Well, when her cycle was 40days, it was an indication the ovary, whose tube was ruptured, was releasing the egg(s). That explains why conception wasn't achieved because the sperm cells couldn't find their way to reach the egg.

Fortunately, with the change in cycle from 40days to 31days, it might just be indicating the other ovary is now releasing its egg(s). But there might still be a problem. If the tube also has endometrial tissue, the zygote might lodge in the tube and cause another ectopic pregnancy.

Your wife's best option is ACM(Assisted Conception Method).

I stil don't understand why people don't pay complete attention to their health, and it baffles me. You may feel ok, look ok, seem ok, but might not actually be ok. Regular check up is advised at least 3 times a year.

Let's do things right next year.

A week after exposure is stil within the window period. The test would come out negative. Personally, I'd advise he be placed on HB vaccine, or antiviral as prophylactic, and hope for the best. The HBV screening should be carried out in 3months time!

HBV is more contagious than HIV.

Her painful urination is likely due to vulvovaginal infection, or vaginal contusion or tear, caused by em, em, em, I rather not air out my suspicion openly.

I'll advise you take your kid to see a family health doc, or a gynae, or a paediatrician for Vaginal Examination.

Let me ask, was that girl left alone with anyone, maybe a stranger? Did she stray out of the house to somewhere you guys didn't know about? Do you have a househelp living with you?

Well, i've never heard of or come across a 4yrs old with VVI, unless it was em, em, let me still keep it discreet. But then, since you want me to play dumb, I'll assume urinary tract infection is responsible for her dysuria. I'll advise you take her to a private specialist hospital(paediatrics) so she can be examined, sample taken in situ, tested, and treatment administered.

Let's just say your unusual anorexia, postprandial defaecation, and phagy, are connected to the peptic ulcer you have. You're losing weight because you're not eating right.

I'll implore you run a stool culture test, and gastroendoscopy.

As for prescription/treatment, treatment will follow after the outcome of the mentioned tests above.

Avoid acohol, avoid skipping meal, avoid very hot meals, avoid peppery food, avoid fried meals, and avoid unripe fruits.

The subserosal and the intramural fibroids are small enough in size not to reduce your chances of conceiving. The submucosal is the significant fibroid that would most likely affect your chances of conceiving because of it's size and location, which explains why you experince heavy menstrual flow.

The indication for surgery depends on the size of the fibroids. Fibroids larger than 5cm are removed surgically, hysteroscopic shelling if it's submucosal and lesser than 4cm, myomectomy if it's larger than 4cm and located either intramurally, serously, or subserously.

Your chances of conceiving without removing the fibroids(surgically) or shrinking the fibroids(medically) is very low.

Your chances of conceiving after removal/shrinkage(surgically/medically) depends on how careful the surgery is carried out. Hysteroscopic shelling, if not carefully done, may pierce the uterus. The myomectomy, after the incision, the fibroids removed, the stitching has to be carefully done so it doesn't lead to complications.

The cost of surgery varies, depending on the kind of hospitals.

But then, fibroids can be treated non-invasively. Some medications can actually shrink the size of fibroids. Fibroids are estrogen and progesterone dependent. Anti-estrogen and anti-progesterone analogs can actually shrink fibroids and curtail their re-growth.

Your irregular menstruation, with your inconsistent cycle, your lack of cervical mucus, all are indicating an extreme hormonal imbalance. Differentials include OC, PCOS, hyperprolactinaemia.

I'll recommend you run a hormonal profiling, then ultrasound to check the state of your ovaries.

The results will determine the next line of action.

Indication: Run a hormonal profile for FSH, LH, Prolactin, E2, and Progesterone, at cd5 for the first three hormones, and cd21 for the last two hormones. Then run a pelvic UltraSound scan to observe the ovary, particularly the right ovary

A matured follicle stops increasing in size after growing to 27mm, or in some cases like yours, 30mm. That explains why at cd15, it maintained the same size as that of cd14. The follicle should rupture anytime from cd16.

You should have begun having intercourse with your partner from cd14 till cd18. If you've not, then you should begin immediately.
