thanks doc! hope hyperthyroidism isn't a big one?? I checked for the symptoms, I'm not fervent with about 60% of them aside the ones you listed tho..

I will be taking the doc's tests on Tuesday,will let you know how it goes. I was told I can only be referred to an endocrinologist..can't just go to him..so invariably, I still have to wait on my 'unresponsive' doc.
The symptoms you'd have will depend on the severity of the condition. Like I said, it's not a diagnosis one makes every other day. Difficult to clinch at the early stages. Worse of all, I could be wrong. But it's the closest possibility. Only the tests would confirm.


You are entitled to a referral to a specialist from your GP. But then, he needs to make an initial assessment and see if the call is worth making.

It's your doctors call. Hopefully it's not an emergency so just hang on.

About the menses thing, clinical manifestations of a condition mostly occur in a spectrum - light flow to scanty flow to no flow - all depends on the severity. However, the slightest occurrence should quicken a high index of suspicion.

You could be having a thyroid problem. Emphasis on "could be". It's not a diagnosis. Only your doctor has the power to make such conclusions. But from what you've told me so far that's the most probable conclusion - hyperthyroidism.

An overactive thyroid will manifest as weight loss despite good or increased appetite, excessive sweating, heat intolerance, menstrual abnormalities (typically your menstruation becomes lighter, less frequent, painless to low menstrual pain, sometimes you don't bleed at all), unusual heart beating, and dry skin (you mentioned the skin of your left palm drying and flaking off).


The diagnosis of hyperthyroidism is quite difficult because it simulates a lot of other conditions. A TSH test is can strengthen the diagnosis or make you look at some other thing.

I think you should try to see an endocrinologist -preferably in a teaching hospital.

I went to a hospital..met with an unresponsive doc! explained my weight loss issue to him , but I was unable to tell him other stuffs as he tends to be impatient.. as if what I had is something 'not to be serious' about!! smh!

I noticed have lost about 5kg of my weight within 14months. I haven't checked my new weight since August last year tho.

Your new weight would be desirable. If you weighed 44kg last year August, let's know how much you weight now. It's the only objective way to answer the question below.

also, I noticed for the past two months.. veins in my arms are visible when I drop my hands downwards.. but if I raise them upwards, they disappear. more like the blood flows downwards when I drop my arms(silly thinking?).
they tend to be a lil swollen at the back of my palm when faced downwards.. and this makes my hands look ugly!

Your veins are becoming prominent because you body fat stores is becoming low. Do you exercise frequently? The veins themselves are not swollen. They become prominent when fat around them is diminishing.

Also, there's something called 'varicose veins". Veins have valves that prevent back flow of blood. When these valves become damaged blood can get engorged in a vein making it look prominent.


What tests did the doctors ask you to do?

Besides the unintentional weight loss, what else have you noticed?

Any slight fever ? Has your appetite changed? What else have you noticed?

Why do IVF doctors transfer do many embryos? Are they irresponsible? The truth is that from the doctor's point of view , this is a success for his clinic. He will proudly add the pregnancy to his clinic statistics as a ‘success”. However, from the patient’s perspective it’s actually been a disaster.

This is because the IVF doctor doesn’t have to provide the pregnancy care or look after preterm babies; and he is blissfully unaware of the adverse consequences of his actions.

Quantity VS Quality
Patients must note that when a clinic is willing to transfer multiple embryos, it’s a sign that it is a mediocre one! They just don’t have sufficient amount of confidence in their lab & have no option but to resort to transferring a larger number of embryos- and this becomes a way of artificially inflating their success rates.

A good clinic is one that can create good quality embryos which have a high implantation rate, so that they don't have to transfer more than 1- 2 embryos to get their patients pregnant !

At times, patients add to the dilemma as they pressurize the doctors to transfer a larger number of embryos and feel that it ups their chances of getting pregnant. Most have also reach breaking point in terms of dealing with their infertility; and even when their IVF doctor educates them about the possibility of multiple births, they just feel it’s better than not getting pregnant at all.

Little do they realize that they are jumping from the frying pan into the fire.



The Multiple Pregnancy Risk
Please remember that transferring more embryos does not increase success rates – it only increases the risk of a multiple pregnancy and this is a classic case of too much of a good thing doing more harm than good. Typically, a good clinic will transfer only 1 embryo (if it's a top-quality blastocyst). However for selected patients ( for e.g., older patients or those who have failed multiple IVF cycles), transferring more embryos may be a justification.

In a good clinic, 50% of top-quality blastocysts will implant successfully. This means their implantation rate is 50%. The reason why the other blastocysts don’t implant is because a lethal genetic problem (which we cannot identify) causes them to stop growing and prevents them from implanting.

This is exactly why it is still not possible for us to predict which blastocyst will become a baby. It’s also why transferring three blastocysts would translate into an extremely high success rate – but would also result in a very high number of multiple pregnancies.


Complete your Family
A good clinic is confident they will have a high pregnancy rate even after transferring just 1 or 2 embryos and will discourage patients from transferring too many of them. What’s even better is that spare embryos aren’t wasted.

The supernumerary embryos can be cryopreserved (embryo freezing) safely, and used later on to complete that patient’s family!

