Quick question regarding passport application process in Georgia State.

Does the consent form from father have to be notarised at the U.S Embassy or any court? Election postponement has spoilt plans

Make an appointment online after printing out the consent form and go to the U.S embassy and get it notarized for a fee of $50. Go with international passport for identification purposes. They will not accept any notary outside the embassy oh for obvious reasons.

Its called the consent form which you can print out from the US embassy website and then either notorise it b4 u travel to US leaving the baby name and the dates blank or let ur husband notorise it after u ve given birth and have it sent by courier to u in the US. It can be notarised at either the US embassy or by a legal practitioner that has a notary seal. You will need a valid ID card for the notorization. Note that u can only notorise at the embassy after the birth of ur child unlike other legal practitioners that can notorise for u before u travel and it is also important for you to confirm if the state u r birthing at accepts consent forms notarised in places other than the embassy hope this helps

You are absolutely correct with a slight variation though. The state agencies have no jurisdiction when it comes to passport matters/ issuance. The state department a federal govt agency is solely responsible and only nortary from the U.S embassy in Nigeria is acceptable for a fee of $50 of course.

greetings to every member in thread. need to know the benefit of given birth in the US as regards to the child, mother and father? all response will highly be appreciated.

I want to believe that you are doing this primarily to secure your child's future. You can only ever benefit after your baby turns 21 and is able to financially sponsor you for legal residence. You absolutely DO NOT become a citizen automatically. You still have to meet the residency requirements for 5years before you can apply for U.S citizenship. One of those requirements entails not spending more than 6months outside the U.S or your residency will be considered abandoned and your green card revoked. It's like a lady who once tried to go through the U.S citizens line at POE (because she had a U.S citizen baby) and she was told to get back to the visitors line because your child's citizenship doesn't give you derivative status until he/she petitions for you.

I am sorry about your loss but Pls Nigerians should desist from imagining we kill people here Age of viability in Nigeria is 28 weeks but am sure it's not unconnected with the fact that incubators require 24 hours electricity to run, that certainly ain't the dr's fault.
So if u want that here you will be charged heavily.
Let's head to Australia age of viability is 18 weeks there, since it's not the pali,USA is inferior to Australia
And 35 week old babies are not premature per sey cos their lungs are developed already so there is more to this,sepsis/jaundice etc
Pls go through the USA court records and find out the medical blunders they commit there!
Find out why no foreign trained doctor works in a teaching hospital without passing Nigerian exams
Find out why Nigerian IVF success rate is higher than the USA.
We do not throw alive babies born at less than 28 weeks into the dust bin we provide "comfort care" and a percentage survive.
Did the hospital you used have a neonatologist present(not peadiatrician) when labour started at 28 weeks these are the issues.
This services are available here we have neonatal ICU's .
Am sorry if I come across as insensitive it's because our patients too are even more insensitive.
As for government vs private hospitals the debate remains endless.
They record mortality all over the world but it's a reflection of not only the health care system but the level of education and financial strength of the patient.if USA had mothers that would spend 20hrs in labour with a doula and then turn up at the hospital m sure their mortality will be 100%
By Gods grace everyone will deliver full term .
One love
Every doctor or nurse has liability insurance. People loose their license in the US for any malpractice and it gets published. I am not sure the same applies in Nigeria drafrican. Are there court records we can go through in Nigeria to see the horrific blunders committed by medical personnel? I can only imagine how many have died or been turned away from emergency rooms because they had no money. There is absolutely no basis for comparison when it comes to healthcare facilities or ethics between the two countries.

Thank you Shagaman. I got someone to book for me, but I read something about obtaining an electronic travel authorisation for most non USA citizens. Heard of this? Does it apply to me? Also, does American airline have any baggage allowance?
Shelly thanks for suggesting southwest airline.
The ETA is for countries whose citizens do not require visas to the United states . As long as they have not previously overstayed on any previous visit.

I totally agree with those that feel appointment with Dr Fagbohun is like a business appointment.

A friend said it felt like a drive through clinic kind of setup. He has too many patients and it surely impacts on the amount of time he spends with his patients she thinks.

To be on the safe side go to Texas first which is your scheduled POE. You can then go to anywhere else thereafter just to avoid unnecessary interrogation.

Hello everyone, i have been a silent reader and this is my first post. i must commend everyone here for their time spent posting useful information for others to learn from. Kudos to you all.

i got my visa early last year and i was not pregnant at the time and no pregnancy related question was asked. i used the visa with my child last year and discovered i am pregnant 4 months after we got back from US. I intend to have my baby in Texas, however my relative stays in another state and i intend to stop there for a week before proceeding to Texas. Please i need advise on my itinerary. Hope they wont start asking why i am not staying with my relative as texas is far cheaper for me in terms of medical cost and i also have a friend to stay with there.

Your advise will be well appreciated.

