At this point, assisted reproduction is the way to go. Since the infertility is male factor, ICSI is most recommended where sperm will be surgically extracted from the testes and injected into the egg. It's more expensive than IVF and I'm not sure about it's success rate.

Extracting sperm from the testes is not ICSI.

ICSI is the injection of a single viable sperm directly into the woman's egg. So if a woman produces 10 viable eggs, only 10 viable sperms are required for ICSI. The sperm's source can be via masturbà.tion, donor, or extraction from the testes.

As per cost issues, it attracts the same cost as the traditional IVF with a better success rate (where male fertility issues are indicated). Many fertility specialists have abandoned the traditional IVF. Many of them will simply use the ICSI method whether sperm count is 10,000 or 60 million.

You need to talk to a fertility doctor. To avoid wasting your time and resources, you can simply do ICSI. All that is needed is a single (or a few) viable sperm, not those millions. Even if nothing is produced via eja.culation, sperm can be retrieved directly from the testicles through a minor painless process. But make sure you have some millions (~3m) for the entire process.

His suggestion is valid but ensure you're financially loaded before you proceed. PGD does not come cheap. You need about 8 million and cost of IVF/embryo freezing (another million+ per cycle). For every failed cycle, you may repeat the cost (especially when you do not produce enough viable eggs or high quality embryo) again and again until it works. Do you still want to proceed?

If you guys do not have say 20 million spare/small chop money somewhere, I would suggest you rethink your steps. Another option is to proceed anyway because of the love you both share and embrace adoption.
