Hello fola,

Please check my posts for a sample of the consent form. I put one up recently. For the applicant's name, its your baby's name as you are applying for a passport on the child's behalf. for email, use your email address.

Hi Mamacajah

Thank you sooo much, I have seen the sample on your post. I have a question. Do they accept scanned copy of consent letter as my husband is in nigeria

Hello house! Long time. Got back to naija last weekend with my American princess!

So I promised to post the process of getting naija passport.

Naija passport is gotten from only three states in the US - Washington DC, Atlanta and New York.

Please note that baby has to be physically present at their office for photo capture unlike the naija visa that can be posted by mail.

I got my baby's passport in DC. First, you have to go to their website (https://portal.immigration.gov.ng/passport/epassport) to fill an application form and pay $77. Make sure u select the right country and state where u want it processed. You can only pay online with a card that has the same surname with your baby. I haven't done change of name on my accounts so couldn't pay online. It's written on the website payment can only be made online but it's not true. Just fill the application form and print it out. Then buy two separate money orders of $77 and $30 each. Then go to their office with baby's birth cert(original and fotocopy), two passport fotos of baby, copy of both parents datapage, online application form printout and money order of $77 and $30. You do not need 2 book an appointment b4 going.

Wen u get to the office,if you went with money order(didnt pay online),they will collect additional $35 from you and say it is to associate the money order to your application form(in my opinion,it was jst a way 2 get extra money *hisss*).

Then you submit all the documents and wait for baby's turn to capture. They say it takes 2weeks for it to be ready but you can get it same day(expedited). It cost $90 for expedited and it will be paid to their supervisor. They will try to form that it's legal but na lie. No receipt for it and you pay codedly. Naija pple are the same anywhere in the world! Smh. Wait for 30mins or an hour max for the passport to be ready.

There's a business centre at their office where fotocopy is done for $10 per page and they can also fill the application form for you there but don't know how much. It's best you fill the form and print from home and go with all you documents to save cost and time.

Please note that this procedure is for DC office as that's where I did my baby's own.

Congrats to all new mums and safe delivery to expectant mums.

This marks the end of this journey for first baby. Will still be here reading(and laughing at funny posts and fights) quietly. I'm also available to ans any questions.

Hi isysleek

Kindly help, I'm trying to apply for my baby's passport in DC.
1. If i'm filling the form online, am I going to put my name or my baby's name (as per the applicant) im asking this because of the places I saw email address etc
2. My husband is not around hence, I need a consent form from him. Will the embassy collect a scanned copy or he has to send the original copy from Nigeria
3. Do you have a sample of the consent letter you used (if you used one)

Hello House, Please I need your urgent help

1. Anybody who has used united airlines to travel back to Nigeria should assist on how to go about buying an infant's ticket (either 2ways or 1way) because one way is more expensive than two ways and someone told me ill also need to show a two way ticket for myself before id be allowed to purchase a two way ticket for my infant.

2. Also, consent form for obtaining a Nigerian Passport. My husband is not around and I intend obtaining a Nigerian passport in the Nigerian Embassy at Washington DC. Id like to know if a scanned copy of the consent form will be accepted at the embassy.

Please help.
Thank You.

Did you eventually have your baby in Florida? if you did, how and where did you go about baby's Birth Certificate, Social Security No, Nigerian Passport/visa and any other document for the baby

What keywords are you using to search? If you put in 'hospitals in miami' i'm sure stuff will come up. Click on each one, and look for the services they offer, if obstetrics or gynaecology shows up, look for their patient services or business office numbers and plan to call them. Most hospital websites usually have a list of doctor associated with them, you can also get numbers for doctors and call too.

This is what I had to do when I was searching for doctors and hospitals in Orlando. I must have called more than 30 doctors' offices before I found my doctor.

Roll up your sleeves and get to work! Good luck.

Maryland is really expensive to give birth. You can try shady groove adventist hospital. Their range is arnd 7,000 dollars. Laurel regional hospital is like 6, 000 dollars. I used Howard County hospital in columbia, they gave us an estimate of 8, 500 for virginal delivery and down payment of 5, 000 dollars upon registration. I stayed in the hospital for 2 nights, had a normal delivery without epidural and total cost for mysef and my baby for 2 nights amounted to 6, 200 dollars. I think they give u estimate but u might not pay up to that amt if there's no complication. Ped fee was 290 dollars. Choice of hospital also depends on where you are staying in Maryland. You can PM me for more details.