I would suggest you stick to Texas as POE first. You can always travel anywhere in the United States afterwards.
Do not give them reason to try to read meaning into why you are going somewhere else. Just keep it clean and simple. My 2cents.

Hello house, pls has anyone obtained dre baby's US passport with an ID other than the Nig passport? My husband is not with me in the US and i already notorized the consent form in Nig without any ID n brought it to the US cos my husband's passport got missing and we were hoping to find it but we still cant find. Pls,i want to know which other Nig ID is acceptable to use for this purpose. This is quite urgent people, pls help out!

I hope the consent letter from Nigeria was procured from the U.S embassy in Nigeria (because you claim to have gotten it without any ID). Anything else will be rejected. All you need is your own passport and notarized consent from your spouse if I understand your question correctly.

The Joy Of Being Detained By U.S. Customs and Border Protection

After a really fun day yesterday in Waterloo at Communitech, which is really impressive, I woke up at 4:30am to make my 6:30am flight from Toronto to Chicago on my way to Little Rock, Arkansas where I’m speaking at the launch of Startup Arkansas. I was going to run the Little Rock Marathon on Sunday but I’m undertrained and – while I could get it done – decided I wasn’t ready to deal with the physical and emotional recovery cycle given all the work I’ve got going on. So I bailed on the marathon, but since they’d organized the Startup Arkansas event around my schedule and managed to procure me a number for the sold out marathon, I decided to go ahead and fulfill my commitment to that.
At 5:30am after checking in and getting my boarding pass, I slammed into the wall of US CBP. I’ve been through the Toronto checkpoint a few times and it’s always a long line that moves slowly. I got to the front by 5:45am so I felt like I had plenty of time to make my 6:30am flight.
As I waited for my turn, I noticed the CBP agent whose line I was in was moving slowly. The guy two people in front of me took about three minutes to clear – he had to take off his hat, then his glasses, and had to do some fingerprint thing on the scanner that I can’t remember ever having seen used before. I didn’t really think much of it, but I noticed it.
When it was my turn, I said a polite hello to the CBP agent, gave him my passport and customs form, and waited patiently. He handed me back my customs form and said “Put your correct address on this.” I looked down and noticed I had written my P.O. Box in Eldorado Springs as my address, since that’s my address. I responded “That’s my correct legal address.” He responded, “Do you live inside a P.O. Box?” I said, as politely as I could, “No.” He then said “Is your house inside your P.O. Box?” I said, “No, I have a house a mile away, but the P.O. Box is my address – at least that’s what the US Postal Service says.” By this point I realized he wanted a street address and not a P.O. Box, so I said “I’m sorry, I’ll write down the house address I have” which I proceeded to do.
I stood there quietly for about a minute as he typed stuff into his computer. He then handed my passport and my customs form to me after scribbling something illegible on it, handed me a yellow card, pointed at a room, and said, “Go into that room.” I asked whether there was a problem and he said more forcefully, “Go into that room.”
I did. It was a bigger room with about 10 CBP people. There were about 5 non-CBP people. No one acknowledged me as I walked into the room. I went up to one of the CBP people and said, “I was told to go to this room – can you tell me what I need to do?” He responded, “Give me your documents and go sit down over there until called.” This time I asked “Can you explain what’s going on?” The response was “Go sit over there until you are called.”
At this point I got anxious. I went and sat down. I sent Amy and my assistant Kelly an email telling them where I was. I tweeted that I had been detained by CBP. I started looking up flight information on my iPhone and a different CBP agent barked at me, “Sir, you must turn off your cell phone in this room.” So I did.
I looked at the clock. It was now 6:00am. I resigned myself that I was going to miss my 6:30am flight. At 6:15am I heard someone from the other end of the room call “Feld.” I got up, took my bags, and went to where that person was. He told me to put my bags down and stand in front of him. He proceeded to empty out my bags and go through them carefully. After he packed them back up, he typed a few things into the computer, asked me a few routine questions, including where I lived, which I answered more precisely this time with a house address, explained that the postal service wouldn’t come to deliver to my house, so I had a P.O. Box as the US Post Office that I used as my address. He stamped my forms, handed them back to me, and pointed at a door and said, “Go through there.”
I had no idea whether things were about to get better or worse. I asked him politely, “Can you explain why you are detaining me.” He responded with “Because you are traveling to the US.” I said, “I was asking why you’ve detained me for 30 minutes. Specifically, what was the reason?” The CBP officer said, “Because you are traveling to the US – we don’t have to give you a reason.”
I quietly picked up my stuff and went through the door he had pointed at. It led me back to the security area, in front of the long line to go through the xray machine, but behind the CBP checkpoint. So apparently I had now cleared customs, but still had to go through security.
It was 6:25am at this point. No way I was going to make my flight. I took a deep breath, realized my heart rate was high (probably over 100), and I was extremely anxious. I went into mellow shut down mode as quickly as I could, just soldered through security, and got to my gate. At the point the normal absurdity of air travel took over and even though I was on a United / Air Canada codeshare, the gate agent for the flight I missed (Air Canada) wouldn’t help me, the United Global Services people couldn’t figure out how to change my ticket, and ultimately I wandered over the gate that I figured out with the United Global Services person was my replacement flight, where the gate agent there was very helpful and soothing for the first time this morning.
I’m now in Chicago waiting for my flight to Little Rock. Given the flight times, I’ve got a lovely three hour wait, which I’m filling up with a long blog post to empty my head, all the phone calls that Kelly scrambled to reschedule, and some email time.
I’ve been awake for 7 hours. I’ve managed to get from Toronto to Chicago. My heart rate is back to normal. I feel fine but the 45 minutes of CBP stress, which was minor compared to what I know a lot of people face, sits with me in a very bitter way. CNN is in the background with the talking heads blathering on about sequestration and all the problems it, and our government, are creating. And I remember that getting into Canada on Wednesday took me literally five minutes, was pleasant, and welcoming.
What a shitty morning.

