The kind of hectic lifestyles led by people often makes them skip breakfast and even lunch in many cases. People believe that these habits will not affect them in any way. Headache, also known as cephalgia, is classified into two categories, primary or secondary, and all headaches fit into one of these two classifications. Depending on classification and headache type, the headache symptoms will vary. Primary headaches are standalone, meaning they are not associated with any other disease or disorder, but are themselves the primary disorder. In other words, they are not associated with any other underlying pathology. Secondary headaches are those attributed to some other pathologic condition. Secondary headaches are the result of infectious diseases, neoplasms (tumors), drug-inducements, and other causes of unknown origin. Primary headaches include tension-type headaches, migraine headaches, sinus headaches, and cluster headaches, and are much more prevalent than secondary types, which are rarely seen. The most common primary headaches are tension-type and it has been estimated that up to 90% of adults have this type. Females are twice as likely to suffer from tension headaches as males. Tension headaches are often mistaken for migraines because they have many of the same symptoms. They generally produce mild to moderate pain that feels like a tight band has been placed around the head. Pressure is felt from the back of the head and neck area all the way around to the forehead. The pain is usually pounding or throbbing, affects the front, sides, back, or top of the head, and can shift back and forth from mild to severe throughout the day. These types of headaches may last from a few minutes to several days. A good indication of whether it's a tension-type or migraine headache is that tension-types do not affect vision, balance and equilibrium, or muscle strength. Migraine headaches are the second most common type of primary headache, affecting as many as 30 million people in the United States. Migraines typically begin manifesting their symptoms sometime between the age of puberty and young adulthood, but can also affect children. In children, migraine pain is experienced equally in males and females, but as adults, females are three times more likely than males to suffer with migraine symptoms. Migraines are typically unilateral and are of a stabbing and pulsating nature. They are more often then not accompanied by temporary neurological disturbances such as bright flashes of light or zigzagging patterns of light. Other symptoms can include nausea, vomiting, and diarrhea. Sinus headaches are caused by inflamed sinus cavities. Sinuses are air-filled cavities in the cheekbones, forehead, and base of the nose that produce a mucus which normally drains out the nose. Sinuses can become inflamed due to infections, allergic reactions, and other disorders, which can prevent the normal flow of mucus. This buildup of mucus can cause enough pressure to result in a sinus headache. Typical symptoms of a sinus headache are pain in the forehead, cheekbones, or base of the nose (sinus locations). In addition to the pain there is usually a high amount of nasal mucus flow, fever, and swelling in the face. Cluster headaches, also known as "suicide headaches," are much more rare than tension-type or migraine and occur in just 0.1% of the population. They affect men the majority of the time and "intense pain" is their prominent feature. They differ from migraines in that they come on quickly; their onset is rapid. They are almost exclusively unilateral which is why they are often mistaken for brain tumors or even multiple sclerosis. Secondary headaches can result from countless numbers of conditions that run the gamut from life threatening to mild. They can be caused by relatively minor events such as caffeine withdrawal or discontinuance of pain medication and also from life threatening disorders such as brain tumors, strokes, and spinal meningitis. They can also result from eye strain, blurred vision and coughing. There are many other types of headaches, each having their own unique headache symptoms. The vast majority belong in the primary category while a lesser percentage of headaches are classified as secondary. Within a month or two of HIV entering the body, 40% to 90% of people experience flulike symptoms known as acute retroviral syndrome (ARS). But sometimes HIV symptoms don't appear for years—sometimes even a decade—after infection. "In the early stages of HIV infection, the most common symptoms are none," says Michael Horberg, MD, director of HIV/AIDS for Kaiser Permanente, in Oakland, Calif. One in five people in the United States with HIV doesn't know they have it, which is why it's so important to get tested, especially if you have unprotected sex with more than one partner or use intravenous drugs. Here are some signs that you may be HIV-positive. Fever: One of the first signs of ARS can be a mild fever, up to about 102 degrees F. The fever, if it occurs at all, is often accompanied by other usually mild symptoms, such as fatigue, swollen lymph glands, and a sore throat. "At this point the virus is moving into the blood stream and starting to replicate in large numbers," says Carlos Malvestutto, MD, instructor of infectious diseases and immunology in the department of medicine at NYU School of Medicine in New York City. "As that happens, there is an inflammatory reaction by the immune system." Fatigue: The inflammatory response generated by your besieged immune system also can cause you to feel tired and lethargic. Fatigue can be both an early and later sign of HIV. Ron, 54, a public relations executive in the Midwest, started to worry about his health when he suddenly got winded just walking. "Everything I did, I got out of breath," he says. "Before that I had been walking three miles a day." Ron had tested HIV positive 25 years before feeling so tired; fatigue during acute, or newly contracted, HIV might not be so obvious. Achy Muscles: ARS is often mistaken for the