Testing Your Baby for Sickle Cell During Pregnancy

Introduction
It is possible to test for the genes your baby has inherited.

There are two main ways of checking your baby for genetic problems while it is in the womb. These techniques are called amniocentesis and chorionic villus sampling (CVS). We explain how the tests are carried out, what sort of abnormalities can be picked up and what risks are involved.

We give general information only. If you want more detailed information about these tests or your personal circumstances do discuss the matter with your GP or hospital specialist.

Amniocentesis
What is amniocentesis?
The test involves placing a needle in to the womb through the woman’s abdomen (tummy) and obtaining a small sample of the fluid in which the baby floats (the amniotic fluid). Most women say that the test is not painful.

The amniocentesis test is usually carried out between 16 and 18 weeks of the pregnancy and can be done in an outpatients clinic without the need for an overnight hospital stay.

As part of the test an ultrasound scan is carried out to check whether twins are present and to see where the placenta and the baby are.

The fluid contains cells which have come from the baby and these cells are then grown in the laboratory so that genetic tests can be carried out on them.

What type of abnormalities can be detected?
Abnormalities caused by extra or missing chromosome material can be detected, the most common of which is Down’s Syndrome (trisomy 21), where babies have an extra chromosome 21 in every cell or their body.

Other chromosomal abnormalities can also be detected. The effects of all these conditions will be discussed in detail with the parents. Many of these chromosomal abnormalities lead to serious conditions and a termination of pregnancy may be one option for the parents to consider.

In some cases, other disorders caused by a single abnormal gene can be found. At present tests for these are usually carried out only when there is someone with the condition in the family of the couple is known to be at risk for a baby with a particular genetic disease, such as cystic fibrosis, sickle cell anaemia or Tay Sachs Disease.

The amniotic fluid itself is also tested to measure the level of a protein called alphafetoprotein (AFP). The level is high if a baby has spina bifida, and this test picks up 95% of babies with this disorder. A detailed ultrasound scan is then carried out to confirm the presence of spina bifida.

What are the risks of amniocentesis, and how reliable is it?
The amniocentesis test carries a small risk of causing a miscarriage, about 1 in 100, but other complications are very unusual.

The amniocentesis test is a very reliable way of examining the baby’s chromosomes, but in order to obtain results the cells must begin to grow in the laboratory. Occasionally this does not happen and a further sample may be needed.

When is the result available?
Within three to four weeks if an abnormal result is found and the couple decides to have the pregnancy terminated, this could be done at around 19-20 weeks of pregnancy.

Chorionic Villus Sampling (CVS)
What is chorionic villus sampling (CVS)?
The CVS test is carried out earlier then amniocentesis – at around the 9th or 10th week of pregnancy, and again it is an outpatient procedure. Rather than testing a sample of the fluid of the mother’s womb, this test take a very small amount of material from the developing placenta.

One way of doing the test is to place a thin tube through the cervix (neck of the womb) to obtain the sample. Most women say that this is only slightly uncomfortable.

Another method is similar to the amniocentesis test, and involves placing a needle in the womb and using an ultrasound scanner to guide it.

What type of abnormality can be detected with CVS?
The sample contains cells that are of the same genetic type as the baby and so can reveal the same sort of disorders that amniocentesis discovers. Similarly CVS can detect rare diseases caused by abnormal genes, but thesetests will only normally be done if there is a family link with a particular disorder. Because no amniotic fluid is obtained the test cannot be used to look for babies with spina bifida.

What are the risks of CVS, and how reliable is it?
The CVS test has a slightly higher chance of miscarriage than amniocentesis, but it has the advantage of being able to be done earlier in pregnancy.

It is a relatively new test and there is not so much information on reliability as with amniocentesis. There is a suggestion that the CVS test may give a confusing result in a small proportion of cases, maybe one in 100.

In such cases it may indicate an abnormality where none exists. Although there is usually additional information which clearly demonstrates that the baby will be abnormal, where doubt still exists it may be necessary for an amniocentesis test also to be carried out later in the pregnancy.

There is also a remote possibility – around 1 in 1000 cases – that a chromosome abnormality will not be detected.

When is the test result available?
Normally just one to two weeks after the test.

Other information
What about future pregnancies?
If a couple do decide to terminate their pregnancy after either an amniocentesis or a CVS test, this should not affect their fertility in future pregnancies. The couple may well wish to be referred to their genetic clinic to discuss the risk of an affected baby next time.

