Topping the list of my crazy food ideas today, I present to you, cheese on ‘toast’. What’s special about that I hear you ask? Well, the ‘bread’ is actually a tin of tuna blended with an egg & a bit of baking powder, microwaved then toasted… witchcraft *Tastes neither overly fishy or eggy. Can also use cooked chicken but I had none. Think tuna melt panini* foods 1 tin tuna (well drained) 1 egg 1/2 tspn baking powder Pepper Can add herbs / garlic powder etc but I got giddy & forgot Blend thoroughly Takeaway container Microwave 4 mins Tip out, cool slightly, slice in two Toast both sides Add cheese Get bubbly Scoff 1. Smoked salmon and asparagus tortellini pasta Smoked salmon and asparagus tortellini pasta with Creme fraiche and crispy salmon skin, tell me that isn’t gorgeous. I dare you! 2. Cheese, pepperoni, spinach, ricotta, mushroom and baked bean pizza My midnight snack! Made with my own bare hands! No takeaway rubbish for me! Cheese, pepperoni, spinach, ricotta, mushroom and baked bean pizza! You’re all so jealous of this masterpiece of culinary perfection aren’t you? Someone actually saw this delicacy and wrote: “Looks like you went out on the lash last night, came back, puked up your beans on toast you had for lunch that day on top of the pizza you were about to cook, placed it in the oven and then ate it.” LOL! Last year my mum went visiting our family doctor and while discussing he checked her lipid level. It was very high but she never knew, she was very healthy. She was placed on medication and exercise. I am sure you must have seen this picture before, the picture shows blood sample collected from a patient. The upper portion you are seeing is (fat), it is very evident that this person had very high levels of fat in their blood including cholesterol. We should however be informed that one can inherit this kind of condition through genetics, others get it because of their lifestyle. This patient with the above blood sample is suffering from a condition called hyperlipidemia and it is treatable, but if not treated on time could lead to serious complications that could result to heart disease. There are foods that contain high amounts of cholesterol and fat that can contribute to this condition. They include: Ice cream Cheese Fried foods Ice cream Processed foods Red meat Cakes, etc. Some people think that eating these type of food is to display their level of wealth and riches. May wisdom fall on them. You should check your lipid levels, talk to your doctor about it. You can come across a lot of colleges of applied arts and technology that offer part-time or full-time certificate and diploma courses. Several of them also provide Bachelor degrees in applied areas of study. As far as a diploma program is concerned, it lasts 2 or three years and a certificate program normally lasts for 1 year or less. You can study courses that focus on skills upgrading, language training and pre-trades and apprenticeship training in Canadian colleges and there are institutions that concentrate on military programs, health science and agriculture as well. Kolpophobia is an anxiety disorder characterized by the persistent irrational fear of genitals especially female genitals. Genitals or Instruments are the external reproductive organs and in the females it is known as the vulva and generally called the Vagina. Kolpophobia is a Greek word derived from “Kolpo” meaning vagina and “Phobos” meaning fear. Like other Specific phobias, someone experiencing this phobia undergoes intense anxiety and stress triggered by the thought or sight of the female genitals and this can affect one’s confidence and ability to interact and relate with females. Kolpophobia is synonymous with Eurotophobia which is the fear or aversion of female Instruments. It is also related to Phallophobia which is the fear of an erect penis. Kolpophobia can affect both males and females and some persons develop this phobia because of the discharge or mucus associated with Instruments. Phobias are more severe than the normal fear sensation one feels. A person suffering from a phobia knows that their response is not rational but is unable to do anything about it. Studies have shown that twins raised separately are likely to develop a similar phobia Traumatic experiences: Rape or sexual abuse by a female perpetrator can leave an indelible mark on a survivor especially when this occurs at an early age. This can lead to several phobias, one of which may be koplophobia. Changes in brain function: This can be occasioned by brain injury leading to a behavioral change. Changes occasioned by this organic disease can lead to the development of a phobia TREATMENT OF KOLPOPHOBIA Cognitive-behavioral therapy: This is a type of talking therapy that aims to change the way you think and act. It is a psychotherapeutic treatment that is based on the theory that thoughts, beliefs, actions, and moods are interconnected and as such, changing negative thoughts associated with a phobia can change one’s reaction to a trigger. Together with the therapist, a person undergoing this therapy, takes a holistic look at the problem and breaks it down into separate parts. Negative thoughts that are identified are challenged and then these thoughts are counteracted with realistic thoughts. Patients with Kolpophobia can also benefit from this therapy. Other