No I dont. Its a boldface, shameless cover up and an insult to Nigerians that requires first, the head of the DG of NIA on a platter.

The emerging truth is that both the NIA and the DSS can be used at whim by people of questionable character to checkmate the EFCC.

It is unclear for whom in the executive the DG has chosen to be an errand boy all the speculations not withstanding.

While I concur there should be no sacred cow in the fight against corruption, the reality of our situation is that corruption has eaten so deep that should we remove it in one fell swoop the whole State could come crumbling down.

For those agitating for a commission of enquiry, to what end exactly? Opportunity for more mudslinging and distraction? For how long will they seat and what will it cost the tax payer? EFCC found the money, let them investigate.

A few scapegoats is what we need to fight this corruption and Nigerians will behave.

It's quite sad that things have gotten to this level in our health sector and if nothing is done quickly to arrest this ugly trend, we will witness a disaster in our teaching hospitals in the next 5 years.

Let me analyse a few points to make you see the way things are going.

1. we feel we have enough doctors as many are looking for residency placements, this is due to the fact that close to ten years the average medical graduated 150-200 doctors annually with few residency slots. With the current NUC directive , no university is allowed to admit more than 100 students into medicine.

2. Out of these 100 freshmen ,probably 70 will graduate as some will be withdrawn and some will repeat classes along the way.

3. Out these 70 fresh doctors, at least 60 will sit and pass the foreign medical exams to relocate to better countries.

4. The present resident doctors are also reading for these exams to get out of the country after going through all the frustrations of our system here.

5.This is a trend i have observed over the last 5 years being in physician in practice myself, no doctor particularly the ones who finished in the
last 5 years are willing to hang around and work here, we have colleagues even going to Lesotho were they will earn at least 1.2 million naira
equivalent monthly as a start.

6.In my set which finished 11 years ago, half are no longer engeged in anythng mediciine,some have gone into ministry, oil and gas ,telecoms etc., less than 25% of us still practise medicine while some have gone into lecturing medical students.

I just pray that what I forsee doesn't come to pass as it will quite a calamity.
Your analysis is right but at the same time very wrong.

Doctors' emoluments have been an issue for ever but you have by far more doctors seeking residency placements today than hospitals can cope with. And that's a fact. Indeed some have gone abroad but they found that passing these exams no longer guarantee them placement if they lack other grounds to stay, like family ties, marriage, citizenship status etc.

Have you asked your self how many of them are actually in practice out there? Many end up in psychiatry and other fringe specialties indegent doctors do not want. An invisible glass ceiling is automatically placed above your head and you realise that most of the patients you are allowed to see are those of your own kind, a small subset of the population.

Yet others damn fulfilling career and glory for the extra change as contract doctors in backwater republics.Why else would any Nigerian doctor go to Lesotho?

The point I am trying to make is that solutions to these problems lie within the confines of this country. Possibly in reasoned discourses like this and not in scaremongering tactics that there will be no doctors left to look after the citizenry.

This argument is dated and should be reserved only at the negotiating table for doctors' pay rise which are emergency quick fixes that has failed to work.

The inefficiency and rot in govt hospitals are stunting devt in private practice where there have been a few glimmers of hope.

Govt should waste no time in selling out these hospitals to the highest bidders and restrict itself to regulation and ensuring standards.

Secondly, they should ensure that every Nigerian is covered by health insurance.

In the meantime govt should allow trained and seasoned hospital administrators man its hospitals in preference to medical doctors who were employed to see patients and then suddenly rewarded with headship of multi million dollar establishment they are ill prepared and ill trained to run. These hospital heads are mere beneficiaries of the age old fight between doctors and paramedical staff. Most of the time, they are arrogant, stubborn and inept like we see in UBTH.

@fostermd thank you for taking me on in respect of this Medical Art Centre incident. I scan-read most topics not paying attention to a whole lot but I have gone back to read the hospital press statement and defence in this Unfortunate incident and I can't help but conclude that it was an unmitigated disaster.

Clearly it's a new hospital with young consultants

They attempted to hide their incompetence under consultant and international nurse toga. This is 2017. U take responsibility for your action not title.

Infact they called consultant so many times it almost made me puke. Another infantile peculiarity with Nigerian medicine.

Worst of all it is implied that the husband massaging the wife's abdomen after surgery caused the bleeding. For a dedicated client who almost opened the clinic with the founder this is an outrage and a stab in the back.

When lies are told about Nigerian victims of doctor's incompetence and hinged on illiteracy and tribal practices of the victim it often gains traction, but this is an educated white man. In any case massaging a uterus even after surgery will cause contraction and stop bleeding. Drugs are routinely given for this purpose after surgery.

The autopsy result, death from retro uterine hematoma is curious and need to be revisited.

The press statement again reflects inexperience on the part of Clinic operators. They should maintain complete silence and wait for time to heal. Any defence at this time will simply exercebate their plight. It's a new clinic with young doctors and a lot of prospects. You gain nothing by starting a fight in your own party just to prove you are strong.