flu, mononucleosis, or another viral infection, even syphilis or hepatitis. That's not surprising: Many of the symptoms are the same, including pain in the joints and muscles and swollen lymph glands. Lymph nodes are part of your body's immune system and tend to get inflamed when there's an infection. Many of them are located in your armpit, groin, and neck. Sore Throat And headache: As with other symptoms, sore throat and headache can often be recognized as ARS only in context, Dr. Horberg says. If you've engaged recently in high- risk behavior , an HIV test is a good idea. Get tested for your own sake and for others: HIV is most infectious in the earliest stage. Keep in mind that the body hasn't produced antibodies to HIV yet so an antibody test may not pick it up. (It can take a few weeks to a few monthsfor HIV antibodies to show in a blood test). Investigate other test options such as one that detects viral RNA, typically within nine days of infection. Skin Rash: Skin rashes can occur early or late in the course of HIV/AIDS. For Ron, this was another sign that he might not have run-of-the-mill allergies or a cold. "They were like boils, with some itchy pink areas on my arms," Ron says. The rashes can also appear on the trunk of the body. "If [the rashes] aren't easily explained or easily treated, you should think about having an HIV test," Dr. Horberg says. Neusa, vomitting and diahiea: Anywhere from 30% to 60% of people have short-term nausea, vomiting, or diarrhea in the early stages of HIV, Dr. Malvestutto says. These symptoms can also appear as a result of antiretroviral therapy and later in the infection, usually as the result of an opportunistic infection. "Diarrhea that is unremitting and not responding at all to usual therapy might be an indication," Dr. Horberg says. Or symptoms may be caused by an organism not usually seen in people with healthy immune systems, he adds. Weight Loss: Once called "AIDS wasting," weight loss is a sign of more advanced illness and could be due in part to severe diarrhea. "If you're already losing weight, that means the immune system is usually fairly depleted," Dr. Malvestutto says. "This is the patient who has lost a lot of weight even if they continue to eat as much as possible. This is late presentation. We still see a lot of these." It has become less common, however, thanks to antiretroviral therapy. A person is considered to have wasting syndrome if they lose 10% or more of their body weight and have had diarrhea or weakness and fever for more than 30 days, according to the U.S. Department of Health and Human Services. Dry Cough: A dry cough was the first sign Ron had that something was wrong. He at first dismissed it as bad allergies. But it went on for a year and a half— and kept getting worse. Benadryl, antibiotics, and inhalers didn't fix the problem. Neither did allergists. This symptom—an "insidious cough that could be going on for weeks that doesn't seem to resolve," Dr. Malvestutto says—is typical in very ill HIV patients. Pneumonia: The cough and the weight loss may also presage a serious infection caused by a germ that wouldn't bother you if your immune system was working properly. "There are many different opportunistic infections and each one can present differently," Dr. Malvestutto says. In Ron's case, it was Pneumocystis pneumonia (PCP), aka "AIDS pneumonia," which eventually landed him in the hospital. Other opportunistic infections include toxoplasmosis, a parasitic infection that affects the brain; a type of herpes virus called cytomegalovirus; and yeast infections such as thrush . Night Sweats: About half of people get night sweats during the early stages of HIV infection, Dr. Malvestutto says. These can be even more common later in infection and aren't related to exercise or the temperature of the room. Similar to the hot flashes that menopausal women suffer, they're also hard to dismiss, given that they soak your bedclothes and sheets. Nail Change: Another sign of late HIV infection are nail changes, such as clubbing (thickening and curving of the nails), splitting of the nails, or discoloration (black or brown lines going either vertically or horizontally). Often this is due to a fungal infection, such as candida. "Patients with depleted immune systems will be more susceptible to fungal infections," Dr. Malvestutto says. Yeast Infection: Another fungal infection that's common in later stages is thrush, a mouth infection caused by Candida, a type of yeast. "It's a very common fungus and the one that causes yeast infections in women," Dr. Malvestutto says. "They tend to appear in the mouth or esophagus, making it difficult to swallow." Ron woke up one day to find white patches on his tongue. He had thrush. For him, "It was not bothersome other than I didn't like having it." The infection was hard to get rid of, but finally cleared up after Ron started taking drugs to combat HIV. Confusion And Lack Of Concentration: Cognitive problems could be a sign of HIV-related dementia, which usually occurs late in the course of the disease. In addition to confusion and difficulty concentrating, AIDS-related anger might also involve memory problems and behavioral issues such as anger or irritability. It may even include motor changes: becoming clumsy, lack of coordination, and problems with tasks requiring fine motor skills such as writing by hand. Cold Sores Or Genital Herpes: Cold sores (oral herpes) and genital herpes can be a sign of both ARS and late-stage HIV infection. And having herpes can also be a risk factor for contracting HIV. This is because genital herpes can cause ulcers that make it easier for HIV to enter the body during sex. And people who have HIV tend to have more severe herpes outbreaks more often because HIV weakens the immune system. Tangliness And Weakness: Late HIV can also cause numbness and tingling in the hands and feet. This is called peripheral neuropathy, which also occurs in people with uncontrolled diabetes. "This is when the nerves are actually damaged," Dr. Malvestutto says. These symptoms can