Even in the US, the FBI may decide to try someone in a state court rather than federal when looking for a death sentence. The only crimes that can lead to a death conviction in a US federal court is espionage and treason. Same rules apply in Nigeria.

Breast cancer is very curable when early usually using a combination of methods - drugs (chemo, synthetic hormones), surgery and radiation.

The pain while being a real problem will soon become the least of her problems if you don't go to the hospital. The cancer cells will go to her spine and she'll not be able to walk and control her poo and pee eventually. They'll also go to her lungs and she'll start struggling to breathe. If she is not lucky, they'll go to her skull and brain too and she'll start acting irrationally, eventually falling into coma if if she hasn't succumbed to the other problems above.

I don't know why people play with health like this. Is it ignorance or what? Have you seen one person that you know who survived breast cancer using herbal treatment? If she starts having liver problems now, we won't know if it's because of the herbal drugs or the cancer cells have reached there.

For your question, there are so many pain relief options in cancer depending on the stage, the aim of the overall cancer treatment, the status of her liver and kidneys and other considerations.
But how will you know if you don't take her to the hospital?

You just missed the whole point of my first paragraph. There's a "doctor-patient relationship". Google it. Many components of this relationship are not compensation-dependent. So it's not a transactional relationship like the other examples you mentioned.
Like in the example I gave, is it likely that a customer's attitude will affect a banker's ability to solve the customer's problem? I think not. A patient's attitude can however affect the ability of a doctor to solve a patient's problem starting from getting adequate history, to informed consent for procedures right down to issues with treatment compliance. It's not as simplistic as you think it is. That's why I said some knowledge is more dangerous than no knowledge sometimes.

Why call doctors "providers" when you can just call them doctors? Same for nurses, pharmacists etc. This is a big issue in some countries right now because it simplifies the relationship I tried to show you above.

When you get there the nurse will tell you to take number and sit down(This is an emergency o) when you start crying after waiting for over 2 hours, nurse will then carry phone and call doctor. The doctor will not turn up till over 1 hour after receiving call. So me "devil advocate" whom should receive the blame in this scenario?

The thing is, as a patient you think where you see the doctor is the only place they're working. The doctor may be in the ward doing a round. It's not his fault that another doctor wasn't employed to cover the emergency. If you know how many hours a week doctors spend in the hospital, you'll understand better.

Most patients I've had issues with are educated patients who have some knowledge about what they think they need. The problem is that some knowledge is a dangerous thing.

First of all, doctors are not service providers. This is why there's now a campaign in the US to stop calling healthcare professionals "providers". It reduces the relationship to a transactional one and makes the patient a customer who is always right.
The doctor-patient relationship is a multidimensional one which I won't go into the details of. Try to look it up online. Anything that compromises this will make the first goal of the relationship which is "Do no harm" difficult to achieve.
In the case of a child, I need good rapport with the caregiver to be able to get a useful history of the illness. When this is not available I won't get the complete picture of what is wrong with the child and I may treat in a way that may cause harm. In such a case, it is my right as a doctor to refuse treatment and refer to a colleague unless in a "life-threatening emergency" (which has definitions). Patients and their relatives have to know this for everyone's good.

Another thing people need to understand is that you don't come to the hospital when you're in a hurry to somewhere else. Coming to the hospital means that you believe the problem is more important than any other thing on your schedule and we understand that. However, we have to triage and attend to patients in order of seriousness of illness not on first-come first-served basis. So if your child has a fever but is still active and they bring another in who has been convulsing for 30 minutes, another one struggling to breathe and another unconscious, we'll attend to all of them before yours and you can wait or come back later if you wish. It's not our fault.

What people recognise (but are just selfish) is that we're seriously overworked and almost always operating beyond capacity. We may be in the hospital from Saturday morning to Monday afternoon then someone decides to come on that afternoon to harass a doctor that hasn't really slept for two days. I'll be too tired to even reply and I'll just ignore you to save my remaining energy for the journey home so as to rest enough for the next day's work. We've admitted 30+ patients frequently in a ward with 12 beds, more people were on the floor. This is us being compassionate but risky because if anything happens, we'd be asked by the court why we didn't send the others away. Now, a 31st patient may come and the relatives may cause a scene because we sent them away. Whose fault is this?

