GentleMimi: What makes you think i aint calm? Talking of alternate moniker, why are peeps trying to decieve themselves, thinking they can decieve others? Ehn?
Patient indeed. Lol!
You sure do. Most of us saw through you from the start. Mr. Patient. grin
If you are truly a medical practitioner like you claimed, you should be able to ask some pertinent questions even if the individual decides to Google it or ask someone, the twists and his manner of presentation can easily give him away, it is obnoxious that in your experience such tactical questions elude you, reason you insist I am a doctor.

lemme give you a realistic example, a well trained police officer by mere examining your hands will know whether or not you have handled a riffle, there are cases where we see them arrest some people without guns and we term them "innocent", most times they are not really innocent but the answer lies in that training the police have that you don't, they will never tell you how they know, of course an innocent civilian who handles guns? can you make the connection?
That is what training keeps on the table.

if you claim to be an engineer in my field, I will ask you just two questions with finesse, no matter who you ask, however you Google, your presentation "MUST" give you away if you are not one of us, I don't need to second guess anybody who claim to do what I do, cos I do it so damn well, but hey!!, what do I know? low IQ's never ask the right questions, its a shame!!

GentleMimi: LMAO! @ jpphilips. I can see you are really hurt by the fact that i countered phantom's lousy and lame examples with so much ease. You want me to shut up? Really? Loooool! Why not shut me up with an adequate argument rather than hiding behind your ''patient'' status. It is obvious you hate challanges. I've met people like you who are eager to trample on others just to exhibit superiority. I will not use insults to shut u up, but wld rather pick on your brain.
You claim to be a neutral body, yet u're too sensitive and make comments which do not support that fact. Come clean, boy. Why the tin dey pain u na? Abi ''phantom'' na ur alternate NL account?
Now,this is a confirmation of the fact that you are in the system, hiding as a ''patient'' just to get cheap sympathy. I dey lie? Lol! Do not be ashamed of ur profession na!
You made a vague statement as ''phantom'',right? If u wanna give examples, why wld it be a lame and vague one? Why not give us something applicable and realistic? Vague comments aimed at making others seem inconsequential.

This was his comment, and he was very clear. Why go further to ask him if he cld apply it in all medical cases?
I hope that starting your comment with my name is not an invitation, the last time I checked, I opened the thread so feel free to come into the ring, If after going through my posts and you still think I am a doctor then I must have over stated your IQ, I must warn you, with this IQ of a tadpole, what Phantom served you will be child's play to what I will dish out for you.
At your age and education (night studies by the way) you cannot even construct an argument let alone throw effective punches, yet you want to argue with JP Philips, you are not even scared!!

nursemyke: oga philips you are most ignorant concerning the politics and practices in the health sector. kindly stop misleading the general public. nobody is stealing the ownership of the patients from the medical consultants, all JOHESU is saying is that healthcare is a teamwork with respect being mutual and reciprocal. let me educate you that the so called members of JOHESU who wants to be adressed and treated as consultants have acquired the requisite academic and clinical competence in their various field of practice. this will enhance more patient care outcome. more consultant specialist in various field more healthcare benefit to the client.
there is paradigm shift in the healthcare industry now lets all accept it.

joker5180: @Op you clearly are one of US.

Good write up, conqour with most of your points but I differ as follows;

NON DOCTOR CONSULTANT
NMA should not be against this, allow them to become CONSULTANTS but each one of them MUST have JOB DESCRICTION so that a NURSE CONSULTANT can not delay or stop a medical doctor's line of management even if he is a HOUSE OFFICER, she can only advice where necessary. Same should apply to other professionals.
NMA should ensure that their regulatory bodies come up with criteria that is internationally acceptable before such appointments can be effected, advanced academic qualifications and new skills rather than years of service should be the yard stick.
Point of correction, I am not one of you, I just went through the demands of the NMA and realized a few that are legitimate from a patient's perspective, reason I opened the thread, that does not in anyway undermine the other demands of NMA, I just considered them inconsequential for the sake of the thread.
Haven said that, I was expecting that asking for consultancy would have been the last on the list of events, IMHO, it is putting the cart before the horse.

