Where I work, when I scrub in for surgery, the nurses gown me and glove me. This is here in Nigeria in a private hospital. This is not because they are inferior to the doctor or the doctor does not have hands to wear his gloves himself but to ensure sterility of the operating feild.

So many wrong things have become the norm in Nigerian public hospitals and many at times it’s the doctors who shot themselves in the foot in order to get results faster.That’s why house officers take blood samples to the lab and retrieve results, dress wounds, regulate Iv fluids themselves or move patients to theatre or radiology just so that work goes smoothly and it has now become their duty.

The only reason a patient is taken to ICU anywhere in the world means he is critically ill. ICU is not for moinmoin sickness and is not cheap.

I agree patients come to the hospital to get well and every member of the health team plays a part. But when you can show me patient that comes to the a general or teaching hospital to see the lab scientist or pharmacist or other health worker and will be happy to go home without seeing the doctor; I will agree with you that it is not the doctor they came to see.

Again 99% of time investigations are recommended by the doctors after they have seen the patients, taken a history, examined and made a diagnosis. Then tests are done to either confirm the diagnosis or assets the severity. If a patient walks into my consulting room and just hands me a test result or xray , I ask them to wait until I have done my bit before looking at any of those.

Antrochoanal polyps do not respond to drugs, the only treatment is surgical excision. If removed completely antrochoanal polyps rarely recurr. Polypvac will not do this unless its an early case. The best type of surgery for them is Endoscopic polypectomy where cameras are used to operate and total removal confirmed intraoperatively.

The danger with not operating is that they keep growing inside the nose and e nchroach into neibouring structures like the eyes and the brain. Also the longer they are there and the wider spread they are the more difficult they are to remove with surgery.
Then the drug Duplimab, it is not a one off drug, it has to be used consistently and costs about $35,000 a year. Most patients treated with it have surgery to debunk and then Duplimab to clear any left overs and prevent further regrowth.
Remember the bigger the polyps the harder they are to remove surgically and the sooner they recurr. Good luck!

Unfortunately the natural history of nasal polyps is recurrence and that happen even in the best of hands. Polyps may be of 4 types , simple nasal polyps that affect both nostrils , antrochoanal that is usually 1 sided, Benign Scneiderian tumours which may become cancerous if not removed and frankly cancerous tumours which initially look like sinple polyps. Surgery is done for biopsy purposes which is to take asample, send to the lab for analyse of the type or to debulk( reduce in size) or complete clearance. Many Patients who have surgery are lucky and dont have any recurrence or recurrences that can be managed with drugs.

Sorry for all you are going through. Unfortunately thee is very little I can do from a distance. Inbox me the names of the doctors and audiologist you are seeing, let me see if any is someone I know and then I will know what help I can render.

Sorry about what is happening to you.You seem to have 2 problems the hearing loss from quinine called Ototoxicity, the left ear being worse and the right ear seems to have a problem called Otitis Media with effusion which gets worse when you have a blocked nose and therefore a blocked Eustachian tube. Unfortunately the ototoxicity cannot be reveresed, the management for that is hearing aids. The other ear if confirmed to have OME can be treated with a combination of drugs and in some rare instances surgery. But you are not a good candidate for surgery because that is your better ear. Also hearing aids do NOT treat, cure or improve hearing loss. They just amplify sound making noise louder so the residual hearing can pick it up. Hearing aids vary in sophistication from simple analogue hearing aids which just make all noises louder to very sophisticated digital hearing aids which can be regulated to tune out some noises which would disturb the understanding of speech and concentrate on noises within the speech range. Technology keeps improving every day and now we have implantables like the bone bridge which are used for some forms of hearing aid but generally not in cases of Ototoxicity.

I just got another dose of lousy customer service from a Nigerian domestic airline. This time peace airline. I bought a ticket from Abuja to Lagos and when I tried to check in I was told my PNR did not exist and later the same day I got a message from the pay gate that my transaction did not go through. I had to hurriedly buy another ticket and travel the next day since I was on a tight schedule. I then sent my bank mail and forwarded the pay gate message to my bank only to be told the payment went through. So I now go to the airline with printouts of the correspondence and I am billed 10k to use the ticket for another flight. No apologies despite the fact that they never sent me a ticket to show that the transaction went through. I stopped using medview airlines for a similar reason lousy customer service and I actually remarked them to people around me because they are lousy. The day I get good customer service from a Nigerian airline I may just faint.
