There is no cure but it can be controlled but taking medications, diet control, moderate physical exercise and regular medical follow up. You need advise from your dr and or dietician.

Women are more concerned about their health and seek help. men assume they are superhuman and will hardly present for medical help Until it gets very bad by which time it may be too late or result in treatment with residual effects.
Men feel that going to seek medical help is sign of weakness. What a misconception.

Say NO and mean NO. It is your number and you decide who gets it. Lying or tricks are acts of cowardice and disrespectful. If a stranger wanted to sleep in your house and you are not comfortable with it you say NO. You don't need to give an excuse. If you are a xtain you speak the truth at all times irrespective of the situation, I think Islam advocates same. You don't ve to be rude, just a gentle and nice smile and politely turn it down. I know this is hardwork for most nigerian ladies because the average nigerian is very rude, sorry to say but that is my impression. I think ladies that give fake numbers or lie are weak and vulnerable so they look for the easy way out.

I can imagine that this must be a very difficult situation for you and may be even embarassing.
Medication should not be the 1st line of treatment for such a condition.
For you to have proper treatment, it will be nice to know what the situation really is.
- Premature ejaculation can be due to alot of reason

- Perfromance anxiety
- Been naive about the act of sexual intercourse

-I think the 1st thing to do is to try and relax before sexual intercoure and not put a lot of pressure on yourself as this can make matter worse

-Engage in pre-intimacy rather than diving in straight away without actually stimulating your genital area
Take your time and dont rush things. You are there to enjoy yourself and please your partner as well, It is not a competition.
when you decide to have coitus, you have to penetrate slowly and when you are about to ejaculate you can withdraw your penis and just engage in pre-intimacy until the urge to ejaculate goes away. Then you can continue and repeat the process as above until you have full control of when you want to ejaculate.
Another way is to get your partner to hold the junction between the glans penis and the shaft when you are about to ejaculate until the sensation goes away and keep repeating this till you have full control over when you want to ejacuate or not.
To master this may take a while

Some people try to distract themselves when in the act, this can also work but does take away from the pleasure as you are not focusing on the sexual act itself.

The last option can be medication eg SSRI

I am sorry to hear that you are having this problem.
From what you described it looks more like Nominal Aphasia, though it could be a symptom of depression or been absent minded depending on the severity of the problem and the presence or absence of other symptoms like your mood or state of mind etc.
Nominal aphasia is due to damage to the Temporal and Parietal lobe of the brain especially the left hemisphere (the Brocas and Wernike areas).
Depression can also mimic this symptoms but clearly you have not stated any symptoms to show you have depression
It would be nice to hear more of the things that you are experiencing to be able to know whats the issue.

I am planning to open a private psychiatric hospital in Nigeria. Can some one advice if this will work and which state will be the best location. I am not very familiar with psychiatric practice in Nigeria but with a population of over 120 million people I think it will be of great benefit to Nigerians.

personal reasoI would have loved to help you with some CBT sessions especially if you had Skype, but this will be very difficult for me due to my very busy schedule and other personal reasons. Do keep me updated if you are able to get CBT from your psychiatrist.

Depersonalisation is a psychiatric disorder in which the suferer cant not feel himself eg can not feel emotions (sadness or happiness). Simply put the person feels numb. Depersonalisation is an uncomfortable and, for some, a frightening feeling in which people feel unreal and detached from their surroundings. It's related to derealisation

Derealisation An uncomfortable and, for some, a frightening feeling in which people feel that things around them are unreal.

It is a very difficult disorder to treat and till date no treatment has been demostrated to be effective.
CBT and antidepresants has been tried but no evidence exist that this works.

Going back to your earlier enqury, I can understand that it will be difficult going back to work but I am not quite sure what difficulties specifically you are experiencing and a brief explanation will help. This will be more so if you have been out of work for a long time. It is good to know that you no longer have the physical symptoms of GAD. But the recovery process is more than just the physical symptoms going away, it includes getting your old self back again. This can take a while and you will have to be patient as alot of people may not believe the impact of your illness on your life and may question if you are telling the truth or giving excuses or been lazy. These does not help and can make things worse.

I am not familiar with the psychiatric practice in Nigeria and what treatment is available.
Psychological treatmet (CBT) would be very helpful at this stage and I do not know of this is available in Nigeria. But you can get books which can help and are CBT based, I am not sure if these are available in Nigeria. But if you have some one in UK they can easily purchase it and send it to you and they can help.
Going back to work will have to be gradual and in small stages when you are ready. It is not going to be easy but you may have to push yourself abit but will need support while doing this from your psychiatrist.
The more you stay away from work the more difficult it may become for you to return, but beaware that rushing back maybe counter productive.

Generalised anxiety disorder (GAD) is common and can be very disabling. It is difficult to know if you have both depression and anxiety disorder or depression only (anxiety can occur a part of depression so if the depression is treated the anxiety will go)
I noticed you said you last had symptoms of panic attack about a yr ago and imipramine (a tricyclic antidepressant) a few months ago.
It would have been helpful if i knew which country you are residing as this will determine what treatment are accessible.
It will have been helpful to know the aetiology (cause) of the depression and GAD.

Treatment is usually on 3 basis;

-Medication: usually antidepressants (many types, SSRI's are good and has more tolerable side effects than the older antidepressants)
can help to relieve anxiety as well as the depression for which they are usually prescribed. They usually take 2 to 4 weeks to work and have to be taken regularly to work properly. One of the newer SSRI antidepressants would usually be tried first – if that is not helpful, one of the older tricyclic antidepressants can be tried, or a newer antidepressant called Venlafaxine.

Beta-blockers are drugs usually used to treat high blood pressure. In low doses, they can sometimes control the physical shaking of anxiety. They can be taken shortly before meeting people or before speaking in public, or having to perform.

Valium can help on the short term to relieve the acute symptoms of anxiety but they are addictive and should not be used for more that 2 weeks and should be prescribed by a professional

- Psychological therapy: Cognitive behavioural therapy (CBT) is the best treatment and will teach you coping mechanisms and challenge those unhelpful thoughts that are usually the problem. The effect will last longer and can be used in future. You can be thought relaxation techniques too like controlling your breathing and relaxing your muscles.
Another helpful idea is mindfulness - a way of seeing unhelpful worries as 'just' thoughts. This means that, instead of being tormented by worries, you can learn to accept them and 'let go'.
If you dont have access to a psycholigist, there are online CBT that are accessible.

-Social treatment: Looking at issues in your life that are causing you problem (stressors) and addressing them.

Hi, I do feel your pain. I will advice you to stop using any methods to get enlargement. It does not work, waste of money and worse still you can do some serious damage to your manhood.

Life is unfair, while some are wishing 4 at least 1 committed rship, others complain of too much to handle.
Anyway, my little contribution is this you dont ve to marry the guy you love the most at a given point in time. Love grows, what you shd look out 4 is hw ready u r to get into a committed rship and then who is available to take that step with you.
The Guy A may be more committed than guy B, but u ve 2 ask urself hw ready his he and r u willing r u to wait 4 something that may not eventually happen. With Guy B the, he may eventually disappoint u cos until a man puts that ring on ur finger u can never say its well. But then the risk is less.
If you prefer to take the risk and wait 4 Guy A be ready for any disappointment cos the likelihood of it happening is very high. Life is a gamble but some are riskier than others.