Now, if JOHESU members really want to advance their careers, I don't see why the title "consultant" should stop them, if it takes 33courses and 6yrs (an example) for a doctor to earn that title, I don't expect anything less from the other allied medicals else, it will be very unfair to the doctors at the same time, nature like we know will follow the path of least resistance.

Answer me honestly, have you seen the curriculum drafted by JOHESU before fighting for the "consultant" title? I was expecting that if this is about career advancement, JOHESU president or whatever they have, should be able to sit with the NMA chairman probably with the minister of health to discuss the blue print and the curriculum before talking about titles, for sake of confusion, they may be given a different title but retain the benefits.

There must be order in our health care delivery, I emphasized so much on the "owning the patient" part because I know that is where the bulk of the responsibility lies, now JOHESU has absolutely nothing in the works to assume that responsibility, is it fair to the Doctors who may likely find himself taking the fall on a case where a consultant nurse has messed up since the consultant nurse is absolved from having the final say on a patient?

See through the bricks, this is about money and position nothing more, I don't have any problems with both, all I desire is where every penny is earned, If a nurse wants to advance her learning and she goes so far where she cannot own a patient, what is the Nigerian tax payers paying her for? or don't you want us to get value for our money?

Let us look at the University for example though not quintessential to the subject of discourse, Professorship is conferred on all depts, but before you get one, there is a guideline, there is a path, once you meet those requirements, you can easily become one, that is why a professor of yoruba studies can stand shoulder to shoulder with prof. of chemical Engineering even though they set out on different career path.

Let me ask you honestly, have you seen the guidelines JOHESU wants to use to become consultants? at least we know how doctors get there, how do they wish to be there? do you want a situation where a radiographer goes to India to learn how to switch off an X ray machine and comes back to claim consultant and we start paying? it doesn't make sense, it will be anarchy in the end, most medical students in sophomore classes knowing there is a way out, will pull out and switch to allied medical courses, give Nigeria just 10yrs and we wont have a single doctor again, It has happened before and we don't want it to repeat again, we have learnt from our Saudi experience.

If JOHESU wish to become consultants by all means they should give Nigerians a workable guideline that will enable them to have the same capacity as the Doctors in terms of patient's care, anything short of that is wasting tax payers money.

GentleMimi: @ phantom,my sister..
Is your explanation so bogus that even you cannot make yourself understood? You sound like a quack in the process of trying to come across as an ''independent'' worker.
Oh you do obtain diagnosis by physical examination only? Interesting. Or u are subjected to mild cases only, while the top guns handle more challanging cases. What the hell has this discussion got to do with rural hspt? We are talking of an average hospital, dude. Even if u find urself in a rural area, certain cases will require u to refer ur patient to the city for proper investigations. Rural hospitals have limits,so stop giving me lousy and lame examples.
You are the one running round in circles in an attempt to seem effective. Prior to sending ur patient out for investigations, what you have made is simply a guess work. If u get called up tomorrow, can u defend your guess work boldly?
It is peeps like u that are giving ur fellow doctors a bad name. A doc thinks he he can diagnose based on symptoms and interrogation only. Lol.
The Ebola virus has quite similar symptoms to acute malaria. I guess u'll give ur patient antimalaria to deal with such a case, right? Loool! Quackery village dokinto.
It takes someone with an average IQ to make out a topic sentence and a direction in an argument unfortunately, you were able to catch the topic sentence but you failed woefully to understand the direction.

For the sake of emphasis, Phantom made a blank statement but you were in a hurry to prove him wrong (even though the statement was too vague to be contested), all he needs to do is to paint scenarios that buttresses his point,
A smart man is what he is, an impatient person is who you are, I will advise you for free, when next someone makes a statement (topic statement) pin him down with applicable scenarios (direction) before you fire your stroke, so far, you sound so unserious, incoherent and goofy.

