Me: Let me make a correction. Antibody test do not detect the virus in the blood. Antibody test detects antibodies that have been produced as a result of the virus.

Now my question; if the viral load is so reduced, how come a test that assay for the virus (the antigen) can still detect that the person has HIV? Is this not contradictory?

Let me break it down. The antigen test check for HIV antigens in the blood. Now, the person has a very low HIV load (little or no hiv antigen in the blood) and you are saying the HIV antigen test can detect the virus (when is very low in the blood).

Here is what happens (I stand to be corrected); HIV antibodies are almost always present in HIV patients (even if such patients have been on ART for a long time). This is because the immune cells will always "remember" that the body was exposed to HIV. So, if a patient has an undetectable viral load, doing a HIV antigen test will come out negative (because there is little or no antigen in the blood). For such patients, doing a hiv antibody test is the best because such patients will always test postive in a hiv antibody test because HIV antibodies are always present in HIV patients (no matter the duration they have been on ART).

HIV antigen test is superior to HIV antibody test in the area of diagnosis (if someone has been exposed to HIV, it can be quicker to detect using Hiv antigen test than a hiv antibody test because of the longer time the body takes to produce antibodies to HIV). However, in the area of detecting if a patient is still hiv + after years of being on ART, the Antibody test is superior to the antigen test