be treated with over-the-counter pain relievers and antiseizure medicines such as Neurontin (gabapentin). Menstrual Abnormalities: Advanced HIV disease appears to increase the risk of having menstrual irregularities, such as fewer and lighter periods. These changes, however, probably have more to do with the weight loss and poor health of women with late- stage infection rather than the infection itself. Infection with HIV also has been associated with earlier age of menopause (47 to 48 years for infected women compared to 49 to 51 years for uninfected women). The malaria parasites of man include four species: Plasmodium vivax, the causative agent of tertiam malaria, Plasmodium malariae, the causative agent of quartan malaria, Plasmodium falciparum, the causative agent of tropical (falciparum) malaria, Plasmodium Ovale, the causative agent of tertian Ovale-malaria. Morphology of plasmodium Blood stages of plasmodium are as follows; 1. young trophozoites (ring forms) 2. growing trophozoites 3. mature trophozoites 4. mature schizonts 5. macrogamerocytes 6. microgametocytes. The merozoite is the youngest form of the parasite, appearing as the result of the splitting of a mature schizont. It is spherical or oval and small in size (1-2 micrometer). As it grows, a pigment appears within it (a product of hemoglobin breakdown) in the form of dark-brown spots. The nucleus and cytoplasm divide and form from 6 to 24 merozoites (it depends on the species of the parasite). The pigment accumulates in the center in a compact clump. The erythrocytes are destroyed by merozoites. The merozoites release into the blood plasma. Some of them again penetrate into erythrocytes. The gametocytes are sex cells and are subdivided into female (macrogametocytes) and male (microgametocytes) cells. The macrogametocytes are 12-14 micrometer, their nuclei are small. The microgametocytes are smaller, their nuclei are large. Life cycle The life cycle involves sexual stage (sporogony) in the mosquito (Anopheles) and asexual stage (schizogony) in humans, Man is an intermediate host and mosquito is a definitive host. All people with diabetes are potentially at risk of developing this condition, depending on how long they have had diabetes, their level of insulin intake and degree of control of the disease. Diabetic Retinopathy is the most common cause of visual problems in people with diabetes. About The disease occurs as a result of a weakening of blood vessels in the eyes, causing swelling and leakage of blood. Fluid and fat are able to escape into the eye, creating swelling of the retina and blockage of vision from blood and fat deposits. As the pressure of blood builds up, due to blockage of larger blood vessels in the retina, many smaller vessels are called upon to release this pressure. Unfortunately, they are weak and may bleed without warning, causing sudden vision loss. Scar tissue may develop at the site of bleeding and increase the risk of retinal detachment. Several risk factors contribute to the progression of Diabetic Retinopathy, in particular high blood pressure, protein in the urine and high cholesterol levels. Damage is usually quite severe before any change in vision is noticed, making it extremely important that the eyes are checked regularly. Prevention Most important in the prevention of Diabetic Retinopathy is a strict adherence to a diabetic diet and medically supervised treatment programme. Development of this condition is closely linked to how well blood glucose is controlled; the better the control, the less likely the disease is to progress. It is important that other risk factors such as high blood pressure, high cholesterol and smoking are reduced as much as possible. Signs and symptoms There are no early symptoms for many people with early stages of Diabetic Retinopathy. Many people do not have any vision impairment until the disease is in advanced stages. If symptoms develop, they may include sudden loss of vision, blurring, flashes of light, pain and changes in central and/or colour vision. Any person with diabetes who notices a change in their vision should be checked by a Doctor or Ophthalmologist as soon as possible. Health care As with all conditions, your Doctor should be consulted. This condition and diabetes in general require close monitoring by a medical practitioner. All people with diabetes should have a retinal examination at least every 2 years. Early diagnosis and intervention can help reduce the effects of Diabetic Retinopathy on vision. Any changes in vision should be reported immediately. Early detection and suitable treatment may help prevent up to 98% of severe vision loss. Health management should be approached as per prevention through appropriate diet (low in fat and sugar and high in fibre and carbohydrates), exercise and blood glucose management. A number of treatment options exist, depending on the severity of the condition. Most effective is laser treatment of the retina which aids in sealing the leaking vessels. Vitrectomy (a medical procedure) may be used to make an insertion behind the iris and relieve pressure on the retina. Diet tips - Following a healthy eating plan and leading an active lifestyle can help improve blood glucose control and regulate bodyweight. - Foods with a low Glycaemic Index (GI) are recommended because they lead to a small glucose response. These foods include wholegrain bread, oats, certain high fibre breakfast cereals, pasta, barley, legumes, temperate fruits, starchy vegetables such as corn and sweet potato and milk products. - Avoid excessive sugar intake from foods such as cakes, biscuits, chocolate and confectionery. - Limit the intake of fat, particularly saturated fat. A diet high in saturated fat interferes with diabetes control, contributes to weight gain and affects blood fat levels, increasing the risk of heart disease and other Diabetes complications. - Limit alcohol intake. Alcohol can interfere with diabetic control and can cause hypoglycaemia. - A Dietitian is an important member of the diabetes management team.