Phantom said he can do a clinical whatever without a lab test (blank statement), the impatient you (not uncommon with women anyways) started shouting the roof down, If I were you the next question would ave been,

Can yo do it in all medical cases? (the answer to this gives you the first base to strike)
Can you apply it in every environment?
Sincerely speaking, will you like the medical labs in your hospital to be withdrawn, if yes why?

You sound learned but you are very far from being intelligent, Logical reasoning is not gotten from text books, you either continue making a mess of your education or you just SHUT UP!!

The title “Consultant” in a hospital setting describes the relationship between the Specialist Medical Doctor and his patient. It will be a source of confusion if the title is applied to any other health worker who statutorily does not own patient. NMA therefore declares with unmitigated emphasis that if “non-doctor consultants” are appointed, it will lead to chaos and anarchy in the health sector. This should not happen.

Samgreguc: if the bolded is true and u still dont see any reason for specialization then u aint telling urself d truth. You know why I said so? Now, this is it, the Pharm. D guy is just a graduate who hasn't specialize in any of the pharm field. He read alot in pharmacy school(frm farm to industrial to hospital) and you expect him to b a magician? Can a fresh MBBS graduate perform better on their field like the Pharm. D fresh grad.?
.
We need to specialize. . . So, that consultation can be done.
I really didn't want to over flog this issue, let us be clear, the Doctors did not stop you from advancing your studies, I have read their press release over and over, they only asked you to drop the title "consultant" because you don't own the patient, it is clear!

Now, about my friend, neither the D.pharm nor the Patient was wrong, suppository for internal hemorrhoid is not a bad management but the doctor had the only cure!!

the D.pharm has tested the potency, professionalism and his understanding of drugs and landed on suppositories (ANUSOL) to be precise, but the surgeon's knife brought the desired result.

Unless you want to tell me that the advancement you desire will cover surgery and other courses, then you don't need consultancy to achieve that.

tmontee: Your attempt at winning the empathy of 'ordinary' patients has been betrayed by your own subsequent posts. Only the undiscerning will fail to see that you are indeed a Doctor.

That being said, I think one sure way out of this mess is privatization. That way, only those who truly have something to offer, including the doctors themselves; will find their way into any hospital. Only then can we begin to talk about putting the 'PATIENT' first. For now it's just politics and ego at work.
I am greatly honored to be confused for a Doctor from my posts, obviously, you are definitely not conversant with that username JP philips.

Nobody ''owns'' any patient. Stop confusing yourself with that phrase. Very funny and naive.
Who does the diagnosis,to confirm the prognosis? Medical scientists.

Who handles patient care in the wards? Nurses.

Who handles therapy in patients with bone issues? Physiotherapists

A patient comes into the hospital with complaints of malaise, nausea, weakness and pain. There are more than ten disease conditions with the same set of symptoms. Automatically,he requests for MP/widal. It comes out negative. What next? If he is a god,let him now determine the cause of the patient's ailment with his ''prognosis''. If he is a god,let him take care of the patient when admitted.
I repeat, nobody owns the patient. It is a collective effort.
Thank you for your opinion, I was admitted by a Doctor, operated on by a Doctor, Discharged by a doctor, my colleagues then(who had a terrible accident at work)was certified dead! by a Doctor, the BSC and Reg. nurse politics couldn't even let me get well better. you may choose to take your med advise from the nearest native doctor, the Conventional doctors work for me,

chucs: Mr man its not every nurse that packs faeces. There is hierarchy and carder in nursing profession which is also found in other profession. U seem to be a patient that has vast knowledge in medical field but found it difficult to acknowledge the existence of nurse consultants as seen all over the world. I find it difficult to understand why a professional will not be allowed to get to the peak of his/her career. The issue on ground is not about ownership of the patients but the a clear knowledge of what consultant stand for. According to wikipedia "a consultant is a professional who provides professional or expert advice in a particular area such as security(electronic or physical),management, accountancy, law, human resources, marketing(and public relations),finance, engineering or any of many other specialized fields"
There you go again, if there are already existing unproductive hierarchy and cadre in nursing, why do you want more by asking for consultancy?
What have the patients benefited from those so called BSc. nurses who fold their legs on front desk while the registered nurses pack all the sh!!t and p!ss? what use are they to us?

