I.e
Your eyes are what you focus on in life, and your hands are the instruments of action, or what you physically do.
I will give you one example: If you watch porn, you are focused on it, you feed your mind with it, you go to strip bars, etc. You are allowing your eye to rest and focus on things that are harmful to your spiritual health. Eventually, if you continue to focus on unclean things, you will find a way to fulfill your desires, either by immoral sex, or by masturbation, both practices are condemned in the bible.

It would be better for you to cut those thoughts out of your mind, deaden your senses, when it comes to sinning in thought, than to let them fester, and eventually make you sin, in action.
The bible wasn't telling you to poke your eyes out or cut off your hands.

Read the context of the rest of the scriptures around the verse you are asking about.
