Military hospital salaries are paid by Ministry of Defence, not the Commandant of the hospital. LASUTHand other state hospital salaries are paid by Lagos state health commission not the CMD's.

Actually that large belly button is an umbilical hernia, most are harmless. However if a child or even an adult with a large belly button starts having severe stomach ache make sure such a person is seen by a surgeon. Not any doctor but a surgeon to assess.

Anybody that uses that kind of language because someone said something they don't like online is from the gutter and obviously had no home training. I was aware when I signed onto naira land that it was like going to the market, you cannot choose whom you interact with even mad men may move around dressed like the sane. The few that have are those under the revolving fund and have to depend on the management to release funds for fueling those generator sets. Feed the public with ONLY the truth!
A revloving fund in a hosptal means it is money raised from services provided used to the operation. Now if there are no patients paying for the services where does the revolving fund get its money. Such money's are not provided by government but from what patients pay. That is why fee paying pharmacies and labs in teaching hospitals tend to render better services than those that offer "free" drugs and investigations. Because they make money they have money to restock. If when patients come back there are no funds to run the generators the same people will say the CMD wasted fuel during the strike. He is damned if he does and damnedd if he doesn't.

I have seen so many cases when a man is down and he sends his children to the brothers house they become house helps to the brothers family. How many uncles put their nephews in the same school with their children. If you value your brother over your wife and mother of your children then in fact you value his children over your own. The same holds in LASUTH, when a patient gets to hospital he has to go through a long protocol of records, finance department, nursing protocol etc before he can then be seen by a doctor. Woe betide him if he needs the result of an investigation done days or even weeks before for the clinic. I have had patients who came to the hospital at 6 am finally show up in clinic at 12noon, sometimes to tell you an investigation you requested at their last visit 2 weeks or more ago is still not ready. I sometimes have to placate angry patients and tell them that the ancillary department is overworked due to the large number of patients they attend to. What do you think such patients will say when they get home? " After getting to hospital at 6 am I wasn't able to see the doctor till 12". Do you think they go round explaining what happened at each stage and then people will blame the doctor only. I don't bring my relations to the hospital because of all the protocol involved and the rudeness of the average staff. Once I identify the consultant they need to see I arrange and get it done privately.
Personally if a patient has been registered in the hospital and the hospital does not have the equipment needed for a procedure and I have access to it in a private setting I will not see the patient privately. I will refer to another teaching hospital. Not because I don't want to help but because I don't want to be accused of diverting any patient. Patients are always asking for a private clinic address where they know they can walk in and be attended to without all the protocol and what they consider time wasting
This afternoon one of my relations is having total knee replacement costing 3 million done at Lagoon hospital Ikeja and it's being done by a Nigerian surgeon, Nigerian anaesthetist and Nigerian nurses. Since they can afford it I advised it be done there because their results are as good as those done anywhere in the world.

I used to have a female college who was always complaining of her husbands wandering eyes and I felt so sorry for her. The day I went to her house I actually had to ask her who was in the photographs in her house, she was unrecognizable. In the pictures there was a young pretty smiling girl. Funny she wasn't even made up but she did have her hair relaxed and nicely done, meanwhile the person in front of me looked very bitter and hardly smiled. Colleges that knew them told me to ignore her, that they used to be a happy family that was the envy of all and things changed when she joined her church.

This guy is a fantastic anaesthetist, when you have a case and you hear he is your anaesthetist it makes your day better. It is a great loss to medicine that he chose politics but I can't blame him, he served humanity for over 20 years as a medic and has moved on to other things.

No nurse practitioners do not perform surgeries, they assist surgeons. In sane climates even general practitioners only do minor surgeries like cutting open a boil called an I&grin, removal of small warts, or in grown toenails and if trained emergency procedures like tracheostomies which are life saving and cannot wait even 10 minutes where necessary to save a life. Otherwise they refer to a specialist. These are even people with post graduate medical training in family medical practice.

