Politics / Re: Imo Judgement Review: Why Is PDP and Ihedioha's Lawyer Asking For More Time? by manontree: 10:12am On Feb 19, 2020
What is the essence of this thread? What does it seek to achieve Are you a lawyer or someone seeking to inflame the polity

Allow the legal processes to end and if you do not understand any you ask questions Not assume conclusions on issues you do not fully understand

A review in itself is unusual by the SC and this is mostly informed out of the controversial nature of their judgement which they may have realized have entrenched precedents that would undermine our electoral processes. Let us assume their judgement stands. It means there is no need to vote as the courts can overule INEC on the number of accredited voters and award victory to even the last placed candidate if he can be able to concoct witnesses claiming he won any cancelled area with any figure he so claimed even if doing so means the numbers would far exceed the total registered voters there

This is a shamble and everyone knows this. In good conscience they all know what happened there.

Same way I do not believe in their ruling on Bayelsa. Truncating the will of the electorates due to the faults of one of the candidates seems abnormal and abhorrent. One would have expected a re run at worst. I can understand that the Deputy Gov elect forged his certificates and should not therefore benefit from his criminality. I can understand the argument that some voters may have voted the winner because of their love for the deputy who is now disqualified. The proper thing here is a re run

These two judgments do not grow our nascent democracy but take us several years backwards. They need to be reviewed and whatever time it takes those aggrieved to make convincing arguments to reverse these anomalies are in order

The few work going on was at the Asaba dinner city and only the ocean pillars have been installed across d water. They look old and unattended and no underwater or over water construction was ongoing

There is no way at this pace this project would even be 20% completed when these clowns leave power. This is just stupid propaganda and I would like anyone to bet me if this government would complete this before the 2023 elections

You are wrong once again.
Lamivudine / Zidovudine is the only drug that has been proven to reduce the risk of contracting HIV after exposure.
first line treatment on the other hand refers to drugs with fewer side effects. Most people stop using PEP due to the side effects that's why health care organizations recommended drug with fewer side effects as first line treatment.

Also, the two links you've shared have further confirmed there was no evidence that PEP actually failed. I wonder if you actually read the article.

https://blog.askdrtan.com/a-case-of-pep-failure/#.Xj6XdzPTXqs
According to the above article PEP was started 34hours after exposure. If you noticed I clearly recommend 24hrs latest, anything short of that is game of chance.

Where did you get this definition? Surely you just argue on what you do not know. Stop posting on medical issues pls

Lol people pay money for information that is widely available and people are even begged to learn.
You do not have any valid argument. Reason why nobody should accept your view.

For the benefit of doubt, I will post a link to CDC website so you can learn about first line treatment used for PEP.
https://www.cdc.gov/mmwr/preview/mmwrhtml/00052801.htm

I think you are a stubborn person. You are here referring me to a 1998 article by CDC!!! Something is very wrong with you

I say this because some people may actually read this and believe in what you say. Social media is internet based and people read all manners of things and some may actually believe you

If your doctor is prescribing this in 2020 you should get a new doctor. I have always said it over and over that if an exposure is serious enough to warrant PEP, the person must be followed by a WHO designated HIV specialist not just any GP. It is precisely for doctors doing this that this rule is in place

I was shocked that LUTH still gives Tenofovir disoproxil, emtricitabine and efavirenz as their standard PEP protocol. This is a long discontinued protocol with debilitating side effects and efavirenz particularly being resistant to most HIV mutations in Africa

So if LUTH does this who is your doctor to know any better and dish out nonsense to you and you rely on that to come here and argue with me

I feel bad that you may have learnt something from me today. Your type shouldn't have such information without paying big time for it

I do not know why someone would be looking for argument. You simply should argue on something you are an expert on and not what you think you have read up. Your first argument about Lamivudine /Zidovudine and what a first line defense is instantly shows what your level of knowledge is. You also stated your doctor prescribed this for you. Taken all together it is obvious you are not a doctor nor even a HIV specialist so why are you making this argument?

I do not have to prove anything to you nor even educate you. It's up to you to believe whatever you want as regards your uninformed opinion. The things I have written are critical information people pay good money to learn but I know you have learnt a great deal from them but choose to make arguments on things you have filial knowledge of
Health / Re: Am I At Risk Of HIV Infection After Sex In Imo Hotel? by manontree: 11:55am On Feb 08, 2020
Cakephp:

As much as you are educated but some of your assertions are false.
no study have ever been conducted on the use of PEP . The available data is merely from observations . No comprehensive data is available. In fact, it is physically impossible to conduct a study on PEP.
On the other hand, comprehensive studies have been conducted on PreP and it has been proven to be 100% effective.