Hello Maryland Mama

Please I need your urgent help/Information as per Howard County General Hospital.
How much was the Doctors fee and what other bills did you pay for (like Anastasia, blood work, office visits, ultrasound scan etc).

Your response will be highly appreciated.

Kind Regards
In all, what was your total bill.

Hello everyone been away for a while.
Arrived maryland safely via british airways.
I sat at the exit door for both the lagos to London and London to baltimore booked my seats online.
My fit to fly was never asked for (thanks to my wide bump)had enough leg room, slept through out my flight except when I had to use the bathroom but thanks to my TED stockings had no worries. Stopped the plenty water drinking when I started peeling every 15 mins. Didn't use the wheelchair cos my hubby felt I would draw unnecessary attention to myself(1st time traveling to the USA).except for MMH british airways time of landing was good cos the airports were empty.(LHR and BWI)
POE was BWI all blacks on my flight were about 9
Me:Good morning
cbp: good morning(a black American about 35yrs )
Passport and landing card
Me:handed it over to him
CBP:purpose of visit?
Me: have my baby
Cbp: 4 right fingers on the green light
Types and types
cBP: i see your birthday is tomorrow,I should stamp you in.
Me:laughs
Cbp:smiles
Cbp: you need to fill a form on the other end stating how much you're with
Me: thank you, he typed the total
At I think secondary inspection was told
Everytime you enter or leave the USA you must declare over 10k including cheques
Officer filled d form for me asked me to go through it if correct sign here.
Told me to go over to Agric cos I had ogi and shea butter (which is all I took )and says he wud meet me there with a copy of my form.
Passed through agric X-ray and then officer asked me to Open, I did.
He found it strange I don't have indomie, kilichi, beans and the other over 15 things he listed so he searched my second bag"your a rare Nigerian"
All in all must have spent about 30-35 mins cos the airport was so empty.
Thanking God for favour and wishing same to everyone planning for the blue Pali.

Hello house,
In the spirit of giving back. Here is my birth story.
I gave birth to my baby girl March 17th at United memorial medical center, batavia New York through C.S. Stayed in the hospital 3 days for all. The hospital offers a 60% automatic dscount for self pay. Got my bill for 3947.69 for mother and 1407 for the child, total of 5354. Doctor's fee for ante natal and delivery was 1499.4 (I was scheduled for natural delivery with a mid wife but had to go for c-section last minute, which included a doctor in the package. The fee above also include all the meal I had for 3 days. The service was awesome, everyone including the doctor's were very friendly, nice and professional.
I let them know before I delivered that I was going back to Naija in less than a month after delivery so they could process my birth cert and bills on time.
I filled the birth cert form before I gave birth and gave them baby's name on delivery. They filled for the cert on friday, I called the county's office on Tuesday and I was told it was mailed to me the monday after that friday. I guess they used regular mail cos I didn't get it that week, went to their office the following Monday, paid 10$ for another copy which I usedto apply for her passport. I paid 165$ for the passport and got it sameday. I mailed her passport to the agent for the Naija visa for 342$ (Tanko hardly picked his calls, felt safer using a company's service). They got the passport Tuesday Morning, got the visa same day and mailed it back to me. I got the passport the next day Wednesday. I have gotten my zero balance letter from the hospital. Leaving for Naija today.
Hope this helps someone.

Please one question. The zero balance letter is just a line. Hope this is okay.
Good luck to everyone

please can you send me the doctors email address or the phone number and where did u stay in new york

Hello mums and mum to be. I want to give back to the wonderful forum that impacted so much to me, though my story is coming in a bit late.
I delivered my baby girl last 2weeks at Parkview Community Hospital, Riverside California. I will tell you that l had a wonderful experience with the hospital and doctor respectively. The hospital bill for cash pay patient Normal delivery is $1150(1day), $2300(2days) and C Section $3950(3days) excluding epidural. Doctor's bill is $1700, laboratory test $200-250, pediatrician $300. Hearing test is free. I believe this ll help someone in d house.

Hello Chikaokpala,