The word 'rarely' is does not mean a total rule out.
Why don't you just admit that you had never heard of Otitis Media with effusion in your whole life. In medicine we say never say never. That a thing is done rarely means that that is not the standard it requires exceptional circumstances. I don't want to do the job of a laboratory scientist that's why they employ lab scientist in the hospital to do the investigations the doctor requests. Heck I don't even want to do the job of a general practitioner. I specialized in a feild so I want to do my job which is consult with my patients make my clinical diagnosis and manage as I deem it fit. If I decide I need a serum E and U for instance I make a lab request. If I need a FNAC I send a consult to the Pathologist, if I need an X-ray is end an X-ray request , if my patient requires surgery I operate and when he requires drugs I prescribe what I deem fit. The problem most doctors I know have with JOHESU is in Nigeria people do not know their boundaries. People like you who think you know everything will start swabbing ears of patients with OME and start treating such results with antibiotics because you believe all otitis media is the same or doing MP and typhoid for every patient with a fever and treating those results when the patient may have a sinusitis and if you do not take a full history and examine you.
But once more I say it is no use preaching to the deaf.

There is no discharge in a patient with Otitis Media with Effusion, so what are you swabbing and doing an M/C/S on. The condition most non doctors know of is Suppurative Otitis media, this is when a patient has a discharge and even then an MCS is not one of the diagnostic criteri. But I really didnt expect you to know that. What I wanted to point out to you is there are so many conditions that do not need any laboratory tests to make a diagnosis at all. And many others that are diagnosed clinically and tests just used to confirm the diagnosis or to guide treatment. No one says investigations are not important but a doctor must first make a clinical diagnosis so he even knows what test are necessary.

I'm sorry someone took offense when a surgeon described how things were done elsewhere and took it as an insult to nurses. There is something called assisted gowning and gloving and that involves the circulating nurse actually putting the gown and gloves on the surgeon,all the surgeon does is stretch out his hand for her or him. This is the standard I saw abroad for most major head and neck surgeries. Unfortunately back home many nurses would think it insultive that a surgeon expects them to dress him up for surgery. It was only after a foreign team of doctors and nurses came to do some surgeries in my local teaching hospital about 2 months ago and our peri op nurses saw this being done that one actually asked me if she should gown me before surgery. I was pleasantly surprised. I know there are many who do this but the vast majority of our nurses would see it as an insult to be asked to put on a surgeons gloves after all the surgeon has hands. What happens to a lot of Nigerian surgeons outside is that we are so used to doing some things by ourselves that when we go outside and try and do the same we are corrected by their nurses because the nursing protocol there is different. You hand your gloves over to the nurse not because she is inferior but because she has to do a count after each case and make sure every consumable from needles to swabs to gloves are accounted for.
I work in a teaching hospital and I can't tell you the number of times I have done emergency surgeries with my house officer or resident acting as my scrub nurse it's not because there is no scrub nurse available. For some reason they will come in and ask "doctor do you need me". Once you ask that question I say no, to me it means you don't want to do that case. I don't believe any trained peri op nurse thinks it is right for a surgeon to operate without a scrub nurse, no matter how small the case is. It just appears to me that the person who asks that has other things to do and I cannot force an unwilling person to be part of my surgery and mess it up for me.

Please educate me, which lab test do I need to diagnose Otitis media with effusion, Ca Larynx, Tonsillities, Sensorineural hearing loss, Tinnitus or epistaxis. These are a few conditions I see everyday and I make my diagnosis based on clinical findings alone. I then investigate my patients to rule out contributing factors but these investigations do not in anyway make my diagnosis for me.

I have been going to work because I am a consultant and MDCAN has asked us to obey the court injunction. But I am not admitting any patient or operating till my residents are on ground. If I try and do more than my carrying capacity and a patient dies while I am trying to "help" nobody will tell me thank you instead they will curse my father and my mother. It has happened to me before, the relations were told the child had a cardiac problem so it was like 70:30 chance going into theatre. Even though there was a strike, I was already billed to go on leave the next day so with their consent we went ahead and unfortunately the child had a cardiac arrest after the surgery. The relations hounded me for weeks until I gave them the address for Medical and Dental Council to go and make a formal complaint. After that I never heard from them. That is when I understood what my colleagues mean when they say " help me help me na I'm dey cause trouble! The average Nigerian only likes the doctor when you are bending over for him to kick you in the behind other than that you are a nuisance.
