You can submit the application anytime withing the time frame, so yes you can submit it even now!

It is advised to use your teachers as your recommendations, as it is believed that teachers will give un-biased details about you.

I don't know how to answer your question as the answer to your question is already in the application form which you downloaded.

You write the ASU essay by answering the essay questions that are asked there and you apply to MSU by reading the instruction on the application form and on the website, which is the same thing I am going to tell you here anyway!