types of therapies include Dialectical Behavioural therapy (DBT), mindfulness-based therapy which makes you aware of your current feelings, and using various techniques like meditation and deep breathing helps you accept and manage your emotions. Exposure therapy: This is a type of behavior therapy also known as shock therapy or imaginal flooding therapy has been proven to be the best therapy for phobias. It is a form of retraining for the brain to dissociate the fear from the trigger and change the response to it. It a form of conditioning that involves gradual repeated exposure of the patient to incremental doses of the trigger to reduce the intensity of anxiety response elicited. For a patient with Kolpophobia, continuous exposure to photos of females can help in conditioning Hypnotherapy: It is a form of alternative medicine that utilizes guided hypnosis to reprogram the mind. Under hypnosis, a patient is relaxed, calm, and has a heightened state of awareness. This can be utilized to explore psychologically distressing issues like phobias as well as to create a positive change through suggestive therapy while the patient is in this trance. Hypnosis is done by a therapist using mental images and repetitions in a calm assertive voice. I told her 4 days and she said I was wrong. 4 days is the duration of my menstruation also called period or menstrual flow which can last from 3 to 7 days. The cycle on the other hand is calculated from the first day of one menstruation to the last date before the next one. It varies from one woman to another but usually ranges from 21 to 35 days with an average of 28 days. She brought a calendar and told me to try to recall the first day of my last period. The good thing is that I remembered because I was stained badly on that day and had to go to the matron to ask for the hostel key during school hours, it was on the 23rd of March. If fertilization does not occur, the lining of the uterus is shed. This shedding is known as menstruation. The menstrual cycle is an interplay of hormones controlled by the Hypothalamus and Pituitary glands in the brain. It has about 2 key phases separated by the ovulation. The first day of your menstruation is considered the first day of the new cycle and the follicular phase. During your menstruation, the hypothalamus sends signals to the Pituitary to produce the Follicle stimulating hormone (FSH). This hormone causes 5 to 20 follicles in the ovary to start developing. This is the follicular phase. A follicle is a fluid-filled sac that contains an immature egg. Maturing of the egg causes an increase in estrogen level which causes the lining of the uterus to thicken in preparation for implantation and also causes the Pituitary to release Luteinizing hormone (LH). LH when it reaches the ovaries causes most developed follicle to burst, releasing the egg in it. The release of the egg is called Ovulation. The now empty follicle called the Corpus luteum continues to secrete estrogen along with the other follicles but also secrets Progesterone which prepare the uterus for pregnancy. This second part of the cycle after ovulation is known as the Luteal phase. This released egg is viable for about 12 to 24 hours during which fertilization can take place if it comes in contact with sperm cells. If the egg is not fertilized however, the egg and corpus luteum die along with the other follicles that developed, the levels of Progesterone and estrogen fall and the uterus lining shrinks and shed off. All these, together with blood and fluid are shed out of the body as menstruation. Ovulation is usually characterized by slight increase in body temperature, breast tenderness, bloating and a thin, clear, slippery, Vaginal discharge that looks like raw egg white. This discharge is cervical mucus signaling fertility. It usually occurs about 14days before the start of the next menstruation. So, for a 28day menstrual cycle, ovulation will be in the 14th day which is the middle of the menstrual cycle and for a 24day cycle, ovulation is most likely to occur on the 10th day. This is what to do when you have a choking baby and the hospital is far away Please note what I said above, take these steps when the hospital is far away, it is almost the same thing the doctor would do when you get to the hospital. Now follow the steps below; Step one: Call for help! This is very important, because no mother or anyone whatsoever would see a choking baby and not fidget. But, when you have people with you the tension becomes lesser. In calling for help you must scream as loud as you can, because it is not the time for you and the neighbors to keep social distancing. Choking kills faster than covid19, that child could die if nothing is done fast. Step 2: When the victim (choking baby) is a baby under the age of 1 (one) year do this; Gently Lay the baby face down across your laps and start using the heel of your palms to give gentle back blows. Please do not hit the baby as though you are trying to save an adult so you don’t end up breaking the baby’s spinal cord. What you are doing will stimulate a vibration that will cause an upward movement and dislodge the object the choking baby had swallowed. Please ensure to use the heel of your palm, and note this might not work in the first attempt. Step 3: If the procedure as highlighted in step 2 does not