I have seen a Bsc nurse who refused to remove an Anal dressing in the name of not wanting to soil her hands and claim it is for the Reg. nurse to do so, meanwhile on another patient, the so called BSc nurse could not put a catheter, it was a doctor who came to her rescue.

BSc nurse vs Reg. nurse is a lot of problem already, the patient don't want more bureaucracy. A Bsc nurse on night duty only chat online in the name of writing report on the system, it is the Reg. nurse that throw away all the p!!s from the P!ss bags, practically they are useless.

Aux. nurse will bathe the patient, fetch water for patients, run around for food, serve drugs, clean the hospital, change diapers etc the BSc will be seating like an oracle dishing out insults on the productive ones.

#Say no to unproductive consultancy# if a nurse, pharmacist or scientist want to be consulted, open a shrine, or store take it outside the Govt hospitals.

GentleMimi: You call it grammar,i call it facts. No,i aint accusing you. Use your tongue to count your teeth.
I will always be realistic. The first point of call in a hospital is front desk officer/receptionist...then nurse....then a doctor...then a scientist....back to the doctor...and finally a pharmacist. Others come in at certain points. But do not EVER take one unit as insignificant. The last point of call is usually the pharmacy.
Simple question behooves simple answer, I may not be a health practitioner doesn't mean I left my common sense abroad.

Who does the prognosis and logs in the patients complain?
who tells the scientist which test to run to confirm his prognosis?
who writes the drug prescription and dosage that wakes the pharmacist from coma?
whose signature admits and discharges the patient?
who certifies a patient DEAD?

back to the issue again, who owns the patient??

adeoladrg: My brother, pharmacy also has specialty areas just like medicine. HIV pharmacists exist, Endocrine pharmacists exist, I personally know a Renal pharmacist.

All you need to is undergo Residency training provided by WAPCP for a minimum of 6 years depending on your area of specialty. I just got an info from a consultant on FB, that's there's also post residency internship for about 3-4 years depending on the area of specialty too.

This is what makes you a Consultant pharmacist. Now tell me how useful such a person is to patient care. Compare how an ordinary pharmacist would optimize an HIV patients polypharmacy prescription to how an HIV pharmacist that has spent almost 8 years studying the HIV drug would optimize!

This still doesn't destroy the patient-doctor link. It just makes sure the patients get the best drug therapy at whatever time.
You cannot be paid as a consultant just because you are responsible for just an ailment, haba! consultant for HIV pharm, consultant Kidney nurse, consultant cosmetic cleaner, the whole thing should sound ridiculous to you (Hope you didn't chip in the HIV clause to scare me grin grin) After all doctors manage HIV patients too or your own HIV now have wings?

please If after your sojourn in the realm of HIV, Pharmacy et al, you cannot own a patient, then it is useless to us. NMA has been able to accept that responsibility and Nigerians are glad to give them the Badge and honor to go with.

GentleMimi: Young man, being a consultant does not mean ''ownership'' of a human being. It means being an expert in ur field of work,so as to be able to give advice and top notch contributions towards the improvement of services.
the posts have four key functions: expert practice (max. 50%); research, evaluation and service development; education, mentoring and overview of practice; professional leadership.
were you the person who accused me of being a Doctor? why are you generous to explain to a doctor what ordinarily he should know?

I disagree with that, now this quote below is what will save the patient's life, an excerpt from my original post.

The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) - have initiated countless cases against nurses in the United States, between 1998 and 2001, they have initiated well over 413 cases of malpractice,http://www.nursingcenter.com/lnc/journalarticle?article_id=423284
you guys form Unions and organizations to make money but up there you can read what JCAHO is doing for the Americans, when will JOHESU and NMA start legal action against its members in the interest of the patient?