Also, PEP has failed to prevent HIV in some cases but researchers did not conclude that PEP actually failed, those failures could not be attributed to PEP. It could be due to exposures during or after PEP is initiated or due to transmitted drug resistance.
This simply means the strain of HIV the individual contracted is resistant to available drugs.

But please sir I want to ask, concerning PEP I am only familier with Lamivudine / Zidovudine combination as prescribed by my doctor. Is this combination effective?

This is so false. Several studies have been conducted on macaques proving the efficacy of PEP and effectively confirming the overall success rate at 87% and how the increase in the volume of inoculation and passage of time decreases this efficacy percentage

I agree that on humans most of the studies done have been observational mostly because of the ethics involving a cohort study where you would divide people into two groups of one cohort receiving PEP and the other group not receiving PEP

The problem with this is that you now have to have people who actually need PEP but you choose to deny them this medication to know if they would catch HIV while you give the other group PEP to check the impact. This is the only way to actually do this study in a lay man's language. This would be unethical

As regards PEP failures which you claimed were not linked to the medication failure, you are wrong. I would post two links now that shows two cases of PEP failure where there were no subsequent exposures and the medications used were not resistant to the HIV variants mutation. The medication failed to work pure and simple

Lamivudine / Zidovudine is NOT a first line best defense. Most PEP failures we have so far experienced have come from the use of these combo. If you check my post above you would see the best first line defense. No failure has been so far observed from this new combo

Check your BP. A lot of people become scared when such incidence happen and the panic manifests in symptoms they assume is HIV symptoms. At 6 weeks you can go for a HIV test and get conclusive result

Okay but what about this- I had an open sore (harmattan or fever blisters). I kissed a girl but she didn't have the sore and I didn't see or taste blood. Am I also at risk?

There is no risk here at all. Even if she does have sores and you also have sores, the risk remains very low

Like he said, risk is strongly dependent on the volume of inoculation. This is why we say that it is safer to have unprotected sex with someone on HIV medication than with someone that is claiming to be HIV negative

The one on HIV meds would likely be undetectable and since we now know that undetectable is equal to untransmittable, due to the low volume of inoculation, your positive HIV sex partner would be unable to pass on the virus to you even without protection but those claiming to be HIV negative may actually be positive in a window period and infect you

so I can not just go for a test and save urself and us from all this wahala.

Sorry it doesn't work that way. When one comes into contact with HIV, or contracts HIV for that matter, he or she is actually highly infectious to others from that point on. The problem is that while he is infectious, he won't be able to immediately know that he has contracted HIV because most of the tests we have detect our body's natural defense system reactions to those invading HIV antigens. When our defense systems notice the HIV invading the body they mobilize to attack them and produce in that process what we have antibodies. It is those antibodies that the tests look for to confirm if you have HIV or not

Significantly it takes an average of 2 weeks to 3 months for most people to produce enough antibodies for the tests to detect them even though they have the HIV all those while and are actually highly infectious even before the tests can detect they have HIV

This gap period, called the window period, is very concerning for scientist as it represents the single major hurdle and barrier for effective HIV management where most people tests negative when they are infact HIV positive

There are more complex tests which looks for the HIV directly but they are too expensive to be commercially viable. So for now when you contract HIV you have to wait out this window period before you are sure you did not contract it

Take PEP for 28 days. No one has gotten HIV after correct use of PEP which must be started within 24hrs but if your lucky it could still work within 72hrs

This is serious misinformation. A big problem that some people may actually read you and believe this and start acting on its premise

This is false. While PEP has shown to dramatically reduce the risk of HIV transmission by stopping the progression of the virons from the dendritic cells to the lymph nodes where the infection would become permanent, there has been several cases of PEP failures even when the medication was properly taken and no further exposure made

Infact the success of PEP has been estimated at just 87% while some studies even estimated less

For the OP I am surprised someone actually prescribed PEP for this exposure which is not PEP INDICATED. There is virtually no risk for this exposure and as such PEP is certainly NOT warranted. The only positive for you here is that the Travuda would serve as PreP for you

