss this matter with a specialist and start off with bite prevention measures such as mosquito nets, fumigation etc |
I am sorry to hear of your loss. In nigeria i am afraid to say it is 'common' for women to die in pregnancy, we are in the top 5 for maternal and perinatal deaths in the world. Worse than some so called less developed african countries. Indeed it is a sad story, there is evidence of medical negligence in the way the hospital did not attend to a pregnant woman in a timely fashion who was obviously in distress. She should have been admitted to a high risk pregnancy unit and the consultant obstetrician on duty should have attended to her immediately. She should have had a doppler scan of the legs performed to rule out an embolus. Emergency radiology cover should have been available to perform any scans deemed necessary. The 'Oga is not on seat' mentality should not be applied to hospitals Unfortunately Nigerian hospitals are not really set up to provide 24 hour round the clock consultant led care, the public simply can not afford it for a start and there are not enough consultants or specialists to provide this service. So we have to cope with what is available. That is why many go abroad to have their babies. It sounds to me like the Lady had a thromboembolic episode which lead to a pulmonary embolus and cardiorespiratory arrest from the history given but alas i was not there so i do not know for sure. It is also possible she might have been given a painkiller which suppressed her breathing but any good hospital should be able to remedy this situation with oxygen and the the right drugs to reverse the effects. The hospital doctors should have had a meeting with the family to explain events and note their concerns. Maybe there would have been closure especially if the death was sudden and unavoidable as sometimes the family may not understand the situation fully. Good communication skills always helps defuse tension in situations like this. If the family is still aggrieved they should request an audience with the management and senior doctors. Unfortunately we don't really have the ethic of accountability ingrained in us as Nigerians. However, my prayers are with the family and i hope they learn to cope with their loss and console themselves with the arrival of the new born child |

These public officers constitute just 0.013 per cent of Nigeria’s population. They include 108 senators who each make over $1.7m a year. That alone is $183.4 million (N28 billion). Then the 360 members of the House of Representatives each takes home over $1.2 million, which amounts to $432 million (N65bn). Again, each state governor collects an average of N200 million naira a month just as security vote. In a year, they each get N2.4 billion naira. So, our 36 governors take home N87 billion naira on security votes alone every year. Add our 38 ministers and ministers of state, 100 plus heads of federal and state agencies, over 432 state commissioners, 774 local government area chairmen or caretakers, almost 10,000 councilors and you will understand where the N1.126 trillion goes.