Finally, patients seem to think we're robots and have ready solutions for everything. They don't understand the highly cerebral nature of a consultation so they expect it to be quick especially those waiting outside. Now, I think this is our fault as we don't engage patients enough. So, personally I try to make patients know my thought process in arriving at their diagnosis and the knowledge and logic behind it. There's however little you can do when you have 80 patients waiting for you and you have to see all of them in about 6 hours.

When as a patient or relative you understand these issues, you'll know how to get the most benefit out of being in the hospital.

The problem is not a high BMI, it's the higher risk of certain diseases associated with a high BMI that's the problem. So even if it's genetic or there are some hormonal problems, one still needs help, has to go to the hospital and put in work to be fit.

That's why the normal is a range not a single value. Once it's 25 and above, fat is responsible.

You know about drugs but don't know how they function in the setting of disease as much as the people who understand pathology and the pharmacology you know.
Now, I manage patients and I see how they respond to drugs (whether intended or adverse). I see these things on a daily basis but you think some book knowledge of pharmacology will make you know what I know. Those of you closer to doctors as clinical pharmacists respect this because they're better exposed. I won't talk to a marketer about pharmaceutics, medicinal chemistry because it will be difficult to make me see how those will uniquely affect direct patient care for a drug being marketed so it largely comes down to the pharmacology which I studied and see in patients every day but you don't.
You already have this notion which is clear from your earlier replies that I have something against pharmacists and it's wrong. I sincerely believe nurses have more (numbers) opportunities (may not necessarily be better paid) in the clinical, field and industrial settings than pharmacists. I also believe it is easiest for them to move overseas among all health professionals and this is common knowledge. You can disagree with points and it's fine but there's no need for insults.

Well, all the pharmacists I know are gainfully employed. Some pharmaceutical companies use the science graduates as reps mainly due to shortage of pharmacists. But it is wrong because those guys know nothing about drugs! I know that many learned doctors send them out of their offices once he notices that the rep isn't a pharmacist. Do you know why?? Because they can't provide the doctors useful information about drugs. When I finished my first degree, I worked for a pharmacist in his distribution company and I know what I experienced trying to detail drugs to doctors.

Stop lying that you have a pharmacist who is unemployed. Unless the pharmacist didn't attend an accredited pharmacy school or perhaps he/she just graduated and awaiting induction. If you are sure of what you are typing, give me the pharmacist's contact, I have a job for him!

The only thing you could say is that we have more nurses in Nigeria who seek overseas opportunities than pharmacists. Nigerian pharmacists are fewer and are not even enough to serve Nigeria let alone going abroad (although some still go). Pharmacists have more job coverage than nurses everywhere in the world and the reason is because their jobs are not restricted to healthcare alone unlike nurses. You have to have drugs first before you talk about using them in healthcare. From research to formulation, distribution, sales and then to dispensing and use in healthcare, pharmacists are in charge of drugs!

Cancel research in Nigeria. No R& is done here. Sales and distribution is also done by biochemistry, anatomy, physiology, microbiology graduates and the companies even prefer them sometimes because they can pay them less. When it comes to dispensing, most community pharmacies just have 1 pharmacist supported by a number of pharmacy technicians.
I have pharmacist friends looking for jobs but I don't have a nurse friend looking for a job.
Like I said, pharmacy looks attractive to people outside but those of us inside know nursing has more opportunities. I'm neither a nurse nor pharmacist.

Those alluding to the fact that nurses have better opportunities abroad seem not to be aware that no pharmacy opens in America even when the pharmacist is only out for lunch. The pharmacy must remain closed until the pharmacist is back from lunch. They also don't know about NAPLEX which qualifies a Nigerian trained pharmacist for licensure in the US.

Where pharmacists can work include:

Hospital, academic, Industry (production, quality control, sales etc), community, NGOs, Government agencies and parastatals, research institutions, military, etc

In a nutshell, pharmacists have broader scope of work than nurses.

It's the numbers. Whether you like it or not there are more openings for nurses even in those places you just mentioned. An NGO may employ 2 pharmacists at a base, 10 nurses and 5 doctors so you see what I mean. Pharmacists don't have as many overseas options as doctors yet nurses even have easier routes to and more jobs overseas than we do.

The problem with this thread is that the answers are from people who are outside the sector.
Opportunities keep increasing continuously for nurses. Even in Nigeria, nurses get NGO jobs more than even doctors, occupational health nurses are more in demand in companies. A traditional hospital can only employ a few pharmacists but majority of staff will be nurses and most reputable facilities are employing degree nurses now.
Overall, both are great professions but I think nurses have more opportunities currently.