Even if PEP is warranted, Travuda alone is NOT the best medication to be used. For the seriousness of this topic I would break protocol and state the best line of defense used as PEP. I know some would still not go to a professional when they have an exposure but patronize pharmacists who continue to dish out travuda, ensure that when you do visit the pharmacist, demand for these meds combo. Its the best combination therapy and failure is rare

Tenofovir disoproxil, 300 mg/emtricitabine, 200 mg (Truvada) once daily (This comes as a single tablet)

plus

Raltegravir (Isentress), 400 mg twice daily or dolutegravir (Tivicay), 50 mg once daily. Dolutegravir is new and highly effective but relatively scarce. In its absence go for Isentress

These must be taken for 28days

You must follow these guidelines before demanding for PEP

(1) The risk must be significant enough to be PEP indicated or warranted

(2) You must see a health care professional and not self medicate

(3) If PEP is warranted, you must do a HIV baseline test to confirm you are HIV negative at the time of embarking on PEP

(4) The exposure must have occured less than 72hrs ago

If PEP is warranted and you do start it, adherence is key to ensuring its success. Do not skip any dosage. Studies have shown that the new combo as stated above are highly effective for almost all significant risks. Travuda alone cannot stop virons progressions and has been shown to at best preserve your immune systems when you do seroconvert but won't stop the seroconvertion itself

Thank you for reading

Family / Re: I Am Sterile But About To Be A Father by manontree: 6:30am On Feb 06, 2020
I have a few issues with this story. He got to know he is sterile through the discharges on his private which somehow has affected his prostate gland. While untreated STIs can indeed cause infertility the symptoms described here is strange and rare. Second, an endocrinologist comes into play if there is a hormonal imbalance caused by the pituitary gland. It is his job to use medication to attempt to balance this and restore fertility

If however the prostrate is responsible for this infertility then that's a different play altogether unrelated to hormonal balance and hence an endocrinologist almost certainly wont be involved

Third in almost all cases where the infertility is caused by STIs viable sperm can easily be retrieved from the man and IVF succesdully done. Which completely means the man is NOT sterile as he claimed many times in his write up

So which is it??

And if somehow you are sticking to this story, then you have to be sure the exact cause of your infertility. Maybe its genetics or something else and how you got to know of this and be sure of it remains a question for another day

However let's assume you have somehow become absolutely sure that you are not fertile and your wife to be comes up with this

You should know that a woman that can do this has NO respect for you, your family, union, honesty, integrity and fidelity. She is not only wayward, she is wicked, irresponsible and empty plus vain. You should expect that she lies easily and has numerous other secrets you do not know of. You only know of this because you know your health issues. Like so many of her dirty secrets, she doesn't expect you to have a clue

It is for this reason I would excuse you for your deceitful conduct. On average we get a negative DNA test a month. This can be randomized to 1 in every 5 men having kids they didn't father. The decadent in the society is growing at an alarming rate and this problem is expected to triple in the next decade so men should be smart.DNA is readily available now and simple to conduct. You simply swipe the inside of your mouth with a cotton bud like collection kit and use another to swipe the mouth of the alleged child and return the samples to the lab. In days the result is out. Be sure of that child before spending millions training them

As women become more brazen and daring , the men must become more crafty like this Egbon has done. If he didnt have this problem he would have joined the 1 in 5 men fathering children that is not theirs and clueless to the truth while the woman continues her escapades

The bossy claim has no grounds. An unproven allegation which can clearly be analysed to be false. He stated she is rude and even her parents are against her. Her messages paints a picture of a woman that disrespects her husband

This is a man that has assisted this illiterate through school. If the claim that she pushed up the wedding bill to N3m is true doesn't this show a woman that is manipulate? Doesn't it show a woman that was scheming to ensure he balks and doesn't go through the wedding on account of the high cost?? Which same bride attempts to delete her fiance's purse if she planned living with him after the wedding

If he is bossy would he have obeyed her and paid for the high cost society wedding given his low income industry as a medical professional. N3m for a wedding is still extreme for a doctor in nigeria

I am not even understanding. Here is a woman he trained and suddenly she is now squaring up to him as man. How did he force her into marriage? Was she cuffed and a gun put to her head. If her mum is even pleading for his cause, doesn't this suggest this rebel is attempting to have her cake and eat it

He caused it all. By agreeing to even support this illiterate graduate. How about she asks from people who go through hell to seek help and go to school. I dont blame her. He was there for him. Now she is a graduate and he is pushing for marriage and she now suddenly realised he isnt good enough and after the marriage she is shaking tables to create confusion and cause him pain

I have not seen what wrong he did . Her rant paints a horrid picture of someone who sees a man she feels isnt good enough for her. I see a cheat who has cheated on the fiance so much she isnt interested in a life together with him. He has been scammed

She is accusing him of being bossy. Of course you have to hang a bone on a dog to hang it. Nonsense and ingredient.