work, take the choking baby, lay the baby face up across your laps and locate the breast bone, this is the bone in the middle of the chest. Step 4: Now you have to put two of your fingers in the middle of the chest or breast one and initiate an upward push five times very sharply! You push as though you’re pushing upwards and inwards. Note that while doing this with full concentration you must still be calling for help if none had arrived. Now if the choking baby is a toddler this is what you should do. Step 1: You Kneel closely behind the choking baby and wrap your hands around the child’s stomach between the navel and ribs and pull sharply towards you upwards and inwards as you remain behind the child. While you are at this is, you are making your way to the hospital. Keep doing these things on your way to the hospital. That is why I said, call for help. What not to do when a baby is choking! NEVER LEAVE THE CHILD ALONE! You must never leave a choking baby alone for any reason. You might want to run out and call for help, but do not do that; you must ensure you have eyes on the baby. DO NOT PUT YOUR FINGERS INTO THE MOUTH OF A CHOKING BABY IF YOU HAVEN’T SEEN THE OBJECT! I have seen this happen so many times, nursing mothers trying to force their hands into the choking baby’s mouth when they don’t have vision of the object. Please desist from doing this. It can be caused by several factors although 90% is from issues arising from the mouth which may be; poor oral hygiene, dry mouth as a result of reduced saliva production, gum diseases or presence of halitosis-causing bacteria on the tongue. Eating some foods with a pungent smell like garlic or raw onions and use of tobacco and other underlying medical conditions like Gastrointestinal reflux disease (GERD), postnasal drips, diabetic ketoacidosis and lung infections have also been implicated. Morning breath is a type of mouth odour experienced as soon as a person wakes up in the morning but it usually disappears after drinking water or eating breakfast. Proper identification of the root cause of the odour will facilitate the treatment choice. Tooth decay, also known as dental caries or cavities is one of the most prevalent non-communicable diseases in the world. Tooth decay can give halitosis-causing bacteria space to fester, causing mouth odour. When food is taken into the mouth, it is chewed with the teeth and mixed with saliva. The saliva contains salivary amylase which is an enzyme that starts the process of digestion of sugars and starch in food. Starch which is contained in large quantities in many staple foods like rice, maize, yam, cassava and potatoes is broken down into glucose. Particles of food and debris remain on the surface of the teeth. Bacteria in the mouth use these sugars as their energy source and in the process, the produce acids which destroy the hard tissues of the teeth. Within 5 months, the outbreak of one infectious disease (Coronavirus) has become the most significant event in the world. This pandemic has affected economies, families and even individual lives, schedules of properly planned and seemingly unchangeable events like the Olympics have been adjusted to curtail the spread of this disease. Amazingly, this highly infectious virus which originated from Wuhan, China in December 2019 was underestimated by all hence, response to it is mostly reactive. A cluster of patients with severe respiratory illnesses was identified and using the real-time reverse transcription–polymerase chain reaction (real-time RT-PCR) and next-generation sequencing laboratory techniques, the novel coronavirus was isolated. INFORMATION ABOUT THE VIRUS/MODE OF TRANSMISSION Coronavirus is a family of viruses characterized by crown-like protein spikes on their surfaces when viewed with electron microscopy. They cause various mild to moderate respiratory tract infections like the common cold. Within the past two decades, three new coronaviruses have emerged causing severe illness: SARS- CoV causing Severe Acute Respiratory Syndrome in 2003, MERS- CoV causing Middle East respiratory syndrome in 2012 and SARS-CoV-2 causing COVID-19 in 2019. Coronaviruses are found in some animals which serve as reservoirs but sometimes can spillover to humans causing illness. The animal reservoir of SARS-CoV-2 is still unknown. It is known as at today that the coronavirus is transmitted from person to person through droplets which are usually released when one coughs or sneezes. These droplets can enter directly through the eyes, nose or mouth which are the portals of entry into the body. They can also fall on surfaces or objects where the virus remains active and can be contacted by touching with the hands and then transferred into the body by touching any of the named portals of entry. The incubation period is from 1-14 days and symptoms range from fever, cough, tiredness, in mild cases to shortness of breath, pneumonia, and death in severe cases. COVID-19 is highly infectious with the number of cases expected to be generated by one case or basic reproduction number (R0) of 2-3. Transmission can be from a patient who is symptomatic or pre-symptomatic; that is even before being symptomatic. The investigation is still ongoing about asymptomatic transmission meaning a seemingly healthy individual can be a carrier and distributor of the