Rhesus Negative Mothers
If a mother’s blood group is rhesus negative, she should receive an injection after an amniocentesis or CVS to protect the present and future pregnancies.

Testing for the Type of Haemoglobin the Baby Has
Testing an unborn baby to see which type of haemoglobin it has can be done using either of these techniques although it is usually done by CVS because more cells can be collected. Later in pregnancy it can also be done by taking blood directly from the foetus. The cells that are obtained from all of these procedures can be tested directly for the presence of the sickle haemoglobin gene itself.

The Governor of Rivers State, Nyesom Wike, has again declared the State as a Christian State, adding that he owes no one any apology.

The Governor spoke at the Redeemed Christian Church of God (RCCG) Holy Ghost Rally, which held at the Adokiye Amiesimaka Stadium on Sunday.

He said, “But for God, the enemies would have taken over the state. I thank the Church for standing firm.

“They prayed for God’s will to be done and God’s will was done. But for your prayers, I won’t be here as Rivers State Governor.

“Anytime I have the opportunity, I repeat the declaration with authority and I owe nobody an apology. I am standing on solid authority. Rivers is a Christian state.”

Wike said he remained Governor due to God’s intervention.

He said God dislodged “forces of darkness” who were primed to rob him of his mandate.

He described himself as a living testimony of God’s blessing.

“Nigerians are aware of what happened on 9th March 2019. If anyone says he does not believe in God, that event of March 9 should make you believe God,” he added.

What will I say, if not to laugh. Its unfortunate Rivers state been reduced to mediocrity. When we begin to celebrate failure, it then means we can not make progress. I mean, Rivers State ought to be competing with state like Lagos in terms of development. Rt. Hon. Chibuike Rotimi Amaechi still remain the best governor Rivers State has had since the return of democracy since 1999 to date. Look at the roads constructed by Amaechi during his tenure. What of the state-of-art schools constructed during his tenure? Health centres was built and equipped by Amaechi across the state .What has the current administration achieved apart from window dressing already constructed roads and schools built by Amaechi? Time will tell between Amaechi and the current governor who truly love Rivers state

Op, it depends on the lecturer or the institution concerned. That was the reason i actually stoped writing the chartered institute of stockbrokers examinations. My dear, i was in my final stage in 2012. Their pass mark is 50%. My dear in that year i wrote two papers. When the results were realised i scored 49% in the two subjects that i wrote. Instead of the institute adding just 1 mark to my scores, they instead failed me and asked me to re-write the exam. Since then i got offended and stopped writing

I don't think so, because mine was hacked by January this year. I tried all I could including reporting to Facebook, but up till now, nothing has been done by Facebook. The hacker changed my email and phone number. I don't even have access to my Facebook account again. So, I had to open another account. Even while I was opening another Facebook account with the same name that I used in my former Facebook account, I could not be allowed to do so. Each time I try, Facebook will require me to upload my picture, when I upload my picture, that is the end. I can not proceed further. So, I had to use a different name entirely before I was able to open another account

I suggest the National Assembly should initiate a bill that will standardise the marriage requirement throughout the country. This rubbish must stop. How can a particular community ask somebody to provide all these before they can give out their daughter in marriage. I sincerely suggest that a state of emergency be declared in the marriage institution all over the country. Since 2007 I married till now, I'm still paying debt due to this rubbish of a long list. Yet, after the lady has finally been married, her parents will still expect the man to assist them financially.

I know that, but its very surprising that a graduate cannot teach a primary 2 and primary 5 pupils. Even when the textbooks contains lots of solved examples. If she is an average student, i will understand. But the truth is that she is below average. Somebody that cannot write a formal and an informal letter, cannot send text message with a simple and correct english and does not even know her "safe period". In this era, a graduate cannot or does not know how to use the computer, does not know what Facebook, Whatsapp, etc is all about. I begin to imagine if the person is in this part of the world. Im complaining, and some persons are telling me that if i truly love her, i should over look all her faults. What kind of love are they talking about? A situation i will tell her to remove the clothe she is wearing and wear another one, and she ignores me. If its you, can you take such? I bought a car for her last year, she will also expect me to fuel the car for on every two days. She is expected to use the car to take my kids to school and bring them back, but she will use the car to run round the whole town and expects me to fuel it and my own car. If she was working or doing business, is she not the person to fuel the car? Must i be the one to do everything? My building project has stopped for over a year because no money to continue. We are still living in a rented apartment. The lady that you call your wife will not want to do anything to assist in the up-keep of the family. Of a truth, i married her when i have not gotten a job but she was not the person that contributed to my getting a job. We were dating when i was writting my professional exams (ICAN), if was reasonable and ready to learn, will she not have joined me seeing that we studied the same course in the university? Hey Toks, you and many other nairalanders are making generalizations and superficial simplistic assumptions without proper analysis of the facts presented by the Op.

