I am happy to learn that you have worked in a hospital for close to two decades. Which specialty are you in and most importantly, are you a doctor?

And while you are at it, blood culture is what I will strongly recommend for anyone with typhoid fever.

I sincerely don't understand your fixation with this. Have you worked in a hospital before or are you just quoting off some textbook?

My job as a specialist is to fill out a form for blood culture, and note that i am trying to rule out Typhoid fever in my patient. A smart and resourceful medical microbiologist can figure out the rest.

Similarly if I suspect avascular necrosis of the left femoral neck secondary to some disease, my job is to request for an xray and trust that the radiologist will know what to do.

It is really that simple.