Can you recall that at the early stage of DIC ,extensive clotting of the small vessels lead to multi organ failure before the platelets are used up and bleeding from multiple orifices occurs.The multi organ failure cannot be treated in Nigeria.Most hospitals do not have well equipped ICU,do not have life support or ventilators and trained Internist.
In addition,whole blood transfusion is not the appropriate replacement in DIC,what is needed are the components of blood that will stop the bleeding.Whole blood transfusion with high risk of incompartbilities causing transfusion reaction is implicated in the aetiology of DIC.Do you wonder why we are not able to treat many of the blood disorders,haemoglobinopathies and blood cancers in Nigeria?

I maintain extensive contact with doctors and colleagues who are consultants in Nigeria now and I listen to their frustrations.The underdevelopment witnessed in every sector of Nigeria is worst with Healthcare due to the fact that healthcare is expensive.Do you know how many Nigerians die from bloodl loss and hypovolemic shock following Road accidents and trauma?

The hospital may however have failed if the index of suspicion of the diagnosis was poor by the team managing her.Inadequate critical care facilities is the order in our healthcare system.

This incident is a failure of the healthcare system.Its the reason I relocated abroad since 2005 to have a fulfilling practice in Europe.
What you have described is a summary of pathophysiology of organ failure in DIC that any medical student should know well. But you are in error in assuming that all cases will result in multi organ failure 'early on' which is where you missed the boat.

As a matter of fact they don't. Even when there is organ failure they are usually reversible. It is not a case any doctor would like to have but it is also one of those cases where a sound clinical mind and good judgement is as important maybe more important than machines. The question is did these doctors exercise that judgement?

I think not. She was taken back to theater apparently to ligate bleeding vessels 24hrs after surgery with blood oozing from everywhere from the accounts stated. It seems to me they were confused. Some of us enjoy what we do. Unsung but heroically saving lives everyday. And we love it here. There are good and bad doctors everywhere even in Europe and America. Nigeria certainly has its share

I practice medicine UK in the past 12 years and 6 years before relocating,I still monitor the state of healthcare in Nigeria.DIC cannot be prevented as such but the management is where there is deficiency in Nigeria.The appropriate blood products/ fraction (Platelets) that is needed to stop the bleeding is not readily available.Whole blood transfusion is not enough to treat DIC ,most hospitals in naija do not even have adequate blood when needed let alone the various components of blood that are needed to treat specific blood disorders.How many laboratories have the capacity to seperate the component of blood.Healcare is practically non existent in Nigeria,that is the reason 1.5billion dollars is spent by Nigerians on health tourism annually to India,South Africa,U.K.,US and Recently Dubai.
Diagnosis in this case is not in doubt very clear. DIC possibly triggered by massive intraoperative bleeding from poor technique and experience 6 units of fresh whole blood could have stopped this disaster. Maybe 8 . More is hardly ever necessary. Relations and paid donors are always ready and willing to donate. We do quite well without ffp even though the amount of money charged this patient should make all options available at finger tip. Patient and family deserved better.

This lady died as a result of Disseminated Intravascular Coagulation(DIC). it's a very fatal complication of Caesaerean section especially when the pregnancy is post dated.It presents as uncontrollable bleeding from all the orifices in the body.
Treatment is almost impossible in Nigeria at the moment (clotting factors and platelets have been consumed leading to bleeding)because there is failure of the clotting system as soon as DIC cascade is set up no matter the amount of blood transfusion the patient gets.it result from post partum haemorrhage or amniotic fluid embolism (following Caesarean section)
The history in this case indicated bleeding from different part of the body and despite the blood transfusion she did not survive.
May God comfort her family.
I don't know the kind of exposure you have had but no one should die from DIC following CS in today's Nigeria. This was an elective surgery for christ sake. These doctors could have done so much more.

I know nothing about this practice but they should have a reasonably good background to command that kind of fee for an ordinary CS. It is obvious that whatever internal controls they have failed miserably. And for this they should be remorseful and not be seen to be exchanging words with an aggrieved husband.

Death of a mother following delivery is always extremely painful both to family and care givers. Sometimes for the doctor it's a career defining experience. I hope the doctor will learn from this and make sure it never happens again. It's a collective indictment to medical practice in Nigeria.

So if you and your wife begin to moan loudly in the middle of the night, it cannot be a problem for her because that is exactly what married people sometimes do in the middle of the night-moan.

It would seem the writer of this piece is on a personal vendetta against the school authorities going as far as advocating for a criminal enquiry.

He proposes some kind of remedial measures without saying a word of what this should be.

I doubt if this borehole was recently sunk. Why didn't the kids have these health problems before now?

From all indications the school has been closed by the State Ministry of Health. Odinkalu is playing the role of a meddling interloper.

His write up sheds no light. It brings only heat.

Reports of water pollution and occasional ill health are not uncommon in schools and communities across Nigeria. This is not the first and certainly won't be the last irrespective of whatever measures are adopted. Should the govt declare a national emergency as requested? Then it should be prepared to do this every other month. This shows naivety and ignorance of how to conduct state business.