You cannot convince me beyond reasonable doubt that a Nurse consultant who cant own a patient will improve my welfare except you are referring to the welfare of her economy, then i agree completely.

For the record,i'm a female.

And what do u mean by ''own'' a patient? U see why i have doubts abt u being a neutral mind? Patient indeed! *scoffs*.
Obviously, making quality health services available to ur ''patients'' at an affordable and attainable cost seems to come at the bottom of ur list.
Do not give me the ''patient'' line. You expect good services when the service provider is subjected to a non-condusive working environment and deprived of basic incentives. Oh wow!

We are in the system,we know the drill.
Of course I am not a patient, I am brandishing a patient's point of view, why do you want me to become a patient at this my prime and what do you want to infect me with to endear such demeaning status?

Owning a patient? any body who is not conversant with that parlance has never been admitted before, what does it take to figure it out? On admission the nurse says wait for doctor for this, wait for a doctor for that, Is it not commonsense to decipher who owns the patient?

@second bolded.....let the relativity be maintained.skipping johesu without skipping doctors will erode that relativity.the relativity we are using now is not the proper one.

@third bolded.....the issue of hazard affects evreybody and that is why i maintain that chukwu should be sacked. the issues in the health sector now can only be settled on a round table iwth all the interested parties involved.
hazard should be paid based on the risk you face at work.simple!!! and it should be negotiated for all health workers at the same time.
lab scientists&pathologists>surgeons(including O&G)>radiographers/radiologists>pysiciians including paediatricians>nurses>pub health docs>pharmacists

@fourth bolded....privatization is the way to go.that is one of the reasons NMA is insisting on 100% coverage for NHIS. they go hand in hand.
privatization will address simultaneously the issues of quality or standard of healthcare(including training of personnel and up to date equipment) and better remuneration of workers
The thread says Patients perspective oga doctor, what you are skipping is not our problem, on a side note,

why do you think JOHESU should not get same hazard allowance as the doctor If the salary parity remains valid? I don't understand why it should be extended to allowances, extended work hours, Nite and day shift etc is same for all, why the parity in allowance?

Now in companies, field allowances are not Taxed, because that is where your hazard and inconvenience allowance is ( i am not implying that some "charge and bail" indigenous companies don't tax theirs) but that is the standard the business owners came with from abroad and it has been a latent standard for sometime now.

All Tax and deductions face the basic salary, now for a field operative, each level have their pay, you only know an Oga from other allowances and salary but not "hazard".

If you and I go to the same rig and you feel your hazard is greater than mine, then i will be forced to throw you overboard to prove it.

snowytee: TF is "I was meant to understand"?
sorry bros, just help me put the right phrase, while doing that, Kindly read meaning into the rest of the message to garner support for our doctors, like I told a friend of mine, it is we the patients that will suffer most if the status quo is not restored.

Look at the competition between the BSc Nurses and the Registered nurses (all JOHESU members) and now they want consultancy?
I don't want a nurse to tell me she is a consultant who can no longer pack sh!!t in the ward.

I didn't say they should be reduced to minions, all I proffer is that a limit of years experience should be inserted, then there should be no monetary gains in being a CMD .
Why will you remove the monetary gains of the CMD simply because JOHESU cannot get there? I don't see the wisdom there, I don't see JOHESU as a big threat to a CMD either, if I were a CMD, I should be worried over my fellow consultants whom I out maneuvered to get the CMD seat, those are the real threat. but hey! what do I know?

for you to call something a career, there is always a step to climb, there is always a status to achieve and the perks there in, that is what breeds competition in an organization, so, if it takes Academic, clinical and managerial superiority to get there, by all means let them compete, but the office of the CMD, should be attractive for House officers to look up to it to work harder.

