It is a 5-year program (October, 2022 to September, 2027) to be implemented by Caritas Nigeria in partnership with; Catholic Relief Services (CRS), Vantage Technology and African Disease Prevention and Research Development Initiative (ADRAP) as the sole Implementing Partner in these states. Job Summary: The successful candidate will primarily ensure data availability in all provided platforms such that stakeholders are able to take data guided decisions particularly in reference to CBO efforts. The person will work with all service delivery focal persons to ensure that quality documentation in all provided national tools (electronic and paper-based) are carried out. He/She will manage the CBO database and be able to update reports to reflect the status of supported clients. He/She will work with the CCFN SI team to keep track of service delivery to CBO led clients through prescribed reporting platforms. They will additionally ensure the availability of quality data through regular scheduled data verifications and validations activities. S/he will ensure the availability and use of Program Monitoring and Evaluation tools. S/he will assist in the general implementation of SI deliverables in the CBO linked health facilities and contribute to the overall success of Caritas Nigeria’s objectives Key Areas of Accountability. Coordinate M&E activities for the CBO to ensure appropriate standards are established for data availability, quality, and general management. Monitor and ensure appropriate service documentation into the HIV/AIDS service delivery tools to be utilized by CBOs are achieved routinely. Ensure that medical record of all patients who are provided services daily are updated into the EMR. Ensure complete entries into the EMR of HTS and Treatment indicators. Assist with the generation and sharing with the program staff, clients’ appointments, missed appointments and IIT lists Participate in monthly Quality, Monitoring & Evaluation meetings to improve documentation and program data quality and reporting including data validation with other CBO technical staff. Generate and submit weekly and monthly program reports as required to CCFN and CBO leadership. Perform oversight functions over the DEAs Manage any special request from the facility or implementing partner (CCFN) as regards patients’ data or reports following the appropriate protocols. Responsible for PMM tools quantification and requisition. Participate in data abstractions and data entries during quality improvement activities. Conduct monthly documentation audit of folders and registers and flag areas that need improvement to the QI backstop. Ensures review of facility performance indicators by the team. Responsible for logistic support activities which include prompt supply of PMM and PME forms and registers in the course of project implementation to prevent of stock outs of same. Participate in relevant trainings, coaching sessions and other forums that increases job knowledge and improves overall quality. Participates actively in Data Quality Assurance (DQA) activities and other data quality and validation activities to ensure our data meets standards set for quality. Draw up weekly list of patients for VL test expected at the facility irrespective of appointment dates. Call out to patients (phone calls) to remind them of a VL test Ensure list of expected patients is domiciled on clinic days at every point within the CBO to ensure patient is identified, notified and directed to the Lab for sample collection. Alternatively, you can pull their folders a day before and file a blank lab form in them as a 2nd step reminder Liaison with all POS within the CBO to ensure all clients eligible for VL draw are identified and are linked to the lab. Ensure that a the VL requisition form completely filled before blood draw. Ensure that blood is drawn for VL test for all those who present at the laboratory. Make a list of patients who missed the scheduled appointment to reschedule for the VL draw and share with the VL champion. Follow up on the patients that will be eligible for repeat VL after EAC and ensure that blood is drawn and shipped to the reference laboratory Provides weekly and monthly updates and reports of VL activity using prescribed templates for laboratory Notifies appropriate authorities of VL commodity shortage or stock out. Ensure that all new patients identified and eligible are offered recency and AHD testing. Responsible for completing and regular updates of the ART register. Carry out nurse refill services to eligible patients in conjunction with the supervising physician. Visit cluster locations to provide ART services. Serve as liaison between the health facilities and CBO on clinical related services. Ensure prompt referral to facilities whenever indicated. Ensure that all clients on refill get evaluated at each refill visit, document findings in the ART Care Card and the nurse refill registers and reports any abnormality to the physician. Ensures all clients have access to nutrition education and counselling at each visit. Follow up on client’s nutritional plan. Provide technical support for integration of the project’s technical approaches for the achievement of successful project implementation using evidence informed, innovative and best practices. By Obio Monday Seventeen passengers were crushed to death in a fatal accident involving an 18-seater bus belonging to Akwa Ibom Transport Company (AKTC). GRASSROOT REPORTERS gathered that only one passenger survived the accident which took place last night, January 20, 2023. The only survivor of the accident, Idorenyin Daniel, said she sat in the front seat with driver but the driver could not make it. Eyewitnesses told our correspondent that the bus lost control and summersaulted several times. Most of the victims of the accident had their body parts severed. Blessing Ebodo, a nurse from Bekwarra Local Government Area of Cross River State, lost her life yesterday in a terrible car accident along Calabar-Ikom Highway in Cross River State. GRASSROOT REPORTERS gathered that two other occupants of the vehicle survived the accident with injuries. Lamenting his loss, Blessing’s husband-to-be, Bernard Ushie wrote on Facebook: “Dear Blessing 27th is close and our cloths have arrived from Aba I’m waiting for you to return from Calabar so we can conclude the plans. You know this marriage will be top notch . “Please do not leave me on this dark place.” Another of his post read: “After the lost (sic) of my only sister I found you a perfect match and feels more comfortable being with you. You gave me love,care and attention more than I expect. You were a strong woman A prayer warrior A dedicated woman to the nursing profession I’ll forever miss the love of my life. The in incident took place on Monday, October 24 at about 10am. A video obtained by GRASSROOT REPORTERS shows the man who was accompanied by his girlfriend shoplifting about four human hairs and hiding them inside his private part. While his girlfriend was apparently distracting the salesgirl, the man was seen monitoring the salesgirl's movement and stuffing the stolen items in his underwear. The shop owner, Janet Odah expressed sadness over the wicked act. She said: "These two came to my shop this morning 10am and stole hair worth N621,000.00 from me. This is wickedness. Reports reaching GRASSROOT REPORTERS says persons, including elderly women were beaten to death by youths from Ndon Nwong community in Odukpani Local Government, Cross River State. Sources told GRASSROOT REPORTERS that the victims were accused of being witches and were taken to the village square where they were beaten to death. The incident took place on Thursday evening, October 13. One of the victims was identified as a widow, Mrs Asuquo Iya. Her husband was said to have passed on last year. A relative of one of the victims who spoke with our correspondent, called on security operatives to ensure that the perpetrators are brought to book. GRASSROOT REPORTERS correspondent reports that the man who hails from Okrika in Rivers State was arrested by a soldier who handed him over to the Lagos State Police Command. The who soldier discovered that he was a man at one of the hook-up joints in Ikeja, accosted him. He immediately took to his heels when he discovered that his identity has been uncovered. When interrogated, he admitted that he is a man. He said he needed money to pay for his mother's hospital bill, adding that his mum who is in Port Harcourt had an accident which required an operation. He said he is based in Lagos and usually plies his trade at night by standing by the roadside, waiting for 'customers'.
