Leave that section 1 and fill the other sections as applicable to you.

That's the individual in Canada that is standing as your Designated Representative, to Request for your notes. You don't need to know him. If you trust your source, just print the form and fill your own part and scan it back to them.

You can pay at the hospital (at the same day u re booked for your medicals). Don't bother paying online. When you get there, tell them you are yet to pay... Go with your atm card.

Is there a way I can book it without paying first ? I’m not getting it please
