Gigantomastia or Macromastia is a rare condition that causes excessive growth of the female breasts. Only a 100 cases have been reported in the medical literature..

The exact cause of gigantomastia isn't known. The condition can occur randomly, but it has also been seen to occur during puberty, pregnancy, or after taking certain medications. It does not occur in men though.

The breast growth can occur over the course of a few years, but there have been some cases of gigantomastia where a woman's breasts grew three or more cup sizes within a few days. Other symptoms include breast pain, posture problems, infections, and back pain.

While gigantomastia is considered a benign (noncancerous) condition, it can be physically disabling if not treated. In some cases, the condition resolves on its own, but many women with gigantomastia will need to have breast reduction surgery

What causes it?

The exact mechanism by which gigantomastia occurs in the body isn't well understood. Genetics and an increased sensitivity to female hormones, like prolactin or estrogen, are thought to play a role. For some women, gigantomastia happens spontaneously without an obvious cause.

Gigantomastia has been associated with:

pregnancy
puberty
including medications such as
bucillamine
neothetazone

certain autoimmune conditions also such as:
Hashimoto's thyroiditis
chronic arthritis
psoriasis

Types of gigantomastia

Gigantomastia can be divided into several subtypes. The subtypes are related to the event that may have triggered the condition.

Types of gigantomastia include:

Gestational or pregnancy-induced gigantomastia occurs during pregnancy. This subtype is thought to be triggered by pregnancy hormones, usually during the first trimester. It occurs in just in 1 out of 100,000 pregnancies.

Puberty-induced or juvenile gigantomastia occurs during adolescence (between the ages of 11 and 19), likely because of sex hormones.
Medication- or drug-induced gigantomastia occurs after taking certain medications. Most commonly, it's caused by a drug known as D-penicillamine,

idiopathic gigantomastia occurs spontaneously, with no obvious cause. This is the most common type

How is it diagnosed?

Your doctor will take a medical and family history and perform a physical examination. You may be asked questions about:

your breast size
other symptoms
the date of your first menstruation
any medications you've taken recently
if you could be pregnant

If you're an adolescent, your doctor might make a diagnosis of gigantomastia if your breasts grew rapidly soon after your first menstrual period. Most of the time, other diagnostic tests aren't needed unless your doctor suspects you have another underlying disorder.

Treatment options

There is no standard treatment for gigantomastia. The condition is usually treated on a case-by-case basis. Treatment is first aimed at treating any infections, ulcers, pain, and other complications. For example, antibiotics, warm dressings, and over-the-counter pain medications might be recommended.

Pregnancy-induced gigantomastia might go away on its own after giving birth. However, in most cases, surgery is considered to reduce the size of the breasts.
Surgery

Surgery to reduce the size of the breasts is called breast reduction surgery. It's also known as reduction mammoplasty (WAHALA DEY SHA)

During a breast reduction surgery, a plastic surgeon will reduce the amount of breast tissue, remove excess skin, and reposition the nipple and the dark skin around it. The surgery takes a few hours. You may have to stay in the hospital for one night following the operation.

If you're pregnant, you might have to wait until after finishing breastfeeding to have a breast reduction surgery. If you're an adolescent, your doctor may want you to wait until after puberty is completed before you have the surgery. This is because there is high chance of reoccurrence. You may be asked to visit your doctor for an evaluation and physical examination every six months during this time.

Another type of surgery, known as mastectomy, has a much lower rate of reoccurrence. A mastectomy involves removing all of the breast tissue. After a mastectomy, you can get breast implants.

Medications

Your doctor might prescribe medications either before or after a breast reduction surgery to help stop the growth of the breasts. These may include:

Tamoxifen, a selective estrogen receptor modulator (SERM) used in breast cancer treatment

Depo-Provera, also known as the birth control shot

Bromocriptine a dopaminergic receptor agonist often used for Parkinson's disease that has been shown to stop breast growth

Danazol a drug typically used to treat endometriosis and the symptoms of fibrocystic breast disease in women

However, the effectiveness of these medications in treating gigantomastia varies. More research is needed.

Are there complications?

[b]The extreme breast enlargement and the excess weight of the breasts can result in physical complications, including:

over-stretching of the skin
skin rashes under the breasts
ulcers on the skin
neck, shoulder, and back pain
headaches
breast asymmetry (when one breast is larger than the other)
temporary or permanent nerve damage (specifically the fourth, fifth, or sixth intercostal nerves), resulting in loss of nipple sensation
difficulty playing sports or exercising, leading to obesity

In addition, extremely large breasts can result in psychological, emotional, and social problems. For example, teenagers with the condition may be harassed or embarrassed at school. This can lead to:
depression
anxiety
body image problems
avoidance of social activities

In pregnant women or women who have just given birth, gigantomastia can result in:

poor growth of the fetus
spontaneous abortion (miscarriage)
suppression of the milk supply
mastitis (breast infection)

What is the outlook?