I for one is not happy that consultant Drs are reduced to doing administrative work in a country where the doctor : patient ratio is disproportionate. A nurse or a pharmacy or a doctor can be a CMD so long as their pay package remains the same. That way, I doctor would not envy that CMD who is a nurse since he/she earns more than him/her. Nigeria like making the top juicier and hence corruption persists.
Stop sounding like all consultants are CMDS? how many CMD's are in the whole country? if a few consultants are given administrative offices, what is wrong with that? don't forget that the same office co-ordinates the affairs of all the other departments in the clinic, In other words, he has a clinical input in matters that is way beyond the other head of departments so don't limit it to paper work.
Either way you see that office, it was attained through hard work and academic superiority, its benefits shall never be compromised.

Consultant nurse or pharmacy is too vacuous an idea for government to approve. The status quo of a dr (only consultant) should remain.
Coming from you, then I am more convinced I wasn't wrong standing with NMA on this one
Sai! Patient's perspective indeed.
I weep for this country. I've followed this strike ish for quite some time, even had a debate with some colleagues.
I read an article online abt the requests of the doctors. Nobody is disputing it. They want increase in hazard allowance to N100,000 per month, and some other things too. That is fine by us. But for NMA to boldly come out, stating it will not condone ''blackmail'' by health workers is so annoying. How does the title of ''Consultant'' result to blackmail? Is it something that has never been done before? Is it strange in other countries? Why are the doctors hell bent on being an obstacle to the progress of other health workers?
Now,back to the issue of consultant, let it be known that there are conditions attatched to the title. Medical scientists, pharmacists, nurses, physiotherapists attain educational level of Phd in their profession.
For example,a medical scientist specializes in haematology in masters and Phd level, obtains his fellows and satisfies all requirements by the MLSCN council. Why should such a fellow with vast knowledge in haematology not be allowed to consult with the patient and the patient's doctor on matters relating to haematological matters? It irks when i read some comments here.
A PHD Haematologist, can he own a patient? if the ans is "no" then, by all means leave him with his PHD the way it is, after all, Pharmacists have been brandishing B.pharm and D.pharm, NMA has never complained over that.

In terms of salary, lets have a rethink. For example,as an intern,a scientist earns N120K, a doctor/house officer earns N170K-N180. Now JOHESU requests for an increase, relative to CONMESS scale....N150/160k. Why NMA wan come get HBP on top the matter? All JOHESU is asking for is a relative increase. Peeps who are shouting abt job the rigors of the medical duty should not undermine what other health workers go through. Being on call in haematology laboratory in a teaching hospital is like being on a rollercoaster!
It is the same time too. So what is wrong abt JOHESU requesting for shift duty/call duty allowance and other special allowance.
As an intern, hazard allowance was a mere N10k. What d hell! We handle body fluids too and get exposed to all the vile things running amok.
The above is not the patient's problem kindly refer to the title of the thread

Those who want privatization of other health sectors are living in a fool's paradise. Who can afford it? Services in the health sector is highly subsidized, but still expensive for certain peeps.
Privatization will only make things worse.
Bros; you are kidding right? there is a Govt private partnership clinic in PH, Amaechi is using them for his free health care, go there and compare it with the shrines you call teaching hospitals.
Ironically, the experts there are still using Nigerian consultants till tomorrow and those guys are very good.
Now, talking about increased cost, yes it will sky rocket but almost all countries in the world operate a form of health insurance, perhaps Nigeria will go down that same route.

I don't see how difficult it will be for Govt to subsidize health care delivery in Nigeria, most states have free health care centers already.

And for the skipping of a thing, why are doctors against skipping when they are doing it? They skip from 10 to 12,we want a skip from 9 to 11. Why dem wan get headache? Lol! *SMH*
finally,on the issue of CMD,i will state here boldly that i have no qualms with doctors being the CMDs. All we ask for is professional courtsey.
The CMD issue have been dealt with and gladly you accept the superior argument of the NMA, while thanking you for your understanding, kindly refer to the thread title when next you want to skip 10 to 1million, it is of no interest to we patients.
