Other given name is usually if you have middle names

The question that ask ' if you have been know by any other name you could put your maiden name(former surname),

Otherwise please stick to the name on your passport

Thank you for the response. Please while filling address for my one year old son, for number of years stayed in the address, I put one year but I can't go beyond this stage as I am required to put an address stayed for at least 2 yrs. He is just a year old pls. What do I do here?

I really need to share this, I didnt share my visa alert here last year, when I was given my student visa , because then , I was so frustrated based on escalation of my application, school already resumed with load of works to do and application was done by school agent,but when my Dependants wanted to apply I called the same agent to help us do it again , she said, I should bring 150k, which I don't have, so I took it upon myself, to do it, so I asked a lots of questions from here@duchess90 and other gurus in the house. I submitted their application ( standard application) on the 20th of January, 2023, they did biometric on the 27th of January, 2023. and collection mail dropped on the 1st of February, 2023 .SO fast within two days and not super priority or priority. God is wonderful, it was like i was dreaming. Documents submitted, my BRP card, enrolment letter, passport data page, and my visa stamped page ,marriage certificate and my child birth certificate their TB test. their POF in my wife account with lots of pictures, charts, joint investment to proof relationship. once again I appreciate you @duchess and other gurus, thanks so much ,Glory be to God.. You have helped me to save 150K. cheers

The parent that filled the visa application form should sign the consent form

If a joint account is used for the child then both parents should sign that part(part 3)

Thanks for your help here Ma'am. As a married lady, while filling my application form, do I need to fill my former surname on the part that ask if I have other given name or I just leave that part.

Please note that my passport is in my husband's surname, my surname now.

I don't want to confuse the ECO.

You can have just your required funds but your husband should make sure his own POF is intact just incase the ECO requests for his bank statement(to proof you’re not recycling funds).

He can also show invoices of payment(tuition fee, rent, feeding…etc), if any.

If your first and last names match those on your passport or other government-issued identification, you're good to go. The middle name is optional unless you are required to include it while filling out official forms.