If not treated, gigantomastia can lead to problems with posture and back problems, which can be physically disabling. It can also cause dangerous infections, body image issues, and pregnancy complications. In rare cases, a person with gigantomastia may need to have an emergency mastectomy because of complications. Gigantomastia doesn't cause cancer and doesn't spread to other parts of the body.

Breast reduction surgery is considered a safe and effective treatment. However, research has shown that puberty and pregnancy-induced gigantomastia may reoccur after breast reduction surgery. Mastectomy offers a more definitive treatment for gigantomastia

INFECTED GIGANTOMASTIA

Case History

A 36-year-old Saudi was admitted on November 6, 1995, complaining of huge enlargement of the breasts, causing her dragging pain and distress, and limiting her mobility. The patient was gravida 9, para 7 + 1, and on admission she was 17 weeks pregnant. All her previous pregnancies had been normal, apart from the last pregnancy, when she noted some degree of enlargement of her breasts, which started at 32 weeks' gestation and was well tolerated until delivery. After delivery, her breasts returned to normal size. During the current pregnancy, the patient noticed enlargement of both breasts during the first trimester, which continued increasing rapidly as the pregnancy progressed.



The patient had been diagnosed with secondary infertility due to hyperprolactinemia after her last pregnancy. Therefore she was put on several courses of bromocriptine until she became pregnant. There was no significant personal medical or family history.



While admitted, the patient was treated conservatively with cold compresses and a supportive bra. During her stay in the hospital, her breasts continued to increase in size and blisters erupted on the left areola, with eventual ulcer formation. There were multiple ulcers measuring 2-3 cm in diameter surrounding the area of the nipple complex. They were very painful, with occasional yellowish discharge. A swab taken for culture and sensitivity yielded a light growth of Acinobacter species. The patient was started on antibiotics and daily dressings of the ulcers. Surgery was consulted for reduction mammoplasty. There was also a question of terminating the pregnancy. This was declined by both the surgeons and the family.



The patient had a complete laboratory work-up, including a complete blood count, chemistry, liver function tests, coagulation screen and hormonal assay. Her hemoglobin was 101 g/L and WBC was 5.6 x 109 L. All hormonal studies were normal except prolactin, which was noted to be raised on two occasions to 51.6 and 46.5 μg/L (0-25 μg/L).



Because of increase in prolactin, bromocriptine 2.5 mg was given twice daily. The breasts initially responded and became softer, but still progressive enlargement was noted. A small right axillary breast was noted; the size was approximately 4 x 5 cm. The patient's disability became apparent as the ulcerations became more prominent, with increasing venous engorgement.



Five weeks after admission the patient started to bleed profusely from the left breast, when the ulceration eroded into a major vein, leading to considerable blood loss. The bleeding was stopped by applying pressure and suturing the ulcers. The decision was then made to take the patient immediately to surgery for a bilateral mastectomy. This was done with immediate reimplantation of the areolanipple complex of both breasts. The procedure was considered to be life-saving. A considerable amount of bleeding was encountered during the procedure and the patient required eight units of packed RBCs together with six units of fresh frozen plasma. There was no attempt to excise the right axillary breast, as this was of a small size. During the postoperative period, the patient had a minor wound infection, but this was controlled conservatively. Subsequently the patient made a good recovery. Ultrasound of the pelvis revealed a viable, progressing, normal fetus.



The patient was discharged home three weeks later and was advised to attend both obstetric and surgical outpatient clinics. Later the patient progressed to a spontaneous, normal, vaginal delivery with a viable, healthy baby. During the postpartum period, the patient started to complain of pain and swelling in the right axillary breast, which had increased considerably, reaching a size of 12 x 10 cm. Therefore the patient was put on bromocriptine to reduce the size of the axillary breast.



Subsequent follow-up in the outpatient clinic revealed that the axillary breast had continued to increase considerably in size, therefore interfering with the normal activity of the patient's right arm. The problem was discussed with the patient and she was advised to have an excision of the axillary breast. The patient underwent the surgical excision of her axillary breast and her postoperative period was uneventful.



From the pathological point of view, the right breast weighed 8 kg and the left breast weighed 7.155 kg (Figure 1). The accessory breast weighed 956 g. Histologically, sections of the breast tissue showed extensive proliferation of medium-sized ducts, with a tendency to aggregate around large distended ducts. The stroma was severely edematous. There was no evidence of duct epithelial hyperplasia, and features were consistent with gigantomastia.