My advise

Tell her to jog on. Look for a more grateful person. In no time that grass at the other side would turn red for her and she would come back, cap in hand. Only then you already know her real self. Refuse to be manipulated and bullied

I do not see what all the cries is about. So no more immigrant visas. So what. We closed our borders too and many of those poor west Africans are crying and we laughed at them. Now a richer nation is giving us a little bit of the same medicine and we are crying foul. What hypocrisy

The ban should please be extended to all categories of Visas. A total ban pls. Maybe only then can we become creative and focus our bright minds domestically to grow. It would take something like this for us to rediscover our doomed path

As for the Americans they are doing what they feel is good to protect themselves. However just like them, we can equally withdraw critical cooperation and show that even though we are small we have some leverage. You do not beg a super nation. You force them to appreciate you. There is a reason Trump won't ban Saudi Arabia, Russia, China and others. Saudi Arabia is particularly guilty of a number of terrorist activities against the Americans but they arent considered in the ban cos of the critical importance they are to them

In the same way we can start by looking at key areas of critical value to the Americans and pull out. Watch how they would quickly rush back to negotiate. You simply don't beg. You show balls

The ban should stay. Let Nigerian stop cooperating with the US on the war against crimes and see what would happen. They would speedily reverse the ban. The thing is that most US state department staff in nigeria would not be happy with this ban knowing Nigeria is one of the most pro American countries in Africa

We have a lot of bargaining chips. This country is home to numerous US business interests. FBI has established network of affiliates with EFCC. Use these to bend them to do whatever you want. We are simply too big for this ban if we twist them at the right spots. They would blink first

For some funny reason I love this. We closed our borders too but do not want others to close theirs against us. What hypocrisy

This is an opportunity for us to look inwards and develop the country, not taking flight at the slightest sign of trouble

Also look at the brighter side:

Nigeria can potentially fight back on this. We can restrict all US related businesses here as well as stop cooperating against the fight for crimes especially cyber crimes. Trust me this would see this ban lifted within a month as the benefit of our cooperation to the US far outweigh whatever they stand to gain from this travel ban

It may be noted that while non immigrant visas are not affected the reality on ground is that 90% of non immigrant visas are refused to Nigerian applicants, potentially meaning a ban of some sorts for that category also exists

The US has the right to protect their country. However if this is managed well, Nigeria can emerge from this stronger

Defectors have been variously taken to court in the past to force them to lose their seats. A celebrated case was when the nPDP members joined APC in 2014.

This is the correct position of the law but the supreme court has so lost credibility that they abeted this illegality many times by ruling with flimsy excuses to allow defectors

Everywhere in the world when a candidate is elected in a political platform and decides he can no longer champion the visions of that platform on which the electorates chose him, the law has always been trite that they resign and a bye election is conducted. They can then present the vision of the new party to the electors and see if they can be returned on the new platform

Here our politicians do whatever they like to con their electorates

So EFCC arrested a former senator and detained him for weeks based on mere allegations without corroborating physical evidence?

If they had a payment from Dauda to Sani in the amount alleged they wont need a lie detector to charge him to court. A polygraph is muted cos they dont have proof. If a payment from Dauda to Sani was made and Sani is providing documentary evidence of a car sales to Dauda, which is possible given the numerous cars given to senators which most auction out to raise money after serving, it can be seen why the EFCC is stumped and confused

It is equally hard to see a former senator, paid millions a while ago, stooping so low as to demand a mere $24k as facilitation bribe. Come on, Sani must know he doesn't have access to Magu and Tanko and it doesn't even make sense. It stinks of fabrication and why EFCC couldnt for the sake of the personality involved, first establish proof before arresting, detaining and humiliating a former senator shows this is sponsored and planned.

This case would collapse and Sani would be stained for nothing. Sadly redress for such humiliation is remote in a country like Nigeria

Therefore customary law applies and in most native laws of Nigeria this heavily favours the man
