What they mean is the application form for what you are applying for either admission or anything. Like uni assist application form. They need to know what you are applying.

Guys, for certification of documents, I got an email saying I should include "original application form (completed and signed)", how do I access it? Booked the appointment and didn't see anything like print form or something similar.

How did you make payment? And have you sent physical documents to Uni-assist, or it's just the online docs they used to ascertain your eligibility?

If you can, you should print another online and certify. You can also attach the faded one.

I do not know how bad it is but if it as bad as you depict, you should contact waec cos you might still face this problem for front.

I'll see about contacting waec soon too, should have done that a while ago, just ignored it all this time.

Uni Jena wants my waec certificate, problem is it's really washed out , like the print somehow washed out over the years, such that you can barely see the grades or name on the certificate, so what do I do, do I print my waec result online and certify. Please I need your suggestions. Danke.

Like things are getting better and interesting...my friend said he was surprised he booked on 3rd May and the AD mail landed on 5th May while his appointment will be on 2nd June. Though his start date as he choose was 3rd week of September.

It depends. If you had your IT in a lab or field, provide the acceptance letter or certificate issued by the firm as at when you began. Otherwise, approach the firm for a written report or note stating duration and kind of activities.

For the thesis, the Abstract and Certification pages can be of help.

My advice for those that are just applying to German Universities is that you should really consider getting a scholarship- that is if you are eligible for one.