Wow. This must have been a very traumatising experience.
May God give you the strength to overcome this. Please don't let this get your spirit down.

Back to the matter, yes "Cancelled with Prejudice" sounds harsh but you can still get a new visa. You have not been banned from the USA based on what you said.

It seems as though there was a major misunderstanding with the POE officers especially with the supposed "evidence" they got from your phone. At the end of the day, everyone has a right to change their minds so what is wrong with changing hospitals or destinations?

Whenever you are ready (perhaps after your baby is born. If there is still time, you can even go before the baby but it will prob be too late for you to fly), gather your documents, reflect on your experience at the POE (to help you remember as many details as possible), make an appt for a new US visa and go. Be honest, open and have all your documents.

You may also want to consider getting an experienced immigration lawyer.

May God help us all with this USA baby journey.

Immigration lawyer for a non immigrant visa? I hope she doesn't end up wasting her hard earned money on a lawyer. I hope you did not implicate yourself when they were bombarding you with questions because I know for a fact that you must have been exhausted coupled with the fact that they type everything in those their amebo computers .
Getting entry into the United States should never be taken for granted by us no matter how many times we may have had a successful POE. A friend experienced the same treatment a few days ago. She had her first child 2yrs ago and came back this year after a successful renewal. She was taken to secondary and grilled for 4hrs. He bags were searched, phone, emails ,call logs etc. she was stamped in for 3weeks! She had to return home because she is due in April and 3weeks wasn't going to cut it and the clown knew it.. I guess she is fortunate that hers wasn't cancelled. We just need grace at POE . The day such inhuman crap happens to me will be the last time i take my hard earned money back there.

I am not a troll. neither I have that much time to behave like troll. My circumstances make me behaving like this. My in-laws really hurt me.

I have read US passport website that if I don't give consent ( Form 3053). they can still use form DS-5525 to claim that father is not reachable. Is that easy to use DS-5525.
If they are able to use form DS-5525 will she be declared as single mother. Any idea on implications on DS-5525.
http://travel.state.gov/content/passports/english/passports/under-16.html

I will tell you one more time my brother.... If you indeed told the consular officer about paying $10k when you did not to get that visa...DO NOT USE IT. Tell your wife to send you a scanned copy of any document she has that will shade more light on this insurance matter.
(she can snap a few pages and send through whatsapp)Anyone telling you to go ahead and travel with this visa doesn't realize the full implications. Use that ticket money towards paying down your child's hospital debt.
If your wife decides to go the DS5525 route instead of providing the insurance documents then she will have to swear an affidavit stating the reason why your consent cannot be gotten. Do you even know the name of your baby? The hospital your wife delivered in etc? This is the time to begin digging yourself out of the mess and not get entangled in more mess. Do not travel with that visa! You will have many more opportunities to travel but in the meantime do this the right way. Do not burn the bridge for others intentionally. I will not say anymore on this topic. Back to preggy n poe gist Jare..My 4cents.

It is 100% legal to have your baby through insurance because even in USA, everyone has one insurance or the other to foot their emergency bills

A lady here has used insurance to pay off her bills and she didn't have any problem so far.
Insurance is like a monthly contribution, anytime you need it, they pay off your bills. Although insurance varies depending on which one you signed for.

If insurance truly paid the whole bill, then you will have a 0 account with the hospital. Take the zero account receipt with you. That's all the evidence you need for future visa issuance. If the insurance company didn't pay all, you have to pay the remaining yourself

Very inaccurate statement. No foreigner is entitled to "free "insurance in the United States. Please,do not encourage people to break the law.
The lady you claim used insurance to pay off her bill...is she a resident or citizen? Was she paying a monthly premium on that insurance she used? I really hope it is not medicaid that your friend has used. In order to be eligible for Medicaid, individuals need to satisfy federal and state requirements regarding residency, immigration status, and documentation of U.S. citizenship.