virus. THE EFFECT OF COVID 19 IN NIGERIA In preparedness of the advent of the virus, on the 22nd of February 2020, as part of a continental initiative to rapidly detect and treat Covid-19, ministers of Health of African countries including Nigeria came together to form the African taskforce for Coronavirus preparedness and response AFCOR. The Director General of the NCDC also visited China to understudy the management strategies being used in tackling the virus. The health system in Nigeria has been rated very low with a lack of infrastructure and medical equipment, high brain drain of medical personnel leading to a shortage of medical professionals and specialists. The few professionals remaining did not have any point of unity especially as a rivalry between doctors and other professionals has been on. In the heat of this, however, the National Medical Association (NMA), Pharmaceutical Society of Nigeria (PSN) and National Association of Nigerian Nurses and Midwives (NANNM) found a way to come together to put out a statement in support of their members at the frontline. This was also at the backdrop of patients hiding their travel history thereby infecting the unsuspecting health workers attending to them. The Nigerian medical association was also strongly opposed the import of Chinese doctors into the county to respond to Covid-19. Research is also being carried out in Nigeria as the Nigeria Institute of Medical research, African Centre for Genomics of Infectious Diseases in Redeemers University, Ede and Centre for Human and Zoology Virology in Lagos University Teaching Hospital on the 6th of March successfully performed the genome sequencing of the coronavirus strain from the index case and proved it to be a match with the virus circulating in Italy and Wuhan. The National Agency for Science and Engineering Infrastructure (NASENI) also developed a ventilator prototype that can be manufactured for use in the fight against the virus. MANAGEMENT OF IMPACTS OF COVID IN NIGERIA Being that there are several aspects to the Covid-19, management strategy target various aspects of the disease. These strategies are done at the individual level and at the level of the government. At the individual level, Maintaining high hygiene standards, change of habits and other preventive measures are the responsibility of each individual for instance, regular hand washing, avoid touching your face and so on. Self-isolation in a properly aerated room on return from a foreign trip as well as when feeling unwell Calling the authorities and informing them when exhibiting the symptoms of Covid 19 and giving complete medical history to help the health professionals manage the cases better. At the level of the government: In Nigeria, the Presidential task force is responsible for coordination of response to Covid 19. Government level management system involves: Surveillance & port entry screening: This consists of contact tracing for confirmed cases as well as active case finding in the community. Laboratory testing and subtyping. Risk communication and community engagement, Infection prevention and control Clinical management: This involves isolation and Individual Case Management: Each suspected case is treated symptomatically using supportive care. There is currently no specific drug or vaccine. As part of the government response to prevent the further spread of Covid 19, there is a key Non Pharmaceutical intervention which has been implemented. This public Health social measure involves closure of borders into the country, restriction of movements, lock down of some states with high number of cases as well as curfews in some states. This lockdown measure demands that citizens stay at home in order to reduce person to person interaction and spread. The major consequence of this is that people who cannot work from home and who earn their daily subsistence cannot work or earn leading to hunger and frustration. In Nigeria, a large proportion of the population falls into this category. The Federal government in order to reduce this economic impact on vulnerable families and businesses has introduced several palliatives and economic stimulus packages including A direct cash and food distribution. As part of the food to be distributed are 70000 metric tons of grains released from the Nigerian grain reserves and bags of rice seized by the customs to be released. The government also issued a three-month moratorium on Marketmoni, TraderMoni, and FarmerMoni loans. As an economic stimulus package, they have created N50 billion credit scheme for households and small- and medium-sized enterprises, N100 billion credit support for the healthcare industry and N1trillion credit support for the manufacturing industry Tax measures by the Federal inland revenue service have been instituted. Creation of 774,000 jobs has been proposed. Individual states also have their packages for their citizens. PREVENTIVE MEASURES Prevention of Covid-19 like any other infectious disease involves breaking the cycle of transmission. In this case, it is the transmission from person to person that is being targeted hence it is recommended that We maintain Physical distance of at least 1m from people coughing or sneezing Cover your mouth and nose with tissue when coughing or sneezing and dispose the tissue into the bin immediately after.