I agree with you that the situation does not warrant divorce as it can be resolved. However, saying that it's largely the Ops fault may be a little narrow-minded.

The key issue to the Op is that his wife is intellectually inhibited and not motivated to improve herself. To this end, the Op desires some sort of continuous improvement in his wife to personally enjoy the marriage. There is no rule or law in marriage that stipulates that the interests of a particular family are important while those of another are mundane. Consequently, wanting your spouse to improve in a certain area is not condemnable by any standards.

In essence, the personal elements that the Op considers vital to a successful marriage may be different from yours or others. You cannot however limit or condition the Ops familial happiness within the context of your own personal preferences. That would be parochial.

By buying brighter grammar for her and pledging 1.5m for her business, the Op has actually taken concrete steps to enable her improve, but marriage is a two-way sacrifice. The wife should not be comfortable in being unprogressive, even in her own areas of interest. And her excuse cannot be child-bearing and cooking skills as some are insinuating!

The Op desires to enjoy his spouse and not "endure" or "manage" the lifelong situation as many Nigerian families are currently doing. Indeed I know of a few families in the Op's situation where the husband does not see an issue with his wife's actual or relative illiteracy or lack of ambition; and others whose marriages are threatened as a result or others who simply manage the situation by deriving satisfaction from external quarters. Saying his wife should be left stagnating is not healthy for the family development, especially when the husband and the kids are progressing.

In summary, the Op should be more diplomatic in dealing with her and try to win her over before actively participating in the developmental initiatives they both agree on. I agree with you on the dressing. However, the Op is right to insist that she learn under an experienced dealer to increase the chances of success at her intended trade. There is a reason why feasibility studies are done even if it is tomatoes you want to sell. Moreover, the Op is simply adding value to her from his own knowledge of financial management.

Don't bother yourself explaining too much. You must know that nairaland is full of children. Just read the matured response and move on. Sincerely i feel your pain. I had very similar experience with the girl I was planning to marry. After trying to encourage her to improve and she kept resisting, I let her go. Yours is different because you're married already with kids so try and forget about divorce, it's not a solution to your problem. There must be something you're wife is good at. Carefully identify her area of strength and build her up. On dressing I noticed that women that have poor dressing sence hardly improve while married but its not impossible. So you need to take her out from time to time and let her get some good clothes that suitable for her especially romantic night wears. You can also buy her some simple books regularly on different subjects to improve her spoken and written English, above all don't forget to pray that's if you are not like Seun of nairaland
My dear, thanks for your comment, but I must let you know that, in my house house, I have reasonable books that can help her improve her speaking skills, but she just dont want to read them. I even bought "Brighter Grammer" volume 1 to 4 and handed over to her but she would not read them. My library is full of different books that anybody can read.

Guys, you may not understand. Its not as if i want her to be extraordinary or super human over night, the truth is that she is not making any effort to improve. Remember the saying that says "you can only take a horse to river, but you cannot force a horse to drink water". God knows, I have tried to see that i "brush" her up to a certain level.

I have taken time to read all your wonderful comments, even though some are very insulting. For crying out loud, why will come online to discuss my family if I don't need a solution? I said in my post that my wife deliberately takes in anytime I remind her of what to do to assist in the family. Some of you are asking if I'm not the person doing it and why don't I use condom. For your information, my wife is the type that does not like her husband using condom on her. In fact, the day she saw condom in the house, she went to our pastor to report me that I'm using it outside. Her reasoning is that anybody with a condom is an "ashawo". On the other issue, it's not as if I never noticed her academic backwardness during our courtshithough my thought was that she will improve beside, she had over 2years extra for a course of 4years and by then we are already married. So, how could I have known that she will graduate with 3rd class? Believe me guys, I cannot lie against her on this platform. All I have said is nothing but the truth. Ok, tell me, will I stop sleeping with my wife? I have even complained to the sisters. I did not initiate this post to be rediculed, all I need is matured advice on how to handle the situation. Agreed, I made a mistake by marrying her in the first place, but it has already happened, what do I do? I'm not foolish or careless as somebody commented above.