People underestimate the power in the cliché, “beside every successful man, there is a woman”. You know how this works. You also know the useful variations of that cliché, like “beside every successful man is a sensible woman”. Sensible because there are women behind and in front who are never seen. We all know how men are. They can’t make up their minds as to what they want, whether it is the wife, the wife’s friend, the girlfriends, or the housemaid. It is your job to make sure he keeps you in your proper place: by his side, as First Lady.

Never confront him about the other women (or boys) you and half the world know he has. In fact, officially, your husband is a kind, god-fearing man incapable of carnal knowledge against the order of marriage (or of nature). Say this to yourself as often as you can.

Show an early interest in politics. This is what will
separate you from the other women – the ability to
support his career. You do not need plenty degrees for this. You do not even need to know how to speak English well. Wisdom will save you. Go into the field, attend party rallies and do not show signs of sleepiness or irritation. Especially during election campaigns, primaries and elections proper, you will be expected to show stamina, stay up all night campaigning, counting votes, manufacturing votes, or waiting for results that will confirm that God has blessed your husband's hustle.

After crossing those hurdles and winning the elections you need to think of your public image.

The pet project is the burden of the First Lady but you can do it gracefully. It is a blessing in disguise. People underestimate it, ignore it, misunderstand it, even mock it. They do not know the potential. Nobody will ask questions or check your budgets. Nobody will know how much you raise or spend. It is just like your husband’s security vote.

Once in a while, take your press team and do something for the less privileged, orphans, widows, that kind of thing. Show you are not afraid of poor or disabled children. Let the cameras capture you smiling while you touch a cripple, hug a poor person, pick up an orphan or push someone’s wheelchair. This will prove that you really love and care for them. Have them show you dancing with them on Children’s day or Christmas or New Year's day. But don’t sweat it. You also need time for other aspects of national politics. The days when first
ladies were all about pet projects are over.

Remember that no matter who the Party appoints as
women leader, you ARE the de facto women leader, the mummy of the federation. Act like it. Host parties and gala nights for wives of governors, ministers and Party men. Sit at the head of the table.

Be innovative. Take the initiative to touch base with your husband’s supporters. Go and thank them for voting him in. It doesn’t matter if you cause massive hold-ups while you are at it. That way they know you are in town.

Consolidate your position by supporting one or two people to get appointments in your husband's
government. You need an eye there. Flex your muscles once in a while by making someone who has annoyed you in the presidency lose his job. People need to know you are not just a pet project first lady.

However, be careful when you are collecting kickbacks from contractors and those who badly need to see the President for favours. You don’t want your husband to leave his serious job of being President to clean up your scandals. Use reliable people to do your money laundering, not people who will blurt out your name the moment they are caught with huge sums of foreign currency.

Because you are getting older, you need to take care of yourself. Not because you want your husband to
suddenly abandon his girlfriends and start desiring you. That ship has sailed. You can pay people to desire you when you want. It is for the cameras. You need to be the best dressed woman in the room. You do not want some other powerful woman outshining you. You are the mother of the people. Do complete makeovers if you have to (I hear Dubai is great for makeovers). But be careful. With the exception of harmless Botox injections, avoid needles. Avoid knives and tubes and suction pipes.
You don’t want to leave your exalted position for some small girl to inherit.

There are people in the media, half-baked journalists and idle bloggers, who will look at everything you do, your head tie, your children, your grammar, your toenails, the texture of your voice. Unfortunately, this is a democracy and you must endure them. But rest assured that God will judge evil journalists, bloggers and jobless Twitter and Facebook people.

To boost your international image, you need to be seen in glossy magazines with wives of leaders of good countries. Aim high; the American First Lady. My prayer for you is that the Americans don’t elect a woman or gay man as president when it is time for your photo.

Finally, don’t get carried away. Those men who kneel at your feet today do it only because your husband is alive and in power. If that changes, they will disappear faster than the money in our treasury. People will no longer print wrappers with your photo or use your name to sing local songs. Nobody will even call you to events as ex-First Lady.

That future might appear grim, but that is why you must face the serious business of enjoying the
present. God bless you as you do.

I think autoimmune diseases generally are under-diagnosed. First is because most of them have one thing in common-they run a chronic course. Nigerians are more likely to present with acute manifestations of a disease than a check up for an abnormal symptom they noticed.

Also, these conditions are not common compared to the others so the training IN the not fine tuned in these areas, leading to underdiagnosis and misdiagnosis.

Finally, there is a dearth of immunologists in Nigeria both in clinical and non-clinical training. So awareness for the commonest autoimmune conditions which could make people see their butterfly rash as a problem and not a "mere skin condition" is infinitely lacking.

Then finally, doctors are not motivated to train in these areas. The definitive diagnosis for these conditions are largely based on immunologic investigations. They are also required for long term monitoring and prognosis.

How many teaching hospitals can you do those ANA titres and the rest.?

If you shaved recently, it's definitely molluscum contagiosum. See a doctor.

If you see burrows on the itch and it's more severe at night, it's most likely scabies. See a doctor

Either way you'd need a doctor.

Health issues are sensitive. I wonder why people have the time to joke over the predicament of others. If you cannot contribute positively, park well.

One word: allergic reaction. Stop taking alcohol or tram.

Alcohol and tram have one thing in common- they cause euphoria by causing your body to produce excess dopamine. Same occurs with heroin, cocaine and amphetamines.

Just stop the drugs and save your life.
